ABSTRACT

Background Diabetes mellitus (DM) presents global challenges, with optimal glycemic control being pivotal in managing complications, notably in type 2 diabetes mellitus (T2DM). Yet, achieving sustained control faces barriers stemming from socioeconomic and gender-specific disparities. This study addresses these gaps by examining socioeconomic determinants and gender disparities in diabetes management, particularly in Maharashtra, India. Methodology This cross-sectional study involved 302 T2DM patients aged 20 to 79 years. Data on sociodemographic, behavioral, and clinical factors were collected through interviews, and records were analyzed via logistic regression to identify predictors of glycemic control. Results Significant associations emerged between gender and education, occupation, and religion. Glycemic control, with a mean HbA1c of 8.45%, remained suboptimal. Logistic regression identified gender, average family income, diabetes duration, treatment nature, comorbidities, complications, and medication adherence as glycemic control predictors. Conclusions Addressing socioeconomic and gender-specific factors is paramount in diabetes management, especially in rural areas where sociocultural influences shape health behaviors. Tailored interventions, including gender-sensitive health education, are vital for improving diabetes care and outcomes. This study provides crucial insights into gender-specific influences on glycemic control among T2DM patients in Maharashtra, advocating for personalized interventions to enhance overall diabetes management.

ABSTRACT

Background Primary health centres are in charge of effectively implementing the COVID-19 vaccination program in rural areas. So, the study was planned to seek insight into the challenges faced by health personnel in the effective implementation of the COVID-19 vaccination program. Methodology The study was conducted in a rural area of Akola district which lies in the western parts of Maharashtra State and belongs to the Vidarbha region and is said to be one of the progressive districts in the region. A qualitative study was planned to understand the barriers and facilitators of the COVID-19 vaccine implementation program at rural and tribal areas. The study participants were medical officers from rural and tribal areas who actively planned and implemented COVID-19 vaccination at the primary health centre. A total of 30 medical officers were interviewed. Interview questions were focussed on the planning of COVID-19 vaccination in their area. Other questions were the problem faced during the implementation of the COVID-19 vaccination program and how it has been tackled. Results The factors identified were grouped into three groups: Health system factors, Human resource factors and Community level factors. Health system factors like shortage of vaccines and syringes, tablet paracetamol, online digital method of vaccination registration, overcrowding at the initial stage, and inadequate infrastructure were barriers to vaccination. Fear about vaccine adverse events, even in healthcare workers (HCWs), and overburdened healthcare workers were also factors affecting vaccination. At the community level, high resistance initially and misconception about the vaccine, and also the fear about post-vaccination side effects have an impact on the COVID-19 vaccination program in rural and tribal areas. Conclusion The successful vaccination rate among the population needs community leadership and a community-centred approach when conducting outreach and strengthening primary health care in terms of infrastructure, manpower, and capacity building of healthcare staff.

Introduction Nephrolithiasis affects all countries of the world with an approximate global lifetime prevalence of 15-20%. In India, 12% of the total population is anticipated to have renal stone disease. This study was aimed at providing a relationship between various dietary factors in the formation of renal stones. Methods A case-control study was conducted among 207 patients (106 cases and 101 controls) attending the outpatient and inpatient departments of a tertiary care hospital in Jodhpur, Rajasthan. All the participants with confirmed renal stones by means of ultrasound and radiographic evaluation, aged 15-65 years were included as cases and were matched on age and gender with controls. Pearson chi-square test followed by binary logistic regression was used to assess significant associations. Results Out of all participants, 71.0% were males and 65.7% were from the age group 41-65 years. The study showed a statistically significant association between renal stones and high salt intake, reduced water intake, less consumption of milk and milk products, daily intake of tea, consumption of oxalate-rich foods and consumption of junk foods. Conclusion Dietary factors play an important role in the risk of the development of renal stones. Simple dietary modifications may significantly reduce the chances of the development of nephrolithiasis, especially in the vulnerable population.

INTRODUCTION: India is one of the countries in the world which contribute to the global burden of multidrug-resistant tuberculosis (MDR-TB). GeneXpert is a method recommended by the World Health Organization (WHO) that uses five overlapping probes (Probe A to E) to detect mutation in the beta subunit of ribonucleic acid (RNA) polymerase gene (Rpoß) responsible for rifampicin resistance in Mycobacterium tuberculosis (MTB). METHOD: All the pulmonary and extrapulmonary samples received in tuberculosis (TB) laboratory for testing of MTB from January 2018 to December 2020 were analyzed for bacillary load and rifampicin resistance was identified by analyzing the number of missing probes. RESULTS: During the study period, a total of 10,021 samples were tested for MDR-TB. Out of those samples, 2674 samples were positive for MTB of which 2321 were pulmonary samples and 353 were extra-pulmonary samples. Rifampicin resistance was detected in 385 pulmonary samples and 63 extrapulmonary samples. These samples were further differentiated according to the bacillary load. The highest number of mutations was observed in Probe E followed by Probe B, Probe A, and Probe D, and the lowest was observed in Probe C. Also, mutations were associated when all probes were present or a few combinations of probes were missing. CONCLUSION: GeneXpert assay is a rapid molecular method that detected MTB and rifampicin resistance with a two-hour turnaround. It uses molecular beacons to detect mutation in the Rpoß gene. This study can be useful in analyzing the prevalence and epidemiology of MTB in a particular demographic area and also the addition of a few more probes can enhance the identification of the mutation in other codons and therefore better therapeutic interventions can be developed accordingly.

Introduction Undernutrition is one of the key determinants of morbidity and mortality in adolescent females worldwide and in India. Malnutrition, particularly undernutrition, is highly prevalent among adolescent females. Although undernutrition affects the health status of adolescent females leading to poor growth and developmental problem issues among them, still, the adolescent group remains to be neglected group. The present paper particularly focuses on challenges and ways forward for improving the nutritional status of adolescent females in Gujarat. Methods and material It's a mixed method study where the secondary data analysis was conducted comparing the National Family Health Survey-5 (NFHS-5) report with the Comprehensive National Nutritional Survey (CNNS) report for the assessment of undernutrition, overweight, and anemia status among adolescent females, and the primary assessment of the nutritional status of adolescent females across Gujarat was conducted through anthropometric measurements of height and weight. Results The NFHS-5 report findings showed total thinness among adolescent females (15-19 years) in Gujarat to be 52.5%, which increased by 3% from the NFHS-4 findings. Anemia among adolescent females has also been reported to be 69%, which also increased by 12.5% from the NFHS-4 findings. Tribal regions/populations had a higher prevalence of undernutrition. Being overweight among urban adolescent females was more prevalent than in rural regions. The key findings of the CNNS report also showed that 24% of adolescent females (10-19 years) were thin while 5% of adolescent females were overweight/obese in India, while in Gujarat, 8% of adolescent females were overweight/obese. The primary data gathered suggest a prevalence of overweight in Gujarat of 8.9% in adolescent females and total thinness of 50%. Conclusion The nutritional status of adolescent females is still a major concern in many parts of India. Considering the complex set of challenges to tackle malnutrition in Gujarat and with specific attention to the adolescent group, it is vital to understand district-specific challenges and plan, program, and design district-specific strategies and implement actions to improve the existing nutritional status of adolescent females.

ABSTRACT

Knee osteoarthritis (OA) is a usual disorder depicted as discomfort and loss of functional performance, including decreased proprioceptive acuity. Symptoms of this condition include stiffness, pain, swelling, joint disproportion, functional incompetency, and muscle atrophy, which may reduce the well-being of the patient. This report highlights the case of a 41-year-old female patient who complained of right knee pain, which was persisting for a month, and was treated with the McKenzie Technique (MT) along with electrotherapy modalities. Following 12 days protocol, there was a substantial reduction in pain, improvement in functional ability and knee muscle strength, and reduced walking time. Various other physiotherapy approaches and techniques were inculcated in the management of OA, which involved a soft tissue approach, therapeutic training, and manual method. Hence, this case report highlights the pragmatic effect of lower limb MT in grade 1 knee OA patients, which may improve the patient's well-being.

Maternal nutrition needs to be addressed during pregnancy for the child's first 1,000 days of life, or roughly between conception and a child's second birthday. The infant requires just breast milk for the first six months of life. The production of breastmilk and its nutritional value is essentially unaffected by maternal privation. The child's health suffers when the mother's diet and health are impaired. This review aims to discuss the importance of pregnant women's nutrition and how it impacts the development and expansion of a child during this critical period of development, which is supported by the most recent literature. Throughout the child's growth in the mother's womb and outside, four distinct stages have been identified: (1) nine months to zero months: pregnancy; (2) zero to six months: breastfeeding; (3) six to 12 months: introduction of solid food; and (4) >12 months: transition to family diet, appreciation of nutritious food offered within each period for the child's development. Moreover, there is a strong link between nutrition, well-being, and learning. The nutritional intake of infants, children, and adolescents maintains the body weight and is sufficient to sustain their normal growth and development. One of the crucial factors influencing a child's development is nutrition. Rapid growth occurs during infancy. Compared to other growth phases, this phase has the largest relative energy and food needs for body size.

Introduction To establish a centralized inventory management system for the efficient functioning of all healthcare facilities, e-Upkaran (equipment management and maintenance system) was launched in 2015 in the state of Rajasthan. This study is conducted to assess the functioning of e-Upkaran in Rajasthan. Methods The assessment of the e-Upkaran system for primary and secondary healthcare centers was carried out using a systematic review of the literature and a multi-indicator stakeholder questionnaire. The benefits evaluation framework focused on the system quality, information and service quality, use and user satisfaction, and net benefits utilized for the assessment. A review of the literature was done to highlight the importance of computerized medical equipment management and maintenance systems and appraise the challenges and benefits associated with such systems as compared to the traditional pen-paper register. Information was gathered based on available documents, field observation, and data obtained from specific hospital staff, including the bioengineers and other users of e-Upkaran. Results The finding of this study suggests that e-Upkaran efficiently improves documentation, reporting, maintenance, and management of medical equipment. It is more efficient than the traditional paper-pen system. It is designed to minimize downtime and maintain equipment in good operating condition and has potential benefits in terms of improving information quality, use, and net benefit. The cost of service ratio is within the benchmark value. This system has also considerably reduced out-of-pocket expenditure. Computer proficiency and the workload of other e-health programs pose a challenge in the implementation of this program. Conclusion The e-Upkaran system is competent in terms of improving information quality, use, and net benefit. Other Indian states could also adopt this system to improve their biomedical equipment management and maintenance system.

ABSTRACT

A prolapsed intervertebral disc (PIVD) refers to herniation of the nucleus pulposus out of the intervertebral space. Mechanical nerve compression brought on by a protruding nucleus pulposus and an increase in chemokines in the affected area causes the underlying pain, which is typically treated conventionally with surgery or medication. Nonetheless, these management strategies may not always be effective and may impact a person's quality of life. This report highlights the case of a 31-year-old male patient, who complained of neck and low back pain associated with tingling sensations for two months, and was treated with Dorn Therapy (DT) which is the basis of pain management intervention in conjunction with spinal traction, spinal mobilization, stretching, muscular strength training, and transcutaneous electrical nerve stimulation. Following a week of pain management techniques, DT significantly reduced pain, improved range of motion, and enhanced muscle strength. This case report highlights the impact of DT on pain associated with a PIVD, which may curtail an individual's well-being.

Background The magnitude of child malnutrition, including severe child malnutrition, is high in India, and Gujarat has a higher prevalence of child malnutrition. Prior studies have employed anthropometric measures to identify the prevalence and associated factors of children's undernutrition. The present study explored community-level determinants of malnutrition among malnourished and well-nourished children in Devbhumi Dwarka district of Gujarat State, India. Methods A qualitative research employing focused ethnographic methodologies was used. In-depth observations of 60 families in a home food environment were carried out. Each child was observed at their respective homes for three consecutive days. Data were analyzed using thematic analysis techniques. Results The study revealed that lack of knowledge on malnutrition, inadequate feeding practices, poor socioeconomic status, insufficient hygiene and sanitation practices, lack of food variety, use of health facilities, and birth complications were the major community-based determinants of malnutrition. Conclusion The study identified community-level determinants of malnutrition among children under five years in the Devbhumi Dwarka district. To tackle the immediate and underlying causes of malnutrition, interventions are urgently needed to create community awareness about malnutrition as a disease and optimal infant and young child feeding (IYCF) practices using behavior change communication strategies.

Improvements in the analysis of child nutrition status can be helpful in increasing the understanding of the magnitude and critical causes of undernutrition. The present study aimed to assess the nutritional status and related factors in children under two years of age in the Devbhumi Dwarka District of Gujarat, India. A descriptive cross-sectional study was conducted for which the sample size was calculated using Open Epi and considering a 20% non-response rate. The sample size for the study was 1200, but the achieved sample size was 1301. Statistical analysis was performed to identify significant determinants of under-nutrition separately for stunting, wasting, and underweight. The prevalence of wasting, underweight, and stunting was 14%, 17%, and 32%, respectively. Prevalence of low birth weight was recorded as 14% in the district. The overall prevalence of overweight was 20% and 6% as per weight for height and weight for age criteria, respectively. The proportion of children with exclusive breastfeeding was observed to be decreasing with increasing age from birth to six months. About 70% of children were exclusively breastfed at the age of six months. Chi-square analyses revealed that parity and spacing are significant determinants of under-nutrition in children under two years in the Devbhumi Dwarka District. A multipronged and convergence approach is needed to combat the menace of child malnutrition.

ABSTRACT

Background Malnutrition among children, adolescent girls and women is a serious public health issue in India. Malnutrition among adolescent girls is likely to carry on this vicious cycle by giving birth to low birthweight babies. Moreover, low birthweight babies who survive are likely to suffer growth retardation and illness throughout their childhood, adolescence and adulthood. Present situation analysis highlights not only the overall nutrition picture of Devbhumi Dwarka but also narrates the uptake of current nutrition and healthcare services in the Devbhumi Dwarka district. Methods A descriptive cross-sectional study was conducted with 3,738 study population (1,301 children, 1,185 pregnant and lactating women and 1,252 adolescent girls) across four blocks of the Devbhumi Dwarka district of Gujarat. Anthropometric assessments were carried out and the WHO Asia Pacific classification was used for the assessment of malnutrition. Results The prevalence of wasting, underweight and stunting was 14%, 17% and 32%, respectively, in children under two years of age. The prevalence of anaemia among pregnant women (Hb <11 g/dL) was 72.92%; 91.36% of lactating women were anaemic (Hb <12 g/dL). The prevalence of underweight (<-2 SD) among adolescent girls was 19.6%. Block-wise variation in prevalence was observed. Overall, utilization of integrated child development services and health services by these target groups remained sub-optimal. Conclusion The study has revealed the suboptimal nutritional status of children, pregnant and lactating women and adolescent girls of Devbhumi Dwarka, which signifies the need for urgent attention. Several areas of priority have been identified and discussed to improve the overall nutrition status in the district.

Introduction Glaucoma is characterized by the loss of retinal nerve fiber tissues and the loss of the neuroretinal rim of the optic nerve head is termed glaucomatous optic neuropathy (GON). The early diagnosis of glaucoma requires measurement of intraocular pressure (IOP) by tonometry. The gold standard method widely used in clinical settings to measure IOP is Goldmann applanation tonometry (GAT). Myopia is also considered a risk factor for glaucoma. Population-based and hospital-based evidence suggests that the prevalence of GON is higher in high myopic eyes than in emmetropic eyes. The present study aims to document the association between myopia and elevated IOP. Materials and methods A total of 400 medium and high myopic patients attending the ophthalmology outpatient department, in a tertiary care hospital, were measured for IOP using GAT and underwent a detailed clinical evaluation. All the patients also underwent slit lamp biomicroscopic examination and fundus examination. Other data included details on demographic, socio-economic, and occupational history. Results A detailed refractive error examination revealed that amongst 400 right eyes, 67.5% had medium myopia, while 66.75% of the left eye were medium myopic. Amongst the medium myopic eyes, the average IOP was 15.51±3.68 mm of Hg, while in high myopic, the average IOP was 16.19±3.33 mm of Hg. A difference of 0.68 mm of Hg with high myopic eyes having higher IOP values than medium myopic eyes was observed, and it was significant statistically with a p-value <0.05. Conclusion There is a statistically significant association between elevated IOP and the high myopic group. In comparison to IOP measured in medium myopic and high myopic patients, elevated IOP was observed in the high myopic group than in the medium myopic group.

ABSTRACT

Introduction Severe acute malnutrition (SAM) carries severe implications for a child's development. The survival of a child suffering from severely acute malnourishment is majorly dependent on the management of SAM, and scaling up the framework for addressing malnutrition is one of its main components. A severely malnourished child has a higher risk of mortality as compared to an ideally nourished child. Bal Poshan Yojana under the aegis of Project Tushti, a public-private partnership (PPP) model, aims at strengthening the framework of treatment for SAM children by working towards building a sustainable eco-system through engaging the government body, private practitioners and implementing bodies. Methods Bal Poshan Yojana is a novel approach implemented for the first time in the Devbhumi Dwarka district of Gujarat for the management of SAM under a PPP model. The private practitioners and centres were empanelled and trained on the treatment of SAM. SAM children with medical complications were screened through anthropometric measurements and appetite tests carried out by Rashtriya Bal Swasthya Karyakram (or RBSK, a program for child health) medical team. High-risk cases were referred to the nearest empanelled Bal Poshan Centre in the area. Children were treated for 14 days at the Bal Poshan Centre as per the protocol of the National Rehabilitation Centre and then discharged if the children fulfilled the criteria. The treatment included a 14-day treatment protocol and three follow-ups in an interval of 15 days each. The data was analyzed with appropriate statistical tests. Results Since its inception, a total of 102 severely malnourished children under five years of age have completed their treatment successfully, including three follow-ups. Around 60.79% of SAM children have been found to become normal in their nutritional status. The mean weight gain upon discharge was 0.57 kg and after three follow-ups, it was 1.051 kg. Conclusion Bal Poshan Yojana is a one of its kind initiative to tackle the growing burden of malnutrition among under-five age group children. The initiative has a focused approach. Strong referral and verification mechanisms ensure complete transparency and yielding of desired results.

AIM: This study aims to study the estrus induction, ovulation, and conception rate of delayed puberty in buffaloes heifers by feeding a herbal plants Aegle marmelos (bael/bili/bhel leaf) and Murraya koenigii (Curry leaf). MATERIALS AND METHODS: Totally, 24 buffalo heifers with delayed puberty were selected for the present study and divided randomly in four equal groups (n=6). Before experiment, all animals were dewormed with albendazole at 10 mg/kg body weight to prevent them from the stress of parasitism. In the present experiment, four group taken and Group I (n=6) treated with A. marmelos, Group II (n=6) treated with M. koenigii, Group III (n=6) treated with mixture of A. marmelos and M. koenigii and fed for 9 days. Group IV (n=6) considered as control and fed with concentrate only. The blood samples were collected from all the animals on day 0 (before treatment), 4, 9 (during treatment), on the day of estrus and day 8 after the onset of estrus. The 10 ml blood was collected from the jugular vein of all the experimental animals for estimation of serum calcium, inorganic phosphorus, and progesterone (P4). The estrus response, ovulation, conception rate along with serum calcium, inorganic phosphorus, and progesterone level were determined by the standard protocol. RESULTS: From Group III 4 heifers, from Group II 3 heifers, and from Group I and IV (Control) 2 heifers each, exhibited the estrus. The estrus response was recorded as 33.33%, 50.00%, 75.00%, and 33.33% in Group I, Group II, Group III, and Group IV, respectively. In treatment Group III, serum calcium found significantly more (p<0.05) on day 8 post-estrus as compared to other groups at a similar interval. Inorganic phosphorus and progesterone show no significant difference between groups. The ovulation and conception rates are comparatively better in Group III (75%) buffalo heifers than other groups. CONCLUSION: Herbal supplementation of A. marmelos and M. koenigii in combination, as well as M. koenigii alone, were found effective in fertility improvement in delayed pubertal buffalo heifers by increasing ovulation and conception rate.

AIM: To compare the seminal attributes of neat, pre-freeze (at equilibration), and post-freeze (24 h after freezing) semen in pure and crossbred Jersey bulls. MATERIALS AND METHODS: Total 36 ejaculates (3 ejaculates from each bull) were collected from 6 pure Jersey and 6 crossbred Jersey bulls and evaluated for various seminal attributes during neat, pre-freeze, and post-freeze semen. RESULTS: The mean (±standard error [SE]) values of neat semen characteristics in pure and crossbred Jersey bulls were recorded such as volume (ml), color, consistency, mass activity (scale: 0-5), and sperm concentration (millions/ml). The extended semen was further investigated at pre-freeze and post-freeze stages and the mean (±SE) values recorded at neat, pre-freeze, and post-freeze semen were compared between pure and crossbred Jersey bulls; sperm motility (80.55±1.70%, 62.77±1.35%, 46.11±1.43% vs. 80.00±1.80%, 65.00±1.66%, 47.22±1.08%), live sperm count (83.63±1.08%, 71.72±1.09%, 58.67±1.02% vs. 80.00±1.08%, 67.91±1.20%, 51.63±0.97%), total abnormal sperm count (8.38±0.32%, 12.30±0.39%, 16.75±0.42% vs. 9.00±0.45%, 12.19±0.48%, 18.11±0.64%), hypo-osmotic swelling (HOS) reacted spermatozoa (71.88±0.77%, 62.05±0.80%, 47.27±1.05% vs. 72.77±1.02%, 62.11±0.89%, 45.94±1.33%), acrosome integrity (89.05±0.83%, 81.33±0.71%, 71.94±0.86% vs. 86.55±0.57%, 78.66±0.42%, 69.38±0.53%), and DNA integrity (99.88±0.07%, 100, 99.66±0.11% vs. 99.94±0.05%, 100, 99.44±0.18%,). The volume, color, consistency, sperm concentration, and initial motility in pure and crossbred Jersey bulls did not differ significantly (p>0.05). The mass activity was significantly (p<0.05) higher in pure Jersey as compare to crossbred Jersey bulls. Live sperm percentage and acrosome integrity was significantly (p<0.01) higher in pure Jersey bulls as compared to crossbred Jersey bulls. However, no statistical difference (p>0.05) was observed in abnormal sperm; HOS reacted spermatozoa and DNA integrity percentage among breeds. CONCLUSION: It may be concluded that the quality of pure Jersey bull semen was comparatively better than the crossbred Jersey bulls.

The blackbuck (Antilope cervicapra) is a small (20-30 kg) Indian antelope that is listed on Schedule I of the Indian Wildlife Protection Act, 1972. Studies were undertaken to develop assisted reproductive technologies, such as synchronisation of oestrus and non-surgical AI, to support the conservation and genetic management of this Indian antelope. Semen characteristics, testosterone levels and the feasibility of short-term cold storage of semen were investigated. Furthermore, different oestrous synchronisation protocols (norgestomet implants and prostaglandin injections) were evaluated for successful AI, defined as the birth of live young. Norgestomet ear implants and i.m. administration of pregnant mare's serum gonadotropin (PMSG) resulted in successful pregnancies in two of five inseminated females, but both had twin pregnancies that were delivered prematurely. In contrast, two injections of prostaglandin 11 days apart were effective in synchronising oestrus in the blackbuck. Transcervical AI in oestrous-synchronised animals 72 and 96 h after the second prostaglandin injection resulted in successful pregnancies in four of six inseminated females (67%) and resulted in the delivery of three live fawns. These studies demonstrate the potential application of AI technology for the conservation of endangered ungulates. To our knowledge, this is the first report regarding the synchronisation of oestrus and successful non-surgical AI in blackbuck.
