ABSTRACT

BACKGROUND: Free radicals lead to inflammation, which in turn could intervene several chronic diseases including cancer. The promising scientific finding for anti-cancer properties of honey is an area of great interest. OBJECTIVE: The present study was designed to investigate the invitro biological effects (cytotoxic, and anti-inflammatory through differential inhibition of metalloproteinases and antioxidant) of unifloral Ajwain honey along with its physicochemical properties (pH, moisture, ash content, electrical conductivity, color, protein). MATERIALS AND METHODS: Three Ajwain honey samples (AJ-1, AJ-2, and AJ-3) were collected from different geographical origins of Western Ghats of India. Melissopalynological analysis was carried out to confirm uniflorality. Physicochemical analysis for ash, moisture content, pH, electrical conductivity, color, and total protein was estimated. Total polyphenol, total flavonoid content, and ferric reducing ability of plasma assay were determined using appropriate methods. The cytotoxic effect was assessed against breast cancer cells (MDA-MB-231) by 3-(4,5-Dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assay and the anti-inflammatory activity was evaluated by gelatin zymography of matrix metalloproteinases MMP-2 and MMP-9 proteins. RESULTS: Melissopalynological analysis confirmed pollens from Ajwain plant's and so-called Ajwain honey. MTT assay depicted inhibitory trend for all honey samples across the concentrations (6.25-100 mg/ml) as compared to untreated cells. Gelatin zymography of metalloproteinases (MMP-2 and MMP-9) showed inhibitory tendency in all Ajwain honey samples. The AJ-3 honey sample had the highest inhibition at 0.625%. A significant correlation between honey color, pH, and protein content was perceived throughout the study. CONCLUSION: This study highlights the invitro biological evidence for possible therapeutic application of Ajwain honey samples in cytotoxic, anti-inflammatory, and antioxidant management as well as can be considered a potent source of supplements in human nutrition.

ABSTRACT

Chronic myelogenous leukemia (CML) blast crisis is an ominous clinical event that is challenging to treat. This can develop at extramedullary sites rarely and is defined as the infiltration of blasts outside the bone marrow irrespective of proliferation of blasts within the bone marrow. We aim to report an unusual clinical presentation characterized by Horner's syndrome, ipsilateral arm weakness, and cervical lymphadenopathy as the first signs of extramedullary blast crisis in a CML patient. To the best of our knowledge, the extramedullary locations involving the brachial plexus along with cervicothoracic paraspinal chloroma have not been previously reported in the literature.

BACKGROUND: Patients with advanced non-small cell lung cancer (NSCLC) have no curative treatment options; therefore, improving their quality of life (QOL) is an important goal. Gefitinib, an epidermal growth factor receptor (EGFR) inhibitor, is a safe oral agent that may be of benefit to a specific population of NSCLC. PATIENTS AND METHODS: A Phase II clinical trial included chemonaïve patients with advanced NSCLC and poor performance status (PS). Response rate, progression-free survival, overall survival, QOL using the Functional Assessment of Cancer Therapy - Lung (FACT-L) questionnaire, and Trial Outcome Index (TOI) were evaluated. RESULTS: Twelve out of 19 enrolled patients were evaluable. The median age for the evaluable patients was 68.8 years (59.7-74.6). Out of all the patients, 7 (58.3%) had adenocarcinoma and 5 (41.7%) had squamous cell carcinoma. The median duration of treatment was 62.5 days (26.5-115.0) in the evaluable patients. Grade 3/4 toxicities included fatigue, rash, diarrhea, and nausea. One patient had partial response, eight patients had stable disease (SD), and three patients progressed. The median overall survival for the evaluable population was 4.9 months (2.3-16). The median progression-free survival was 3.7 months (1.9-6.6). TOI was marginally associated with the overall survival, with a hazard ratio of 0.92 (95% confidence interval: 0.84, 1.0) (P = 0.061). FACT-L score and the TOI were highly correlated (r = 0.96, P < 0.0001). TOI scores were higher in African Americans compared to Caucasians and increased with age. CONCLUSION: Our results suggest that gefitinib use in patients with NSCLC and poor PS may improve the QOL of older patients and African American patients.

ABSTRACT

The relationship between anti-TNF therapy and development of lymphoma in Rheumatoid arthritis (RA) patients is controversial. However lymphomas of unusual types and sites have been reported among RA patients receiving anti-TNF therapy. Primary lymphoma of the breast is a rare entity and has never been reported to occur among RA patients taking anti-TNF therapy. This is the first case of primary lymphoma of the breast reported among RA patients on anti-TNF therapy. Rheumatoid arthritis patients currently on anti-TNF therapy should undergo routine gynecological examination.

The present study was designed to evaluate the lipid peroxidation and non-enzymatic antioxidants in pregnancyinduced hypertension (PIH) compared with healthy normal pregnant and non-pregnant as controls. 75 cases were studied of which 25 were normal healthy controls (Group A), 25 healthy pregnant women (Group B) of third trimester and 25 were PIH (Group C) of the same trimester. In PIH, malondialdehyde a lipid peroxidation product was significantly increased as compared to normal pregnant and non-pregnant controls. The nonenzymatic antioxidants like reduced glutathione, Vitamin-E, Vitamin-C and Vitamin-A were significantly decreased in normal pregnants as compared to controls. A further significant decrease was noticed in PIH compared to normal pregnants. A significant negative correlation was detected between lipid peroxidation and non-enzymatic antioxidant levels. Our study clearly indicates a relationship between elevated lipid peroxidation, decreased non-enzymatic antioxidant in PIH. The measurement of non-enzymatic antioxidants in plasma may be useful predictor of the likely development of PIH.
