ABSTRACT

The presented analysis is based on the case reports of 166 patients who had been treated in the A. L. Myasnikov Cardiology Institute, USSR Academy of Medical Sciences, for rheumatic heart diseases (97), postinfarction cardiosclerosis (35), myocardiopathies (24) complicated by Stage III or IIB circulatory insufficiency. Chronic circulatory insufficiency free of complications was the cause of death of only 1/3 of the mortality cases. The rest of the patients' death was considered to be caused by thromboembolism, predominantly to the pulmonary artery system, pneumonias, haemorrhages from acute and subacute gastric ulcers, etc. The factors most frequently complicating the chronic circulatory insufficiency in patients suffering either from heart diseases, cardiosclerosis, or myocardiopathy, were thromboembolisms.

ABSTRACT

An analysis of archives material (1,500 case reports of patients with circulatory insufficiency) revealed that 200 patients (13%) had persistent, refractory circulatory failure. The revealed factors providing for the development of the refractoriness were: relapsing rheumocarditis, pneumonias, pulmonary infarctions in patients with rheumatic heart diseases, extended cicatrical changes in the myocardium of patients with atherosclerotic cardiosclerosis, or the underlying pathological process in cases of idiopathic myocardial lesions. These factors were taken into account, along with individualization of the therapeutic regimens of cardiac glycosides.
