ABSTRACT

The flap of anterior­lateral portion of the sole with inclusion of lateral plantar artery (a. plantaris lateralis digiti quinti­fibular plantar marginal artery) and variable aа. metatarseae plantares of the fourth intermetatarsal gap in conjunction with plasty, using the fifth toe soft tissues (fillet flap), was proposed and applied in clinical practice. The method permits to remove the sole defect, using a supporting flap, owing multiple axial blood supply and sensitive innervation. The flap was applied successfully for the tissues restoration in neurotrophic ulcer of the foot anterior portion.

ABSTRACT

Exodermotension method for preparation to the wound defects plasty, using composite flaps, in which the necessary tension degree on the wound edges is guaranteed without obstacles for visualization (transparent film coating, knotting adapting sutures etc.) and do not exceed maximal threshold level, was improved. In 5­7 min, while absence of coloring changes in the stretched skin and a "white spot" symptom, the wound edges were additionally fixed, using coverage of a great adhesive force for redistribution of stretching forces on a large square of skin, surrounding the wound. Exodermotension method proposed was applied successfully in 7 patients while preparing them to plasty, using composite flaps

ABSTRACT

Experience of a late plasty, using a thin cutaneo-adiposal flaps of abdominal wall of a full-layered wounds of soft tissues of the thumbs and a wrist in 6 injured persons, was presented. Critical postoperative complications and the flaps loss were not observed. Parts of the edge necrosis of the flaps in 3 patients were excised, the wound suturing was done or autodermoplasty, in 1 of them osteosequesterectomy was performed. The reconstruction purpose--restoration of the cover tissues and revascularization of the thumbs and wrists defects--was achieved in all the patients.

ABSTRACT

The method of plasty for the hip region sores, based on transposition of proximal part of m. tensor fascia latae in content of the flap, using her transsection between place of attachment to spina iliaca anterior superior and place of the main vascular pedicle entry into the muscle, was proposed, what permits to prevent vast mobilization of the muscle and to secure existing in normal conditions and formed in pathological conditions anas- tomoses between vascular net of the flap and surrounding tissues. The method proposed was successfully applied for plasty of the hip region sores of degrees III-IV in 2 patients.

ABSTRACT

The improved method "delay", providing preservation of a small bridge of tissues near the flap tip, which are compressed with ligature or the strip of the glove's rubber during 7 - 12 days, was proposed for prevention of critical ischemia in the flap tissues. It permits to avoid the flap tissues critical ischemia up to the moment of the vascular net and blood flow enhancement while a compelled division of the flap in conditions of the damaged tissues or the cases of the flap area bigger than the area of the nutrient vessels basin. The improved method "delay" was applied successfully while the flaps preparation, formatted on temporary and constant nutritive pedicle, to plastic operation in 7 injured persons, suffering trauma of extremities.

ABSTRACT

The method of rotational cutaneo-fascial plasty, applying preservation of perforating arteries in subfascial dissection with mobilization and subsequent rotation of the flaps was improved, guaranteeing preservation of blood flow from multiple perforating arteries in them. The method was successfully applied in 3 patients with a Grade IV sacral bedsores.
