ABSTRACT

A trial of a novel pathogenetic method to prevent recurrent scarring in surgical treatment of patients with adhesive otitis media was made. The test group consisted of 36 patients with adhesive otitis media in whom surgical treatment was combined with application of mithomycin C (0.2 mg/ml) to the wound area. 27 patients of the control group received conventional treatment. The efficacy of the surgical treatment was assessed by reduction of the osteopneumatic interval. The results show that postoperative hearing was more stable in the test group.

ABSTRACT

Acoustic measurements were made of the external and middle ear resonance in health and disease in patients after radical surgery on the ear and in chronic perforative otitis media. It is shown that the absence of the posterior wall of the acoustic meatus deteriorates hearing acuity in the frequency 4000 Hz. Reconstructive surgery of this wall normalizes acoustic resonance of the ear. The value of bone-air interval on pure tone audiogram is different after cavity operation and perforation of the tympanic membrane. Characteristics of acoustic resonance of the ear are decisive for choice of reconstructive surgery.

ABSTRACT

The paper presents the results of examination and treatment of patients with different traumatic injuries of ear structures. The most characteristic injuries were detected in different mechanisms of traumas: mechanical (direct, indirect), altitude-induced, and thermal injuries. Major diagnostic signs of traumatic perilymphatic fistulas were revealed. The efficiency of surgical treatment for different injuries was evaluated. The surgical management of traumatic perilymphatic fistulas was comparatively assessed in different periods following injury. Evidence is provided for that it is expedient to close perilymphatic fistulas in ear injuries as early as possible.

ABSTRACT

The paper presents the results of surgery performed in 75 patients with labyrinthine fistula in otitis media chronica purulenta with cholesteatoma; describes the proposed procedure of selective laser destruction of vestibular receptors using energy of neodymium laser which provides the relief of vertigo without damage to the initial level of hearing; shows its advantages over conventional surgical treatment.

ABSTRACT

Tissue samples obtained intraoperatively from different parts of the middle ear in the course of the radical operation (35 cases) or reconstructive operations in patients with the history of surgical intervention on the ear (67 cases) were examined histologically. Fibrous-atrophic changes in the trepanation cavity coating correlated with the duration of the disease and the time since the radical operation. Pathomorphological and topographic profile of the changes in the operated ear coating copies relevant picture of otitis media purulenta chronica. The tympanomastoid cavity coating was characterized by increasing fibrous-atrophic changes reflecting regeneratory-plastic insufficiency of the epithelium and persistent tympanofibrosis in conditions of open trepanation cavity with disturbed anatomic relations in the middle ear.

ABSTRACT

The authors suggest that Meniere's disease may arise as a psychovegetative syndrome with impairment of labyrinthine homeostasis. It is thought valid to distruct the cupular receptor of the horizontal bony semicircular canal by means of impulse neodymium laser radiation brought to the ampullare bone wall via quartz light guide. Short- and long-term effects of such laser treatment are available. This technique of laser destruction is symptomatic, aimed at relief of severe episodes of vertigo.

ABSTRACT

25 patients with subjective noise in the ears, primarily in neurosensory hypoacusis, were exposed to pulse low-frequency electromagnetic field (PLFEMF) generated by INFITA unit with attached device ELEMAGS. The noise complaints began 1 to 10 years before. The previous conservative therapy failed. PLFEMF eliminated the noise in 2 patients, reduced it noticeably (by 60%) or changed its tone in 19 patients. At 6-12-month follow-up positive effect persisted. The results of the study allow the authors to recommend PLFEMF for wide clinical practice.
