ABSTRACT

We retrospectively studied the outcomes of adults with de novo acute myeloid leukemia treated in a reference center in Brazil and analyzed the association with the human development index (HDI) of the United Nations used as a socioeconomic factor. Among 123 patients, 46 (37%) died during induction, 65 (53%) reached complete remission and 45 (37%) received high-dose cytarabine (Hidac) consolidation. The 5-year overall survival and leukemia-free survival (LFS) were 17 and 26%, respectively, for all patients and 36 and 30%, respectively, for those receiving Hidac. In multivariate analysis, an HDI <0.660 was associated with a lower probability to receive Hidac (P = 0.001), a trend for higher mortality in remission induction (P = 0.062) and a decreased LFS (P < 0.0001). However, it was not associated with outcomes for patients receiving Hidac. In conclusion, survival for patients who received Hidac consolidation is satisfactory; however, socioeconomic factors may have selected patients to receive intensive Hidac consolidation.

ABSTRACT

On the assistencial-teacching-learning-activities the demand for prirnary care is of 60 percent for the feminine sex, 65 percent for aged from 20 up to 65 years. One third of the problerns are of organic nature, other one third are psycologic and the remainder one third organic and psycologic, This acknowledgment seems to be necessary for the arrangement of the didatic-pedagogical medi- cal schools projects in order to graduate professionals able to work adequately at the primary care system.


Cerca de 60 por cento dos pacientes que procuram o Centro de Saúde, são do sexo feminino, 70 por cento na faixa entre 20 a 65 anos. Um terço deles apresenta problemas orgânicos, um terço problemas psicológicos e, o restante, associação dos dois. Esse conhecimento deve direcionar os projetos de atenção primária à saúde, formação de recursos humanos e atualização de profissionais que atuam neste nível.

Os principais problemas diagnósticos em 1.000 pacientes adultos que procuraram atenção primária à saúde em Centros de Saúde de (CS) foram, nas mulheres: leucorréia, marginalidade social, falta de planejamento familiar, prolapso uterino, distúrbios da sexualidade; e, nos homens: hipertensão arterial, doença pulmonar obstrutiva crônica, alcoolismo, marginalidade social, doenças sexualmente transmissíveis.


Leucorrhoea, social maginility, familiar planning deficiency, uterus prolapso, sexuality disrurbances on the woman and arte- rial hypertension, chronic obstructive pulmonary disease, alco- holism, social marginility, venereal diseases on the man are the principal diagnostic on the adult population which demand for primary care.

Estas drogas devem constituir o padräo básico de farmácia de um CS que atua em atençäo primária à saúde.
