ABSTRACT

Aim: Tuberous sclerosis complex (TSC) is a rare disease that produces multisystemic disorders. Everolimus (EVR) is the only immunosuppressive drug approved to control the symptoms and progression of the disease. The aim was to evaluate the genotype-phenotype association to improve the pediatric TSC pharmacotherapeutic outcome. Patients & methods: Ten pediatric TSC patients were recruited. Concomitant treatment and main metabolic enzymes and transporter coding gene variants of EVR were analyzed. Results: Significant associations were found between CYP3A4*22 allele and concomitant treatment with valproic acid (CYP3A4-inhibitor) with a poor metabolizer phenotype and the presence of pneumonia. Conclusion: This is the first pharmacogenetic study of EVR in pediatric TSC patients. The authors propose to consider concomitant treatment and pharmacogenetics due to their multifactorial status.

ABSTRACT

BACKGROUND: The screening of high-risk populations using dried blood spots (DBS) has allowed the rapid identification of patients with Pompe disease, mostly in Neurology departments. The aim of the study was to determine the prevalence of late-onset Pompe disease (LOPD) among patients not previously diagnosed or tested for this entity despite presenting possible signs or symptoms of the disease in Internal Medicine departments in Spain. METHODS: This epidemiological, observational, cross-sectional, multicenter study included a single cohort of individuals with clinical suspicion of LOPD seen at Internal Medicine departments in Spain. The diagnosis of LOPD was initially established on the basis of the result of DBS. If decreased enzyme acid-alpha-1,4-glucosidase (GAA) activity was detected in DBS, additional confirmatory diagnostic measurements were conducted, including GAA activity in lymphocytes, fibroblasts, or muscle and/or genetic testing. RESULTS: The diagnosis of LOPD was confirmed in 2 out of 322 patients (0.6%). Reasons for suspecting LOPD diagnosis were polymyositis or any type of myopathy of unknown etiology (in one patient), and asymptomatic or pauci-symptomatic hyperCKemia (in the other). The time between symptom onset and LOPD diagnosis was 2.0 and 0.0 years. Both patients were asymptomatic, with no muscle weakness. Additionally, 19.7% of the non-LOPD cases received an alternative diagnosis. CONCLUSIONS: Our study highlights the existence of a hidden population of LOPD patients in Internal Medicine departments who might benefit from early diagnosis and early initiation of potential treatments.

ABSTRACT

Sensory systems preferentially strengthen responses to stimuli based on their reliability at conveying accurate information. While previous reports demonstrate that the brain reweighs cues based on dynamic changes in reliability, how the brain may learn and maintain neural responses to sensory statistics expected to be stable over time is unknown. The barn owl's midbrain features a map of auditory space where neurons compute horizontal sound location from the interaural time difference (ITD). Frequency tuning of midbrain map neurons correlates with the most reliable frequencies for the neurons' preferred ITD (Cazettes et al., 2014). Removal of the facial ruff led to a specific decrease in the reliability of high frequencies from frontal space. To directly test whether permanent changes in ITD reliability drive frequency tuning, midbrain map neurons were recorded from adult owls, with the facial ruff removed during development, and juvenile owls, before facial ruff development. In both groups, frontally tuned neurons were tuned to frequencies lower than in normal adult owls, consistent with the change in ITD reliability. In addition, juvenile owls exhibited more heterogeneous frequency tuning, suggesting normal developmental processes refine tuning to match ITD reliability. These results indicate causality of long-term statistics of spatial cues in the development of midbrain frequency tuning properties, implementing probabilistic coding for sound localization.

ABSTRACT

Common variable immunodeficiency (CVID) constitutes a heterogenic group of primary immunodeficiency disorders with a wide-ranging clinical spectrum. CVID-associated non-infectious morbidity constitutes a major challenge requiring a full understanding of its pathophysiology and its clinical importance and global variability, especially considering the broad clinical, genetic, and regional heterogeneity of CVID disorders. This work aimed to develop a nationwide, multicenter, retrospective study over a 3-year period describing epidemiological, clinical, laboratory, therapeutic, and prognostic features of 250 CVID patients in Spain. The mean diagnostic delay was around 10 years and most patients initially presented with infectious complications followed by non-infectious immune disorders. However, infectious diseases were not the main cause of morbimortality. Non-infectious lung disease was extraordinarily frequent in our registry affecting approximately 60% of the patients. More than one-third of the patients in our cohort showed lymphadenopathies and splenomegaly in their follow-up, and more than 33% presented immune cytopenias, especially Evans' syndrome. Gastrointestinal disease was observed in more than 40% of the patients. Among biopsied organs in our cohort, benign lymphoproliferation was the principal histopathological alteration. Reaching 15.26%, the global prevalence of cancer in our registry was one of the highest reported to date, with non-Hodgkin B lymphoma being the most frequent. These data emphasize the importance of basic and translational research delving into the pathophysiological pathways involved in immune dysregulation and diffuse lymphocytic infiltration. This would reveal new tailored strategies to reduce immune complications, and the associated healthcare burden, and ensure a better quality of life for CVID patients.

ABSTRACT

Seasonally dry forests in Peru are a combination of ecosystems that include three large floristic groups: coastal, inter-Andean and eastern forests. Except for the seasonally dry forests of the northern coastal plains of the country, until recently it was difficult to explore what occurred floristically within each group due to lack of data. However, in the last 20 years various floristic studies and botanical inventories focused on woody plants have managed to fill knowledge gaps in critical areas. With these studies we have generated the DRYFLOR Peru database that to date includes 526 quantitative inventories (lists of species in discrete areas, including records of their abundances) and that allows us to confirm the floristic distinction of the three large groups. Additionally, we were able to clearly recognize two subgroups of seasonally dry coastal forests (on coastal plains and along the Andean piedmont), two inter-Andean subgroups (within the Marañón-Mantaro and Pampas valleys) and three eastern subgroups (within the Huallaga, Tambo and Urubamba valleys). All subgroups have an assemblage of woody plant species that distinguishes and characterizes them in terms of abundance, frequency, species richness, and levels of endemism. Although we can now better describe the floristic heterogeneity of seasonally dry forests in Peru, we have identified important knowledge gaps that require urgent attention: i) we require additional inventory efforts in the eastern forests, ii) we need to resolve the floristic affinities of the forests of the Apurímac valley, iii) our data agree in little more than 75% with the definitions and distribution of dry forests of the recent National Ecosystem Map of Peru, and it will be necessary to review the concept of seasonally dry forests to adequately capture its distribution in this management tool.

ABSTRACT

The COVID-19 pandemic has had an important negative psychological impact on people worldwide, especially nursing professionals who seem to be more vulnerable to the development of psychopathological symptoms. Objective: To analyze relationships between variables from the social and clinical contexts with psychopathological symptoms in nursing professionals from different geographic regions of Brazil during the COVID-19 pandemic. Methods: Cross-sectional study carried out with 1737 nursing professionals from the five regions of Brazil. Data collection was carried out online with a questionnaire made available via Google Forms containing sociodemographic, occupational, and clinical questions, and the Symptom Assessment Scale-40-R, for the assessment of psychopathological symptoms. Differences between mean scores for the severity of psychopathological symptoms were assessed using the Mann-Whitney and Kruskall-Wallis tests. Results: There was a significant difference in the psychoticism domain scores according to Brazilian geographic region, with greater severity among professionals from the North and Northeast regions when compared with those from the South region. Social context variables (gender, age group, and marital status) and clinical variables (psychological and psychiatric follow-up; psychological or emotional support by the institution; family member, friend, neighbor, or co-worker with COVID-19, and death among them; use of psychiatric medication without a medical prescription; and taking steps to take care of their mental health) were significantly related to psychopathological symptoms. Conclusions: The results point to the importance of mental health promotion strategies for professionals through psychological or emotional support, as evidence indicates that this support can be a predictor of reduced psychological distress.

ABSTRACT

Chronic Kidney Disease is progressive and permanent, causing functional losses and psychological interference that culminate under personal exhaustion and imply a change in daily life. The objective of the study was to assess the levels of subjective well-being and self-esteem of people undergoing hemodialysis, and to identify the relationship between these constructs. This was a cross-sectional quantitative study, involving 152 people, in a Nephrology Center. Data were collected using a sociodemographic survey and the Subjective Well-Being and Self-Esteem Scales, respectively. Pearson's Correlation Coefficient was applied to study the association of subjective well-being with self-esteem; for association with sociodemographic variables, the following statistical tests were applied: Student's T test; Analysis of Variance, Spearman's Correlation Coefficient. There was a predominance of males (65.1%), average age 55.3 years old (± 13.6), married (36.2%) with a family income of 1 to 5 minimum wages, and complete high school education (36.9%). In relation to subjective well-being, there was a satisfactory level of positive feelings, and low expression of negative feelings, and a higher level of satisfaction with life. The average self-esteem score was 29.63, indicating satisfactory indices and significant correlations. A strong, significant, and proportional positive correlation was found. The greater the person's subjective well-being, the greater their self-esteem was, and an association with sex, age, marital status, family income, and education level was identified. Family income was related to all dimensions of the constructs.
