ABSTRACT
Introducció. La biòpsia del gangli sentinella (BGS) com a estudi oncològic pediàtric no està adoptada sistemàticament. Presentem un cas en què aquesta tècnica sassocia amb lestudi molecular. Observació clínica. Nena de 21 mesos, diagnosticada de melanoma maligne després de la biòpsia duna lesió ulcerada preauricular. Es va fer lampliació dels marges i el recobriment del defecte amb un empelt cutani. Lestudi dextensió va incloure una tomografia per emissió de positrons (PET) i la BGS (el jugular digàstric). A més a més, es va practicar lanàlisi immunohistoquímica i molecular. Els marges de resecció, la PET i les anàlisis histològiques i immunohistoquímiques de la BGS foren negatives. Lestudi molecular mitjançant PCR-RT, amb el GD2 com a marcador, fou positiu al teixit limfàtic. Comentaris. Cal incorporar la BGS com a pràctica habitual en lestudi dextensió del melanoma maligne, ja que ens ofereix més sensibilitat que la PET i disminueix lagressivitat quirúrgica dun buidament ganglionar. Per tal devitar noves cirurgies i la distorsió causada per les prèvies, el moment idoni per fer la BGS és durant el temps de lampliació dels marges. Background. Sentinel lymph node (SLN) biopsy has not been systematically incorporated into the management of pediatric cancers. We present a case of pediatric melanoma in which a SLN biopsy and molecular studies were used. Material and Methods. A 21 month-old girl was diagnosed with malignant melanoma of the face when she presented with an ulcerated hyperpigmented preauricular lesion. A complete surgical resection, which included a skin graft, was performed. Staging studies included a positron emission tomogramphy (PET) and a biopsy of a sentinel yugulodigastric node. Immunohistochemical and molecular studies were performed. Results. The margins of the resected specimen, PETscan, and the immunohistochemical evaluation of the SLN were all negative for malignant melanoma. However, reverse transcriptase PCR using a GD2 probe was positive in the SLN sample. Conclusions. We encourage the incorporation of SLN biopsy into the evaluation of malignant melanoma in children. Its sensitivity is higher than PET scan, and it minimizes surgical interventions. In order to avoid additional surgical procedures, we recommend that the SLN be performed at the time of the original surgery. Finally, molecular methods allow for the detection of patients at risk of recurrence (AU)
