ABSTRACT

For the purpose of optimization of antibacterial treatment of urinary tract infection in 700young patients under the condition of North it was examined the responsivity of urinary tract pathogens to antibiotics in 1998-2009. The most effective group of antibiotics is revealed. The most frequent types of urinary tract pathogens are: in men - S. epidermidis, S. saprophyticus, S. aureus; in women with non-complicated UTI - E. coli, S. epidermidis, S. saprophyticus u S. aureus; with complicated - E. coli, S. saprophyticus, S. epidermidis. The abovementioned pathogens showed the low antibiotic resistance (in vitro) to gentamicin and ofloxacin; high antibiotic resistance to cephalosporin (3rd generation) and ciprofloxacin. It is better to prescribe ofloxacin and cefotaxime for patients with complicated UTI; for women with non-complicated UTI - ofloxacin and ceftriaxone. It is necessary to consider nephrotoxicity and low compliance before prescription of gentamicin.

ABSTRACT

With the purpose of improvement of diagnosis of induced gastric ulcer were examined 11 patients who took aggressive agents for simulation of gastric ulcer and 33 patients who took pseudo-aggressive agents. Observables, conduced diagnosis of local chemical burn of mucous coat of stomach during initial 6 days after taking aggressive agents. Stages of ulcerous process, resulting from local chemical burn of mucous coat of stomach, coressponds to real gactric ulcer. Gelatin capsule using as a container for delivery of aggressive agents, melts in stomach in 5-6 minutes after taking. Independent from body position, mucous coat of greater curvature of the stomach is damaged. It is impossible to simulate duodenal bulb ulcer using the gelatine capsule or ball made of breadcrumb. The last method of delivery of aggressive agent can damage the small intestine because of uncontrollability of the place of breaking the ball.

ABSTRACT

Military hospital of Plesetsk Cosmodrome was founded on 20 December 1958. The aims of the hospital were always connected with medical support of the cosmodrome, including emergency situations. On 18 March 1980 a Vostok-2M rocket exploded on its launch pad during a fuelling operation. Experience of medical assistance under conditions of large patient load showed the necessity of constant readiness to medical assistance to patients with combined pathology (burn injury, orthopedic trauma and thermochemical injury of the upper respiratory tract), expediency of compact patient accommodation along with the modern anaesthetic machine and readiness to frequent suction bronchoscopy.

ABSTRACT

16 female patients,aged 28.6 +/- 3.2 years, with recrudescent inveterate cystitis were examined for the purpose of improvement of treatment of chronic cystitis. The examined patients noted relapse of disease after sex intercourse. Spectrum of diagnosed uropathogens in female patients corresponds to the structure of bacterio from urina taken after prostate milking and prostatic fluid of patients' intercourse partners. The given uropathogens also corresponds to the structure of contagium of urinary tract in young men. Connection between urogenital biotope and biotope of prostate as a vessel of persistence infection is related. Recrudescent inveterate cystitis in young women may be result from latent inveterate bacteritic prostatitis in their intercourse partners. Effectiveness of treatment and prophylaxis of recrudescence of inveterate cystitis in young women depends on timely diagnostics, treatment, and prophylaxis of inveterate bacteritic prostatitis in their intercourse partners.

ABSTRACT

Results of hospital check-up and treatment of female soldiers, female members of servicemen's family and retirees of Ministry of Defence of Russian Federation were analyzed for the purpose of improvement of prophylaxis, treatment of urinary tract infections and estimation of factors of military service in the progress of UTI in female soldiers in conditions of North. It was determined that in the structure of UTI in female soldiers aged before 40 prevail acute cystitis and acute pyelonephritis combined with phologistic gynaecological abnormality, aged after 40 - chronic pyelonephritis combined with concomitant urolithiasis and chronic cystitis. Factors of military service influence on the progress of acute UTI and early approach of chronic UTI in female soldiers.

ABSTRACT

For determinination of the significance of asymptomatic bacteriuria (AB) among the conscripts in the North was performed a comparative study of 101 volunteers who had no complaints and urinary tract infection (UTI) and a history of 206 patients treated at the stationary UTI. Diagnostically important AB in soldiers was 3% and corresponded to inflammation of the urethra and prostate. Negative impact of subextreme conditions of service in the development of AB from the soldiers in the 12 months has not been established. AB in young men in 27% of such cases was accompanied by leukocyturia, in 36% with bacteriuria, determined by the overall analysis of urine, which is a comprehensive method of identifying the primary UTI.

ABSTRACT

To investigate results of treatment in patients with acute pyelonephritis (AP) among the population of giving age in the North of Russia for prognostication of the disease and decision on further therapy, we studied 1109 case histories of patients treated for urinary infections (UI) including 794 patients with pyelonephritis (119 with AP). Age of both males and females with AP was younger than of UI patients. AP among female inpatients with UI occurred twice more frequently than in males. Ascending AP was characterized by dysuria, mild lumbar pain, symptoms of general intoxication, leukocyturia, bacteriuria, moderate leukocytosis, elevated leukocytic intoxication index (LII) both in males and females. Hematogenic AP in young males presented with lumbar pain, symptoms of general intoxication, leukocytosis, LII elevation in the absent or mild leukocyturia. The main route of renal infection in population of giving age in the North in AP - ascending. Hematogenic route of infection occurs in young males and is typical for pyodestructive AP.

ABSTRACT

For the purpose of improvement of the effectiveness of the treatment of urinary tract infections in military personnel in condition of High North were studied 505 clinical records of military personnel took the treatment from 1998 till 2009. It is established that the part of urinary tract infections from the urologic pathology in contract military personnel is higher than in draft military personnel. Among urinary tract infections prevails pyelonephritis associated with urolithiasis and abnormal development of kidneys. The main type of contamination under the acute pyelonephritis in military personnel is ascending. For this type dysuria, leukocyturia and bacteriuria are characteristic. For the hematogenous contamination pain in lumbar region, imperceptible leukocyturia, proteinuria are characteristic. Kidney carbuncles are higher in 2.5 times in draft military personnel. The main pathogen is aurococcus. "Gates" of this infection are furunculuses of different localization.

ABSTRACT

The article presents the analyzes of medical histories of 16 patients, treated from suppurative diseases of burgeon (13 carbunculosis, 1 nephrepostasis, 2 abscesses of paranephric cellulose) from 1997 to 2009. Effectuated examinations and treatment of patients with suppurative diseases of burgeon speak to insufficient educating work of the medical service of a military unit in the sphere of prophylaxis if suppurative and catarrhal diseases, what leads to delayed call for medical aid. The second place by level of negative influence of appearance of supparative diseases of urinary organs is taken by conditions of service in northern climate and not full adaptation of service men to these conditions. Changing of food ration also leads to decreasing of immunity and organism general-defense status in conditions of extreme conditions of military service. The article presents indications to operative and conservative treatment of carbunculosis of burgeon.
