ABSTRACT
Bakstim, a new biostimulating preparation obtained from the organs of the immune system of animals, was developed. The impact of Bakstim on the growth and toxigenic function of C. tetani production strain Copenhagen-471 was evaluated. The addition of the preparation to Gluzman commercial medium for obtaining tetanus toxoid led to an increase in the yield of bacterial biomass from 1.9 to 4-fold and an increase in the toxoid production from 2 to 2.8-fold. The optimum concentration of this biostimulant ensuring the maximum yield of tetanus toxin from the production culture was determined (1,000 mg/l). Bakstim will supposedly be used as additive to nutrient media for the production of tetanus toxoid.

Dieulafoy's lesion is one of numerous causes of acute gastrointestinal bleedings. A prospective study of the Dieulafoy lesion were carried out from 1989 till 1995. Under study there were 8 patients. Acute Dieulafoy ulcers are most often localize in the upper third of the stomach and cause massive bleedings. The authors consider urgent laparotomy with gastrotomy, dissection of the ulcer and suturing the vessel to be the operation of choice.

The authors have observed 361 patients with postresection syndromes and 61 patients with postvagotomy disorders. A considerable part of the patients had a combination of different disorders detected mainly by special methods of examination. Individual surgical tactics for recurrent ulcers, evacuatory disorders and dumping-syndrome gave positive results in most of 182 reoperated patients.

A number of functional disorders were revealed in patients with ulcer disease of the stomach and duodenum prior to operation and in the postoperative period. The investigation of them before operation facilitates prognosis of possible postoperative functional disorders (dumping syndrome, post-vagotomy atony of the stomach) and individualization of the surgical tactics.

A considerable diagnostic value of endoscopy for the detection of recurrent ulcers and other postoperative syndromes was shown on the basis of the experience with endoscopies in 21 patients after gastroenterostomies , 281 patients after gastric resection and 96 patients after vagotomies performed for the ulcer disease of the stomach and duodenum. The methods of examining and the endoscopic picture after different operations are found to be of particular character. To study the state of the afferent loop after Bilroth -II resection of the stomach it is expedient to associate endoscopy with rentgenological examination. Endoscopy can be also used for some postoperative complications.

Having analyzed the experience with 417 resections of the stomach, 116 vagotomies, 27 repeated operations in late complications after vagotomies and treatment of 323 patients with postresection syndromes, the authors conclude that the operative treatment should be used mainly in complicated forms of peptic ulcer. Resection of the stomach remains the operation of choice for ulcers localized in the stomach. In duodenal ulcers both resection of the stomach and vagotomy (mainly selective proximal ones) may be used. The kind of operation must be chosen individually. Ineffective vagotomy must be followed by resection of the stomach. Further search for objective functional tests is required for prognosing and assessment of results of surgery.
