ABSTRACT

According to traditional Chinese medicine theory, tongue coatings reflect changes in the body. The goal of this study was to identify a metabolite or a set of metabolites capable of classifying characteristics of traditional Chinese medicine syndromes in erosive gastritis. In this study, we collected tongue coatings of patients with erosive gastritis with damp-heat syndrome (DHS), liver depression and qi stagnation syndrome (LDQSS), and healthy volunteers. Then, we analyzed the differences in metabolic characteristics between the two groups based on metabolomics. We identified 14 potential biomarkers related to the DHS group, and six metabolic pathways were enriched. The differential pathways included pyrimidine metabolism, pantothenate and CoA biosynthesis, citrate cycle (TCA cycle), pyruvate metabolism, glycolysis/gluconeogenesis, and purine metabolism. Similarly, in the LDQSS group, we identified 25 potential biomarkers and 18 metabolic pathways were enriched. The top five pathways were the TCA cycle, sphingolipid metabolism, fatty acid biosynthesis, pantothenate and CoA biosynthesis, and the pentose phosphate pathway. In conclusion, the DHS group and the LDQSS group have different characteristics.

ABSTRACT

Objective:To investigate the diagnostic value of the post-vascular phase of contrast-enhanced ultrasound in the differentiation of hepatocellular carcinoma (HCC).Methods:The contrast-enhanced ultrasound data of 115 patients who were admitted to Tongji Hospital, Tongji Medical College, Huazhong University of Science and Technology for liver space-occupying lesions from September 2020 to September 2021 were retrospectively analyzed. All patients underwent contrast-enhanced ultrasound with perfluorobutane microspheres for injection within 1 week before operation and were confirmed to be HCC by postoperative pathology. According to the pathological results, the lesions were divided into two groups—a well differentiated group and a poorly differentiated group. The time intensity curves of the regions of interest were drawn to obtain the quantitative parameters of arterial phase and the post-vascular phase. The diagnostic performance of each quantitative parameter in differentiating the two groups of lesions was evaluated and the cut-off value was calculated.Results:117 lesions from 115 patients were finally included in the study, including 70 lesions in the well differentiated group and 47 lesions in the poorly differentiated group. There were no significant differences in the initial growth time, peak time, rise time and the absolute value of peak intensity of lesions between the two groups in the arterial phase (all P>0.05). In the post-vascular phase(12 min), the absolute value of enhancement intensity, intensity ratio and intensity difference of the well differentiated group were lower than those in the poorly differentiated group, and the difference was statistically significant (all P<0.01). The sensitivity and specificity of the absolute value of enhancement intensity<56.117 dB, intensity ratio<1.212, intensity difference<9.184 dB to distinguish the HCC lesions as well-differentiated group were 48.9% and 80.0%, 87.2% and 77.1%, 89.4% and 75.7%, respectively. Conclusions:The quantitative analysis of the post-vascular phase of contrast-enhanced ultrasound is helpful to identify the differentiation degree of HCC and has certain clinical application value.

ABSTRACT

We report the first local transmission of the SARS-CoV-2 Delta variant in mainland China. All 167 infections could be traced back to the first index case. Daily sequential PCR testing of the quarantined subjects indicated that the viral loads of Delta infections, when they first become PCR+, were on average [~]1000 times greater compared to A/B lineage infections during initial epidemic wave in China in early 2020, suggesting potentially faster viral replication and greater infectiousness of Delta during early infection. We performed high-quality sequencing on samples from 126 individuals. Reliable epidemiological data meant that, for 111 transmission events, the donor and recipient cases were known. The estimated transmission bottleneck size was 1-3 virions with most minor intra-host single nucleotide variants (iSNVs) failing to transmit to the recipients. However, transmission heterogeneity of SARS-CoV-2 was also observed. The transmission of minor iSNVs resulted in at least 4 of the 30 substitutions identified in the outbreak, highlighting the contribution of intra-host variants to population level viral diversity during rapid spread. Disease control activities, such as the frequency of population testing, quarantine during pre-symptomatic infection, and level of virus genomic surveillance should be adjusted in order to account for the increasing prevalence of the Delta variant worldwide.

ABSTRACT

The dietary exposures to fatty esters of 3- and 2-monochloropropanediol (MCPD) and glycidol were estimated for children aged 2- to 3-year-old from two areas of China using duplicate diet collection method. The 24-h daily duplicate diet samples over three consecutive days were collected from 40 healthy children aged between 26 and 36 months. The analysis of these contaminates in food samples was measured by an indirect method that entails MCPD/glycidol cleavage from their esterified forms for GC-MS analysis. Over 71% of the mixed diet and dairy products samples were found to be contaminated with 3-MCPD and glycidyl esters. The estimated daily exposure to bound 3-MCPD (mean: 0.48-0.49 µg kg-1 bw day-1; P95: 1.00-1.11 µg kg-1 bw day-1) were well below the health guidance values and were considered of low safety concern. The daily exposure to bound 2-MCPD was estimated to be 0.031-0.038 µg kg-1 bw day-1 on average and 0.12-0.14 µg kg-1 bw day-1 for the P95 exposure. However, it was not possible to assess its risk due to the lack of health guidance value of 2-MCPD. The margin of exposure (MOE) estimates for the mean exposure to bound glycidol (0.10-0.12 µg kg-1 bw day-1) were far above 10000 and were considered of low safety concern. However, the margin of exposure estimates for the P95 bound glycidol exposure (0.41-0.45 µg kg-1 bw day-1) were below 10000 and indicated a health concern. Our data indicated that the mixed diet accounted for nearly 76% to 91% of bound MCPD and glycidol exposure. In addition, the follow-on formula was also an important source for the children aged 2-3 years.

ABSTRACT

Objective:To analyze the effect of target-oriented treatment based on nutrition-oriented information software on nutritional standards of adult patients with severe traumatic brain injury (sTBI).Methods:Adult patients with sTBI admitted to the department of emergency intensive care unit (EICU) of Huzhou First People's Hospital were enrolled. Taking the online time of information software as the node on March 1st 2019, the patients who underwent early standardized enteral nutrition (EN) process from March 1st 2018 to February 28th 2019 were taken as the control group. The patients who received nutrition management by the nutritional support management system software for critical patients from March 1st 2019 to February 29th 2020 were used as the experimental group. The software was integrated with critical information system software. The effects of nutritional support in two groups were evaluated, including starting time of EN; total energy supply, total protein supply, energy compliance rate on 7 days and 14 days; the total albumin. And the related indicators of critical illness management were evaluated, including the survival rate of intensive care unit (ICU) at 28 days, duration of invasive mechanical ventilation (IMV), successful rates of weaning from IMV, rapid shallow breath index (RSBI) after spontaneous breathing test (SBT), serum cholinesterase on 7 days and 14 days, etc.Results:Fifty-one patients with sTBI were included in the analysis, 28 in the control group and 23 in the experimental group. There were no significant differences in baseline data between the two groups, such as gender, age, body mass index (BMI), acute physiology and chronic health evaluation Ⅱ (APACHEⅡ) score, sequential organ failure assessment (SOFA) score, nutritional risk score (NUTRIC), etc., which were comparable. Compared with the control group, the starting time of EN in experimental group was significantly earlier (hours: 26.82±8.33 vs. 36.73±12.86, P = 0.046). The total protein supply on 7 days and 14 days [g·kg -1·d -1: 1.87 (1.36, 1.92) vs. 1.02 (0.87, 1.67), 2.63 (1.49, 1.92) vs. 1.23 (0.89, 1.92), both P < 0.05], the total energy supply on 14 days (kJ·kg -1·d -1: 154.26±68.16 vs. 117.99±112.42, P = 0.033), the energy compliance rate on 14 days [80.0% (16/20) vs. 35.7% (10/28), P = 0.002], and the serum cholinesterase on 14 days [U/L: 5 792.5 (4 621.0, 8 131.0) vs. 4 689.7 (3 639.0, 7 892.0), P = 0.048] in experimental group were significantly increased. There were no significant differences in other indicators between the two groups [total energy supply on 7 days (kJ·kg -1·d -1): 91.50±30.50 vs. 92.88±28.16, P = 0.184; energy compliance rate on 7 days: 34.7% (8/23) vs. 21.4% (6/28), P = 0.288; total albumin (g): 97.80±46.29 vs. 114.29±52.68, P = 0.086; 28-day survival rate of ICU: 87.0% vs. 78.6%, P = 0.081; duration of IMV (days): 14.33±7.68 vs. 15.68±6.82, P = 0.074; successful rates of weaning from IMV: 69.6% vs. 67.9%, P = 0.895; RSBI after SBT (breaths·min -1·L -1): 26.84±10.69 vs. 33.68±8.94, P = 0.052; serum cholinesterase on 7 days (U/L): 4 289.7 (2 868.0, 7 291.0) vs. 3 762.2 (2 434.0, 6 892.0), P = 0.078]. Conclusion:The development and clinical application of nutrition support information software is helpful for the standardized implementation of the nutritional support treatment process for adult patients with sTBI, which is worthy of further clinical research and promotion.

ABSTRACT

BackgroundCOVID-19 pandemic is underway. Some COVID-19 cases re-tested positive for SARS-CoV-2 RNA after discharge raising the public concern on their infectivity. Characterization of re-positive cases are urgently needed for designing intervention strategies. MethodsClinical data were obtained through Guangdong COVID-19 surveillance network. Neutralization antibody titre was determined using a microneutralization assay. Potential infectivity of clinical samples was evaluated after the cell inoculation. SARS-CoV-2 RNA was detected using three different RT-PCR kits and multiplex PCR with nanopore sequencing. ResultsAmong 619 discharged COVID-19 cases, 87 were re-tested as SARS-CoV-2 positive in circumstance of social isolation. All re-positive cases had mild or moderate symptoms in initial diagnosis and a younger age distribution (mean, 30.4). Re-positive cases (n=59) exhibited similar neutralization antibodies (NAbs) titre distributions to other COVID-19 cases (n=150) parallel-tested in this study. No infective viral strain could be obtained by culture and none full-length viral genomes could be sequenced for all re-positive cases. ConclusionsRe-positive SARS-CoV-2 was not caused by the secondary infection and was identified in around 14% of discharged cases. A robust Nabs response and a potential virus genome degradation were detected from nearly all re-positive cases suggesting a lower transmission risk, especially through a respiratory route.

ABSTRACT

COVID-19 is caused by the SARS-CoV-2 coronavirus and was first reported in central China in December 2019. Extensive molecular surveillance in Guangdong, Chinas most populous province, during early 2020 resulted in 1,388 reported RNA positive cases from 1.6 million tests. In order to understand the molecular epidemiology and genetic diversity of SARS-CoV-2 in China we generated 53 genomes from infected individuals in Guangdong using a combination of metagenomic sequencing and tiling amplicon approaches. Combined epidemiological and phylogenetic analyses indicate multiple independent introductions to Guangdong, although phylogenetic clustering is uncertain due to low virus genetic variation early in the pandemic. Our results illustrate how the timing, size and duration of putative local transmission chains were constrained by national travel restrictions and by the provinces large-scale intensive surveillance and intervention measures. Despite these successes, COVID-19 surveillance in Guangdong is still required as the number of cases imported from other countries is increasing. Objective@#To analyze liquid milk exposure of thiocyanate among Chinese population and preliminarily assess its health risk.@*Methods@#A total of 2 059 raw milk samples were collected during 2013 and 2014 from 12 Chinese provinces, New Zealand and Netherlands. Farms were chosed according to the main sources of dairy companies, the distribution of farms and the yield of milk. Content of thiocyanate were detected by ion chromatography. Liquid milk consumption data were taken from Chinese beverage and alcoholic beverage consumption survey in 18 cities or counties in 9 provinces, including 16 775 subjects older than 3. A simple distribution model was used to estimate the exposure of thiocyanate from liquid milk. The tolerable daily intake (TDI) of thiocyanate was made 0.08 mg·kg-1·d-1. Then the exposures of different age groups were compared with TDI.@*Results@#Finally, 1 331 samples out of 2 059 were detected to contain thiocyanate. The detection rate was 65%. The average concentration of thiocyanate was 2.11 mg/kg, with a range of 0.10-16.20 mg/kg. The general population's consumption of thiocyanate by drinking liquid milk was 0.001 mg · kg-1 · d-1, which was lower than TDI. The P95 of general population and consumers were 0.009 mg · kg-1 · d-1 and 0.020 mg·kg -1·d-1 respectively, which were also lower than TDI. Mean exposures of population aged 3-6, 7-12, 13-17, 18-59 as well as elderly aged 60 and above were 0.007, 0.003, 0.002, 0.001 and 0.001 mg · kg-1·d-1 respectively, which were all lower than TDI.@*Conclusion@#The results suggested that the health risk of thiocyanate exposure by drinking liquid milk among Chinese population was at a low level. However, milk products for children deserve more concern.

ABSTRACT

AIM: To study the effect of hydrogen sulfide on wound healing of skin ulcer in diabetic rats . METHODS:Male SD rats were randomly divided into 3 groups, including non-diabetic control (NDC) group, untreated diabetic control ( UDC) group, and treated diabetic administration ( TDA) group.Diabetic rats were induced by intraper-itoneal injection of streptozotocin (STZ).After 1 week, wound healing model was prepared by making a round incision ( 2.0 cm in diameter ) on the dorsal skin in full thickness .The rats from TDA group received 2%sodium bisulfide ointment on the skin ulcer wound , and the animals from UDC and NDC groups received control cream .After 21 d of treatment with sodium bisulfide , blood samples were collected for biochemical analysis , including prothrombin time ( PT) , thrombin time (TT), and fibrinogen (FIB) in plasma, as well as the activity of superoxide dismutase (SOD) and the content of malondi-aldehyde ( MDA) in the serum.White blood cells ( WBC) and lymphocytes were also counted .Granulation tissues from the wound were processed for histological examination and Western blot analysis was used to detect heme oxygenase -1 ( HO-1) and tumor necrosis factor α(TNF-α) expression.RESULTS:Compared with UDC group, sodium bisulfide treatment accelerated wound healing of skin ulcer (P<0.01), and increased the activity of SOD in serum (P<0.01) in the diabet-ic rats.The declined number of WBC and lymphocytes , prolonged PT and TT , and decreased FIB levels in rats treated with sodium bisulfied were also confirmed .Pathological section showed that there were inflammatory cell infiltration , and irregu-lar and loose fibril alignment in the granulation tissue of rats from the UDC group , but there were regular fibril alignment and increased angiogenesis in the granulation tissue of rats from the TDA group (P<0.05).Furthermore, sodium bisulfide treatment raised HO-1 protein expression , and decreased TNF-αprotein expression in the diabetic rats .CONCLUSION:Hydrogen sulfide accelerates the wound healing of skin ulcer in the rats with diabetes .The beneficial effect of H 2 S may be associated with formation of granulation , anti-inflammation , and antioxidation .

ABSTRACT

Objective To evaluate the effect of laparoscopic myomectomy. Methods Clinical data of 62 cases of laparoscopic myomectomy were analyzed retrospectively. Results Success was achieved in all the operations.Of the 62 cases,intramural myoma was confirmed in 45 cases,and subserous myoma,17 cases (non-pedicled,10 cases; pedicled,7 cases).A total of 101 myomas were removed,with a diameter of 1~9 cm (mean,6 8 cm).The mean operative time was 25~270 min (mean,99 min),and the intraoperative blood loss,10~400 ml (mean,63 ml).The postoperative hospital stay was 2~8 days (mean,4 7 days) and the overall complication rate was 25 8% (16/62).Follow-up observation in 42 cases for 2~36 months (mean,9 months) found recurrence in 2 cases and full-term pregnancy in 2 out of 10 cases with infertility. Conclusions Laparoscopic myomectomy has advantages of safety,quick recovery,short hospital stay and fewer complications.

Objective To detect the dynamic changes of antibody and cytokines in the serum induced by single vaccine delivery system (SVDS) of biodegradable microspheres of recombinant glutathione-S-transferase from Schistosoma japonicum (rSjGST) in mice. Methods By using the constructed expression plasmid pET28a(+)-SjGST, rSjGST was expressed by E.coli BL21. After purified through the column of resin,the biodegradable microspheres of rSjGST were prepared with PLGA (Poly lactic-co-glycolic acid, PLGA 75/25).Six-week BALB/c female mice were immunized subcutaneously with a single vaccine, and the blood was collected from eyes on the 6th, 9th, 12th, 15 th, 18th, 21th week respectively. The dynamic levels of serum specific antibody IgG, IL-2 and IFN-? were detected. Objective To prepare biodegradable microspheres of Schistosoma japonicum recombinant glutathione-S-transferase. Methods By using the constructed expression plasmid pET28a(+)-SjGST, fusion protein of Schistosoma japonicum recombinant glutathione-S-transferases were expressed in LB by E.coli BL21,rSjGST fusion protein was purified through column of resin,and then the fusion protein concentration was detected; by using an oil-in-oil emulsion-solvent extraction method, the rSjGST biodegradable microspheres were prepared in PLGA (75/25).The properties of microspheres including shape、diameter、diameter’s distribution and span were assayed under microscope. Results The concentration of protein after purification was 8.323 mg/ml, and the microspheres were spherical and showed normal distribution.The diameter was (7.3?1.2) ?m,the span was 0.52,the load of the rSjGST was 7.62%. Conclusion Biodegradable microspheres of Schistosoma japonicum recombinant glutathione-S-transferase are successfully prepared, which provides the basis for further studies on protective immunity of mice.
