ABSTRACT

Staphylococcus aureus se presenta como un patógeno cada vez más importante, debido al arsenal de factores de virulencia que presenta, sumado a su elevada capacidad de generar resistencia a los antimicrobianos Los objetivos de esta investigación fueron: confirmar la resistencia a meticilina mediante la amplificación del gen mecA y detectar la presencia de los genes que codifican el factor de virulencia leucocidina de Panton Valentine (PVL). Se investigaron estos genes empleando la reacción en cadena de polimerasa (PCR). Todos los aislamientos presentaron el gen mecA, el 50% de estas cepas resultó portador del gen para PVL. El 54,17% de las muestras de pacientes pediátricos, dio positivo para esta leucocidina. El mayor porcentaje de aislamiento se encontró en muestras de piel y tejidos blandos (85,7%).


Staphylococcus aureus is an increasingly important pathogen, due to the arsenal of virulence factors which presents, in addition to its high capacity to generate antimicrobial resistance. The objectives of this research were: confirm methicillin resistance by amplification the mecA gene and the presence of genes that encode Panton Valentine leucocidin (PVL) virulence factor. These genes have been investigated using polymerase chain reaction (PCR). All isolates showed the mecA gene, 50% of these strains were carrying the gene for PVL. 54,17% of the samples from pediatric patients, yield positive for this leukocidin. The highest percentage of isolation was found in samples of skin and soft tissues (85.7%).

One of the world-wide most prevalent sexually transmitted pathogens is Chlamydia trachomatis. Technical limitations in their detection difficult the estimation of it true prevalence. The aim of this study was to investigate C. trachomatis prevalence in symptomatic and asymptomatic patients by two PCR assays. DNA extraction of endocervical swabs from 105 patients were performed and the samples tested by PCR. Positive samples for C. trachomatis were considered if fragments in both PCR reactions were observed. The prevalence rates were 13.7%, 7.4% and 10.4% for the symptomatic, asymptomatic patients and the total population, respectively. Highest prevalence was registered in symptomatic 20 to 30 years old patients. (19.2%) Chlamydia trachomatis prevalence in the population was relatively high and is desirable to apply diagnostics strategies as the reported one in this study to detect the pathogen and offer an adequate and opportune treatment to patients.

ABSTRACT

Chlamydia trachomatis es uno de los patógenos de transmisión sexual predominantes en el mundo. Limitaciones técnicas para su diagnóstico dificultan la estimación de su verdadera prevalencia. El objetivo de este trabajo fue investigar la prevalencia de infecciones por C. trachomatis en pacientes sintomáticas y asintomáticas, a través de dos ensayos de amplificación por RPC. Se hizo extracción de ADN de hisopados endocervicales de 105 pacientes y se analizaron por RPC. Se consideraron positivas para C. trachomatis, aquellas muestras en las cuales se detectaron fragmentos en ambas reacciones de RPC. Las tasas de prevalencia fueron de 13,7 por ciento, 7,4 por ciento y 10,4 por ciento para las pacientes sintomáticas, asintomáticas y la población total, respectivamente. La mayor prevalencia se registró en pacientes sintomáticas del grupo etario de 20 a 30 años (19,2 por ciento). La prevalencia de infecciones por C. trachomatis en la población fue relativamente alta y se debe resaltar la importancia de aplicar estrategias diagnósticas, como la reportada en el presente estudio, que permitan detectar el patógeno y brindar a las pacientes terapias adecuadas y oportunas.

ABSTRACT

Se realizó un estudio retrospectivo y descriptivo con 207 pacientes de todas las edades operados en el Servicio de Cirugía General y Pediátrica en el Hospital Militar de San Cristóbal, a quienes se les practico Herniplastia en la pared abdominal anterior. Durante el periodo de dieciocho meses comprendidos desde mayo 2005 a octubre 2006. Más de la tercera parte de los pacientes tenían entre 16 y 30 años de edad, con predominio del sexo masculino, en una relación 2:1 a excepción de la Crural. El mayor número de cirugía de urgencia se realizó en personas mayores de 76 años. La hernia Umbilical fue el diagnóstico operatorio más frecuente (52.17 por ciento). La Hernia inguinal fue la segunda en frecuencia y constituyo la asociada con mayor relevancia. La Epigástrica predominó ante la Crural. La mortalidad por complicación fue nula. Cerca del 4 por ciento de los pacientes fue intervenido de emergencia. Por lo que concluimos que nuestros resultados en los diferentes tipos de hernias estudiados no escapan de los observados en las estadísticas nacionales e internacionales.

ABSTRACT

Phase I of the Zurich Prometheus Study is a cross-sectional study focusing on an up-to-date serology for HIV and hepatitis B/C and associated risk factors for all clients in four participating clinics offering opiate substitution in Zurich, Switzerland. The mean age of the 603 respondents is 30.7 years (SD = 6.2) and 38% of them are women. Seventy-five per cent of the respondents have a history of injecting drug use (IDU), and over half have injected within the past six months. Laboratory-confirmed seroprevalence for HBV (50%) and HCV (57%) is twice that of HIV (24%). There is an 80% risk reduction for all three viral infections among those starting IDU after 1991--when harm reduction efforts were in full swing--compared to those who began before 1988--before clean needles were widely available. These findings suggest a strongly protective effect of harm reduction measures. But while a stabilization in HIV prevalence at 15% can be seen among drug users who started injecting after 1991, prevalence rates for HBV and HCV still remain several times higher. The prevalence data in this study support data showing continued high incidence rates for HBV and HCV, even among new injectors in the harm reduction era.

ABSTRACT

INTRODUCTION AND OBJECTIVES: Moderate-to-severe hypertension is less prevalent than mild hypertension, but it is responsible for more incidences of complications. Its complex treatment requires several drugs, and is often inadequate. This study assessed the efficacy and safety of nifedipine GITS (oral release osmotic system) as monotherapy, in addition to the effects on left ventricular hypertrophy, after a long term follow-up (one year). PATIENTS AND METHODS: Thirty patients with diastolic blood pressure above 105 mmHg were studied after a short placebo phase. They received nifedipine GITS as monotherapy in a single daily dose of 30 mg; dose titration was made the first three months according to response and until they reached figures equal to or below 95 mmHg. By M-Mode echocardiogram, left ventricular mass index and systolic function were calculated at the end of the placebo phase and at 3, 6, 9 and 12 months. Hematological parameters, lipid profile, electrolytes and liver enzymes were assessed at the same periods. RESULTS: In 70% of the patients the blood pressure reached values of 140-90 mmHg. In 16 patients with adequate M-mode recordings, a 12% reduction in left ventricular mass was observed without modification in systolic function. Five patients were retired: two due to adverse events and three due to different reasons (drop out, evidence for secondary hypertension). There were no changes of clinical significance in the hematological or biochemical parameters and no hypertensive crisis occurred. CONCLUSION: The monotherapy with nifedipine GITS was effective in reducing high blood pressure, induced regression in ventricular hypertrophy and showed good tolerance in one year follow-up.
