ABSTRACT

Incisional hernia (IH) is a postoperative defect of the abdominal wall through which the contents of the peritoneal cavity are externalized beneath the skin in a peritoneal sac. IH differs in anatomic complexity, but also in its associated comorbidities and surgical history. As IH enlarges, complications occur and these become part of its natural history. The goal of the study is to review the impact of loss of domain upon abdominal wall before and after abdominal wall reconstruction. The absence of anatomical and functional linea alba leads to a combination of muscular disturbances, chronic respiratory and gastrointestinal conditions, and psychosocial issues. The pathophysiological changes are also due to the decrease of the intra-abdominal pressure (IAP). During repair, the sudden reintroduction of the viscera into an unprepared cavity leads to a sudden increase in cavity volume and an increase in IAP with fatal pathophysiological implications. For an optimal repair, preoperatively, the abdominal wall must be augmented by achieving the following objectives: reducing the volume of the sac contents, optimizing compliance, enlargement of the container. At the same time, for the optimal repair, the following must be taken into account: increased knowledge about this condition to manage systemic and local changes, CT scan evaluation, monitoring IAP, plateau pressure (PP), and Positive End Expiratory Pressure (PEEP). In conclusion, the goals can be achieved by systemic optimization with a multidisciplinary team, using Preoperative Progressive Pneumoperitoneum (PPP) and/or Botox (BTX), and abdominal wall reconstruction through a mesh with augmented component separation technique.

ABSTRACT

Currently there is not a single trusted infrastructure used for the exchange and storage of medical data along the healthcare value chain and, thus, there is no platform used for monitoring patients' traceability within the entire healthcare chain. This situation leads to difficult communication and increased procedural costs, and thus it limits healthcare players from developing a better understanding and know-how of patients' traceability that could further boost innovation and development of the best-fitted health services. PatientDataChain blockchain-based technology is a novel approach, based on a decentralized healthcare infrastructure that incorporates a trust layer in the healthcare value chain. Our aim was to provide an integrated vision based on interoperability principles, that relies on the usage of specific sensors from various wearable devices, allowing us to collect specific data from patients' medical records. Interconnecting different healthcare providers, the collected data is integrated into a unitary personal health records (PHR) system, where the patient is the owner of his/her data. The decentralized nature of PatientDataChain, based on blockchain technology, leveraged the proper context to create a novel and improved data-sharing and exchange system, which is secure, flexible, and reliable. This approach brings increased benefits to data confidentiality and privacy, while providing secure access to patient medical records. This paper presents the design, implementation, and experimental validation of our proposed system, called PatientDataChain. The original contributions of our paper include the definition of the concept of unifying the entire healthcare value chain, the design of the architectural model of the system, the development of the system components, as well as the validation through a proof of concept (PoC) conducted with a medical clinic from Bucharest, using a dataset of 100 patients and over 1000 transactions. The proof of concept demonstrated the feasibility of the model in integrating the personal health records from heterogeneous sources (healthcare systems and sensors) in a unified, decentralized PHR system, with enhanced data exchange among healthcare players.

ABSTRACT

INTRODUCTION: A pathological fracture appears after a low-energy trauma or minor trauma on bones with a modified histological structure; sometimes, the patient reports shoulder pain antedating the fracture. The most common benign bone tumors that cause pathological fractures in children are simple bone cysts, aneurysmal bone cysts, non-ossifying fibromas and fibrous dysplasia. This type of bone tumors is usually asymptomatic until they reach a large size and cause a pathological fracture after minor trauma. The optimal treatment remains controversial. Our objective was to describe our modern treatment strategies of the large benign osseous tumors of the humerus complicated by pathological humerus fractures and histological aspects in these cases. PATIENTS, MATERIALS AND METHODS: The study was prospective and included patients who were diagnosed with pathological humeral fractures, which required surgically orthopedic treatment. We selected three cases of pathological fractures of humerus in children each with its particularities, treated in the Department of Pediatric Surgery and Orthopedics, Emergency County Hospital, Arad, Romania. Treatment included curettage of the cyst, sampling for histopathological (HP) examination, bone substitution and titanium elastic nails (TEN) osteosynthesis. RESULTS: No complications and no recurrence were seen in the early postoperative period. CONCLUSIONS: Osteosynthesis with TEN and bone substitution is a viable option for treatment of pathological fracture of humerus, secondary to the osseous benign tumors, which required surgically treatment, despite the different HP aspects. By using a combined treatment in these cases, we eliminate the disadvantages of isolated use of the described techniques in the literature.

ABSTRACT

Distinguishing between benign and malign adenopathies remains a challenge and could represent a source of error in a diagnosis. We report a case of right laterocervical adenopathy in a 17-year-old teenager admitted to hospital with an episode of fever associated with dysphagia, congested pharynx, and pultaceous deposits. Initially the adenopathy was considered to be secondary to a coinfection with Streptococcus B-hemolytic and Epstein-Barr virus, as suggested by the positive bacteriological and serological tests. The onset of the adenopathy before the episode and the ultrasound modifications raised the suspicion of a malignancy, later confirmed by the histopathologic examination of the lymph node excision. The final diagnosis was nodal metastasis of an undifferentiated lymphoepithelial carcinoma with an ENT starting point. Currently, the adolescent is hospitalised in the ENT ward, where the pharynx carcinoma with nodal metastasis was confirmed. Sometimes the infectious context can mask or unmask a malign chronic disease with insidious evolution.
