ABSTRACT

OBJECTIVES: Subgingival instrumentation (SI) with probiotics may be a proposal for the treatment of periodontitis (P), for patients with type 2 diabetes mellitus (T2DM). The Lactobacillus reuteri probiotic as an adjunctive therapy in the treatment of P associated with T2DM was evaluated. MATERIALS AND METHODS: Forty diabetic participants diagnosed with P (stage III and IV, grade B) were randomized into SI + Placebo (n = 20): subgingival instrumentation plus placebo lozenges and SI + Probi (n = 20): subgingival instrumentation plus probiotics. Probing depth (PD), gingival recession (GR), clinical attachment level (CAL), plaque index (PI), bleeding on probing (BoP), and PISA index were performed at baseline and 30, 90, and 180 days. Cytokine concentration in the gingival crevicular fluid, subgingival biofilm sample, and LDL and HDL subfractions were evaluated. RESULTS: In the deep pockets, PD in SI + Probi showed increased values (p = 0.02) compared to SI + Placebo at 90 days. For CAL, SI + Probi showed increased values compared to SI + Placebo, with a significant difference at 30 days (p = 0.03), 90 days (p = 0.02), and 180 days (p = 0.04). At #PD ≥ 7 mm, SI + Probi had a more frequent number of sites (p = 0.03) compared to SI + Placebo only at baseline. For the PISA, SI + Probi showed a significant difference (p = 0.04) compared to SI + Placebo at 90 days. For cytokines, SI + Probi showed higher quantification than SI + Placebo for IL-10 (p < 0.001) at 90 days, IL-12 (p = 0.010) at 90 days, IL-1ß (p = 0.035) at 90 days, and IL-8 (p = 0.003) at baseline. SI + Placebo showed higher quantification of IL-1ß (p = 0.041) compared to SI + Probi only at 30 days. There was a reduction in all microbial complexes. SI + Probi improved LDL size (246.7 nm vs 260.4 nm; p < 0.001), while large HDL subfractions were reduced aft 180 days of treatment (24.0% vs 20.3%; p = 0.022) when compared with SI + Placebo; this response was dependent of probiotics (1.0 mg/dL vs - 6.2 mg/dL; p = 0.002). CONCLUSION: Subgingival instrumentation improved the clinical periodontal parameters in patients with T2DM. The use of L. reuteri probiotics had no additional effects compared with the placebo; however, there was a positive effect on the lipoprotein subfraction. CLINICAL RELEVANCE: Scientific rationale for study: subgingival instrumentation with probiotics may be a proposal for the treatment of periodontitis (P), especially for patients with type 2 diabetes mellitus (T2DM). PRINCIPAL FINDINGS: the use of L. reuteri probiotics had no additional effects compared with the placebo; however, there was a positive effect on the lipoprotein subfraction.  Practical implications: L. reuteri as an adjunct to subgingival instrumentation may have significant therapeutic implications in dyslipidemia.

ABSTRACT

Alternative therapies against pathogens are under intense investigation because of their increasing resistance to antibiotics. Photodynamic therapy (PDT) is one such alternative that has shown promising results. However, for the widespread use of PDT, it is essential to decipher underlying mechanisms, so as to improve PDT's therapeutic applications. Because of this, we have studied biochemical changes in pathogen Pseudomonas aeruginosa, a medically important bacteria that has developed antibiotic resistance, after PDT with curcumin photosensitizer. Results show a drastic decrease in α-helix protein and increased disordered and ß-sheet secondary structure proteins in P. Aeruginosa post-PDT compared to control. Interestingly, these biochemical changes differ from PDT of pathogens Leishmania braziliensis and Leishmania major with photosensitizer methylene blue. This observation underlines the need for extensive studies on PDT of different pathogens to understand mechanisms of action and develop better PDT strategies.

ABSTRACT

A associação de probióticos ao debridamento mecânico pode ser uma proposta de tratamento das doenças periodontais, em especial para pacientes portadores de Diabetes Mellitus tipo 2 (DM2). Avaliou-se os efeitos da administração do probiótico Lactobacillus reuteri como terapia coadjuvante no tratamento da Periodontite (P) associada ao DM2. Um total de 40 participantes diabéticos e diagnosticados com P foram randomizados em Grupo RAR+Placebo (n=20): receberam debridamento mecânico associado ao probiótico e Grupo RAR+Probi (n=20): tratados com debridamento mecânico associado a um placebo. Foram realizadas avaliações de profundidade de sondagem (P.S.), recessão gengival (RG), nível de inserção clínica (NIC), índice de placa (IP), índice de sangramento gengival (IG) e índice PISA no baseline, 30, 90 e 180 dias. Foi realizada dosagem da concentração de citocinas (INF-γ, IL-10, IL-12, IL-13, IL1ß, IL-4, IL-6, IL-8, TNF-α) do fluido crevicular gengival (FCG), no baseline e 180 dias após o tratamento. Informações sobre efeitos adversos do uso de medicamentos e sobre qualidade de vida foram coletadas. Os dados foram obtidos em média e desvio padrão, e analisados pelos testes Fridman/Tukey e MannWhiteny. Considerado a metodologia do presente estudo, os resultados obtidos apontam que o debridamento periodontal promoveu melhora significativa (p<0.05) nos parâmetros clínicos periodontais em ambos os grupos, mas o uso do probiótico não foi eficiente para resultados adicionais quando comparado com o placebo. A terapia periodontal interferiu nos níveis de citocinas do FCG, porém não se pode afirmar que o uso de probiótico apresenta o mesmo efeito(AU)


The association of probiotics with mechanical debridement may be a proposal for the treatment of periodontal diseases, especially for patients with type 2 Diabetes Mellitus (DM2). The effects of the administration of the probiotic Lactobacillus reuteri as an adjunctive therapy in the treatment of Periodontitis (P) associated with DM2 were evaluated. Measurement of the concentration of cytokines (INF-γ, IL-10, IL-12, IL-13, IL-1ß, IL-4, IL-6, IL-8, TNF-α) of the gingival crevicular fluid (FCG), at baseline and 180 days after treatment. Information on adverse effects of medication use and on quality of life was collected. The data were obtained in mean and standard deviation, and analyzed by the Fridman / Tukey and MannWhiteny tests. Considering the methodology of the present study, the results obtained point out that periodontal debridement promoted a significant improvement (p <0.05) in periodontal clinical parameters in both groups, but the use of probiotic was not efficient for additional results when compared with placebo. Periodontal therapy interfered with FCG cytokine levels, but it cannot be said that the use of probiotics has the same effect. Cytokines(AU)

ABSTRACT

Frequently, the clinicians are addressed to decide between the preservation of Grade III furcation molar and the implant replacement, due to the increased access among the population to this therapy over the years and high success rate of the osseointegrated implants. This case series presents clinical and radiographic data collected from 10 patients who underwent 13 root amputations for the treatment of degree Grade III furcation in maxillary molars with follow-up until 16 years. The results showed improvements in probing depth, bleeding on probing, and radiographic aspects. The follow-up time indicates that root amputation is an effective long-term treatment solution, especially when the patient's local, systemic, or financial conditions make it difficult or impossible to implant placement.

ABSTRACT

The objective of the present study was to establish if individuals with Diabetes Mellitus (DM2) and periodontal diseases (gingivitis or periodontitis) presented an increase in the concentration of modified LDL (moLDL) and what is the influence of periodontal treatment on the decrease of moLDL particles with consequent improvement in the parameters of DM2. Twenty-four diabetic patients with periodontitis (Group 1) and twenty-four diabetic patients with gingivitis (Group 2) were followed up for a period of 12 months. Group 1 was treated with periodontal debridement, and Group 2 received supra-gingival scaling and prophylaxis. In both groups, periodontal clinical parameters: probing depth (PD), clinical attachment level (CAL), gingival resection (GR), bleeding on probing index (BOP) and plaque index; inflammatory serum markers (glycemia, A1c, total cholesterol, HDL-cholesterol (HDL-c), LDL-cholesterol (LDL-c), triglycerides and hs-CRP) and oxidized LDL (oxLDL) were measured at baseline, t = 6 and t = 12 months after treatment. Solutions of LDL were analyzed using the nonlinear optical Z-Scan and optical absorption techniques. The periodontal clinical parameters showed significant improvement (p < 0.05) in both Group after 12 months. For both groups, total cholesterol, HDL-c, LDL-c, triglycerides and A1c levels did not show significant reductions after periodontal therapy. hs-CRP levels in Group 1 presented a significant reduction after 12 months. The glycemic rate and the oxLDL concentrations did not show significant differences as a function of time. The optical measurements of LDL solutions revealed an improvement of the LDL-c quality in both groups. Periodontal debridement was able to improve periodontal parameters and the quality of LDL-c in diabetic patients but without changes in the oxLDL concentration in both groups. Considering the clinical relevance, the reduction of infectious and inflammatory sites present in the oral cavity through periodontal therapy may help with the control and prevention of hyperglycemia and precursors of cardiovascular diseases.

ABSTRACT

Specimens of Leptogium collected in ten localities from the State of Rio Grande do Sul were studied. Sixteen species were found of 28 records mentioned to the state, which represents around 57% of the Leptogium diversity known for Rio Grande do Sul. Leptogium exaratum is proposed as a new species. Leptogium atlanticum, L. milligranum and L. vesiculosum are reported for the first time to the state. The lectotype of L. javanicum is reported here and detailed descriptions are provided to the examined specimens. An identification key is showed with all the 28 mentioned species of Leptogium. It is the first published taxonomic key for Leptogium from Rio Grande do Sul, the State with the highest diversity of this genus in Brazil.

ABSTRACT

BACKGROUND: The aim was to evaluate the use of vinegar, hydrogen peroxide and sodium bicarbonate solutions for the disinfection of pacifiers. DESIGN: Different types and concentrations of the substances were screened in planktonic and biofilm for antimicrobial activity on Streptococcus mutans, Staphylococcus pyogenes, Staphylococcus aureus and Escherichia coli. Also, the effect of the most effective substance was tested against polymicrobial biofilms aiming to mimic the multispecies colonization found in a pacifier. RESULTS: Based on the agar diffusion test, minimal inhibitory concentration (MIC) and minimal microbicide concentration assays, 70% apple vinegar (70%AV) and 70% hydrogen peroxide (70%HP) were selected (P < 0.05). All tested solutions were able to significantly reduce viable cells of S. pyogenes, S. mutans and E. coli from the biofilms A statistically significant reduction of viable cells in S. aureus biofilms were observed after treatment with 70%AV in comparison to the other treatments. When compared to other microbial species, it was found that 70%HP was particularly effective against E. coli. The solution of 70%HP was able to reduce the viable cells of Enterobacteriaceae/pseudomonas of a polymicrobial biofilm formed from a pacifier. The other solutions were not effective in relation to control group. CONCLUSION: Solutions of 70% HP showed to be a good alternative for chlorhexidine digluconate for the disinfection of pacifiers.

ABSTRACT

Deficiencies in the alveolar ridge, dental migration and malocclusion are conditions that may hamper restorative treatment and thus must be resolved by taking a multidisciplinary approach. This clinical case report is associated with orthodontics, implantology and prostheses for rehabilitation. A 34-year-old male patient presented himself in a private practice complaining of aesthetics and discomfort when chewing. At the clinical examination, teeth numbers 11, 12, 21, 22 and 25 were absent, with a removable partial denture replacing them. Due to the prolonged use of this prosthesis, wear of the artificial teeth and the extrusion of the anterior inferior teeth were present. A depression was also present in the vestibular region of the upper incisors, which did not provide adequate lip support. The tomographic examination revealed that despite the deficiency of the vestibular-lingual bone volume, the remnant in this region allowed for the implantation of implants. Orthodontic treatment was performed for the intrusion of the inferior elements and the alignment of the arches, with the goal of providing more spaces for rehabilitation. Afterward, the patient underwent surgery to install implants in the 11, 21 and 25 regions, concomitantly with bone regeneration in the vestibular region of the upper incisors. During this time, the patient used a new removable partial denture. After eight months, the prosthetic phase was started. Ten years after the end of treatment, the patient is satisfied with the results and performs maintenance every six months. (AU)


Deficiências no rebordo alveolar, migração dentária e má oclusão são condições que podem dificultar o tratamento restaurador, exigindo uma abordagem multidisciplinar para sua resolução. Este relato de caso clínico associou ortodontia, implantodontia e prótese para a reabilitação. Paciente do sexo masculino, 34 anos de idade, apresentou-se no consultório particular queixando-se da estética e desconforto ao mastigar. Ao exame clínico, apresentava ausência dos elementos 12, 11, 21, 22 e 25, os quais eram substituídos por uma prótese parcial removível. Em decorrência do uso prolongado desta prótese, houve desgastes dos dentes artificiais e extrusão dos inferiores anteriores. Apresentava também uma depressão na região vestibular dos incisivos superiores a qual não proporcionava suporte labial adequado. O exame tomográfico revelou que, apesar da deficiência de volume ósseo no sentido vestíbulo-lingual, o remanescente nesta região permitia a instalação de implantes. Foi realizado tratamento ortodôntico para intrusão dos elementos inferiores e alinhamento das arcadas, visando ganhar espaços para reabilitação. Após, o paciente foi submetido à cirurgia para instalação de implantes na região do 11, 21 e 25, concomitantemente com regeneração óssea na vestibular dos incisivos superiores. Durante este tempo, o paciente usou uma nova prótese parcial removível. Passados 8 meses, a fase protética foi iniciada. Dez anos após o término do tratamento, o paciente mostrase satisfeito com os resultados e realiza manutenção semestralmente.(AU)

ABSTRACT

A Doença Periodontal (DP) e o Diabetes Mellitus tipo 2 (DM2) apresentam a mesma etiopatogênese inflamatória e demonstram uma relação bidirecional, pois DM2 afeta a severidade da DP, e esta pode contribuir para a carga inflamatória total do indivíduo, influenciando a evolução do DM2. O objetivo do presente estudo foi avaliar os fatores de risco para o desenvolvimento de aterosclerose em pacientes portadores de DM2, com e sem periodontite crônica. Foram analisados 48 pacientes, os quais foram divididos em 2 grupos: Teste (pacientes diabéticos com periodontite crônica) e Controle (pacientes diabéticos sem periodontite crônica). O grupo teste foi tratado com debridamento periodontal e o grupo controle recebeu profilaxia supragengival. Ambos os grupos receberam controle de placa a cada 3 meses. No baseline e 6 meses após o tratamento, foi realizada tomada dos parâmetros clínicos periodontais (IP, IG, PS, RG e NIC) e coleta de sangue para avaliação dos marcadores séricos inflamatórios (oxLDL, CT, TG, LDL, HDL, glicose, HbA1c, PCRus, leucócitos e neutrófilos). Os parâmetros periodontais mostraram melhora significativa (p<0,05) no grupo teste, exceto RG. IP e IG também diminuíram significativamente no grupo controle após 6 meses. CT e HbA1c apresentaram taxas significativamente maiores no grupo teste, em comparação com o grupo controle. As taxas de PCR-us e HbA1c diminuíram significativamente no grupo teste após a terapia periodontal. A contagem de leucócitos mostrou uma diminuição relevante no grupo controle durante o tempo do estudo. A terapia periodontal promove melhoras nos parâmetros clínicos periodontais, nos parâmetros inflamatórios e nas taxas de HbA1c em pacientes portadores de DM2 e periodontite, mas não interfere nos níveis séricos de oxLDL(AU)


Periodontal Disease (DP) and Type 2 Diabetes Mellitus (DM2) present the same inflammatory etiopathogenesis and demonstrate a bidirectional relationship, since DM2 affects the severity of PD, and this may contribute to the individual's total inflammatory load, influencing the evolution of DM2. The objective of the present study was to evaluate the risk factors for the development of atherosclerosis in patients with T2DM, with and without chronic periodontitis. We analyzed 48 patients, who were divided into 2 groups: Test (diabetic patients with chronic periodontitis) and Control (diabetic patients without chronic periodontitis). The test group was treated with periodontal debridement and the control group received supragingival prophylaxis. Both groups received plaque control every 3 months. Periodontal clinical parameters (IP, IG, PS, RG and NIC) and collection of blood for evaluation of serum inflammatory markers (oxLDL, CT, TG, LDL, HDL, glucose, HbA1c, hs-CRP, leukocytes and neutrophils). The periodontal parameters showed significant improvement (p <0.05) in the test group, except for RG. IP and IG also decreased significantly in the control group after 6 months. CT and HbA1c presented significantly higher rates in the test group compared to the control group. Rates of Hs-1c and HbA1c decreased significantly in the test group after periodontal therapy. The leukocyte count showed a significant decrease in the control group during the study time. Periodontal therapy promotes improvements in periodontal clinical parameters, inflammatory parameters and HbA1c rates in patients with DM2 and periodontitis, but does not interfere with serum levels of oxLDL(AU)

ABSTRACT

O objetivo deste relato foi apresentar um caso clínico no qual o levantamento do seio maxilar foi realizado em uma região com comunicação bucosinusal prévia decorrente da extração dos elementos 16 e 17. A comunicação bucosinusal foi fechada por um retalho mucoperiosteal no mesmo ato cirúrgico das exodontias. Após seis meses de cicatrização, o levantamento do seio maxilar foi associado ao enxerto ósseo bovino (Bio-Oss) e membrana reabsorvível (Bio-Guide). Depois de mais nove meses de cicatrização, dois implantes foram instalados pela técnica convencional, tendo perfeito travamento. As próteses foram confeccionadas após seis meses de osseointegração. A paciente foi acompanhada trimestralmente no primeiro ano e depois semestralmente. Passados três anos do término do tratamento, a paciente mostra-se satisfeita com os resultados. O protocolo apresentado neste caso para resolução da comunicação bucosinusal pós-extração dentária mostrou-se efetivo. Entretanto, o clínico deve considerar outras alternativas cirúrgicas, já que nem todas as perfurações apresentam as mesmas dimensões.


The objective of this report was to present a clinical case in which the maxillary sinus lifting was performed in a region with previous bucosinusal communication due to the extraction of elements 16 and 17. The bucosinusal communication was closed by a mucoperiosteal flap in the same surgical procedure of the exodontia. After six months of healing, maxillary sinus lift was associated with bovine bone graft (Bio-Oss) and reabsorbable membrane (Bio-Guide). After another nine months of healing, two implants were installed by the conventional technique, having perfect locking. The prostheses were made after six months of osseointegration. The patient was followed quarterly in the first year and then in the semiannual period. Three years after the end of treatment, the patient is satisfied with the results. The protocol presented in this case for resolution of oral buccal communication after dental extraction was effective. However, the clinician should consider other surgical alternatives, since not all perforations have the same dimensions.

ABSTRACT

This study aimed to evaluate the social and academic aspects and factors related to scientific journal that influence and motivate the authors to choose the journal to publish their academic work. Material and Methods: The study was conducted with professors and graduate students of the Institute of Science and Technology of Sao Jose dos Campos, UNESP. Participants responded to an online questionnaire, sentvia email, and the data were tabulated and analyzed. Result: The Thomson Reuters impact factor, followed by Qualis CAPES Index were considered of greater importance at the moment of choosing the journal for 56% of respondents. All respondents considered peer review relevant and 89% pointed out double-blind review as the most important aspect. Indexing and the journal’s quality were considered the most significant factors by most respondents. 98% of respondents have knowledge on both open access journals and Digital Object Identifier (DOI). Conclusion: The assessed academic community demonstrated good knowledgeon the factors inherent to the publication of scientific papers. O objetivo deste trabalho foi avaliar aspectos sociais e acadêmicos e fatores relacionados a revista cientifica que influenciam e motivam os autores na escolha do periódico para publicação de seus trabalhos acadêmicos. Material e Métodos: O estudo foi realizado com docentes e pós-graduandos do Instituto de Ciência e Tecnologia de São José dos Campos, UNESP. Os participantes responderam a um questionário online, enviado via e-mail, e os dados coletados foram tabulados e analisados. Resultados: O fator de impacto Thomson Reuters, seguido pelo Índice Qualis CAPES foram considerados os de maior importância no momento da escolha da revista para 56% dos entrevistados.Todos consideram relevante a revisão por pares e 89% assinalaram a revisão duplo-cego como a mais importante. A indexação e o conceito da revista foram considerados os fatores mais significativos pela maioria dos respondentes. 98% dos participantes responderam ter conhecimento sobre as revistas de acesso aberto e sobre o DOI (Digital Object Identifier). Conclusão: A comunidade acadêmica avaliada demonstrou ter bons conhecimentos sobre os fatores inerentes a publicação de trabalhos científicos. ABSTRACT

A atividade física espontânea na roda de corrida é capaz de interferir no trofismo e na plasticidade neuroquímica e estrutural do tecido nervoso através da ativação neuronal, modificando os níveis endógenos de fatores neurotróficos e de neurotransmissores, bem como o comportamento cognitivo e límbico. Ratos Wistar jovens (21 dias) realizaram atividade física espontânea na roda de corrida por um período de cinco semanas, e ratos sedentários permaneceram em caixas desprovidas da roda pelo mesmo período de tempo. Ao término do período de atividade fisica, os animais foram avaliados no labirinto em T-elevado para análise da aprendizagem e ansiedade, e no campo aberto, para análise do comportamento motor através de feixes de luz infravermelha. Um grupo de animais foi mantido no biotério após término do período de atividade física, para as avaliações comportamentais após 6 meses e 1 ano. Alguns ratos foram decapitados após as cinco semanas de atividade física espontânea e submetidos à análise tecidual, os seus cérebros foram processados para a hibridização in situ, permitindo a visualização e quantificação do sinal do RNAm do fator neurotrófico derivado do cérebro (BDNF), fator neurotrófico derivado da glia (GDNF), da proteína associada ao crescimento 43 (GAP-43), da glutaminase, do neuropeptídeo Y (NPY) e seus receptores Y1 e Y2 em áreas hipocampais, corticais e subcorticais...(AU)
