ABSTRACT

Introduction: Horizontal cleavage tears (HCT) of the meniscus are treated with fish-mouth closure with sutures using different methods. Using a no. 0 fiber wire and taking bites through both the flaps using a suture passing device and tying multiple knots is a cost-effective technique. A racking hitch knot is an option for all inside repairs of HCT. It needs multiple half hitches for locking the knot. Surgical Technique: We used a modification of the racking hitch knot (HP knot), making it a self-locking knot, so it does not need half hitches. Loop of no. 0 fiber wire is passed across both the flaps of the meniscus and a modified racking hitch knot tied in one goes closing the fish mouth. Conclusion: This modification (HP knot) saves operative time, it is cost-effective and has the distinct advantage of a reduction in the size of the final knot construct.

ABSTRACT

INTRODUCTION: The main objective of this case report is to demonstrate endoscopic calcaneoplasty (ECP) using two lateral portals. CASE REPORT: The patient is middle-aged male aged 40 years, with higher basal metabolic index (BMI) and was diagnosed having a Haglund's bump and was on conservative treatment for a period of over 6 months, with minimal relief. We did ECP with the patient in lateral decubitus position using two lateral portals instead of using one medial and one lateral portal, his American Orthopaedic Foot and Ankle Society score has improved from 64 to 98, and his Fowler-Philip angle is reduced from 71° to 45°. CONCLUSION: ECP using two lateral portals with patient in lateral position is comfortable for the patients with higher BMI and the surgical time can also be reduced. However, we need to perform more number of cases through this technique to conclude to the results. Endoscopic calcaneoplasty, two lateral portals, Haglund's bump.

ABSTRACT

INTRODUCTION: We describe a rare case of a patient with unilateral musculoskeletal manifestation of tuberculosis presented as bursitis of the left shoulder with rice bodies, without coexisting active tuberculosis or tuberculosis in the previous history. CASE REPORT: A 21 year old patient was examined, who complained of pain and swelling in the left shoulder for 2 years. MRI showed a large amount of rice bodies with joint effusion in the left shoulder with intact rotator cuff. The histological examination showed a tuberculosis-specific inflammatory response with giant cells and epithelioid granulomas. Arthroscopic debridement and removal of the loose bodies was done. A brief summary of the literature is given. CONCLUSION: We report a unique case of tuberculous subdeltoid bursitis with rice bodies formation in absence of any other concomitant focus of tuberculous infection, managed with arthroscopic debridement and anti -tuberculous regimen with a long follow up of twelve months.
