ABSTRACT

UNLABELLED: TNF alpha receptors participate in programmed cell death. TNF R2 efficiently assists TNF R7 effects by ligand passing. Structure of TNF alpha receptors influences TNF activity in vivo and the structure of TNF R2 gene may suggest post-transcription modification based on alternative splicing. The aim of the study was to analyse the expression of gene coding TNF alpha receptors R2 and R7 by estimation of mRNA expression of differentiated thyroid carcinomas in comparison to surrounding tissue free from neoplastic infiltration and search for differently spliced TNF alpha R2/R7 isophorms. The study included seven patients with histopathologically confirmed follicular thyroid cancer. Tissue samples removed from tumor region were obtained from the follicular thyroid cancer patients undergoing surgical treatment. The samples were divided into two parts. The first one was routinely examined histopathologically, the second was used for RNA extraction and the number of TNF and its receptors mRNA copies were subsequently quantified. RESULTS: 1) The presence of TNF alpha expression was observed in all examined samples, in contrast to TNF R1 expression; 2) The high level of TNF alpha expression was noted both for typical and sought TNF R2/R7 isoforms and 3) A considerable number of samples displayed higher levels of TNF R2 isoforms than TNF R2/R7 mRNA expression. Genetic disregulation observed in neoplastic disease usually concerns dysfunction of cytokines receptor genes.

ABSTRACT

Authors indicate the usefulness of surgical procedures in treatment of Graves-Basedow disease to obtain the best and permanent therapeutic effects and suggest its important place in modern methods of treatment. They pay attention to difficulties in deciding about the range of thyroid gland tissue resection for preserving the state of hormonal balance in postoperative period. Importance of right surgery techniques, for minimizing the number of postoperative complications is also emphasized.

ABSTRACT

We present a case of 64-year old female patient with rare and severe complications after the operation of recurrent goitre in the form of bilateral paralysis of vocal folds, pulmonary embolism, bronchial tree dysplasia. A good early and late result after an intensive interdisciplinary treatment was achieved.

ABSTRACT

Authors presents two cases of low obturation ileus caused by gallstone which had obstructed sigmoid colon. They pay attention to diagnostic problems and usefulness of sonography and X-ray techniques also in case of biliary ileus. Authors shows difficulties in surgical procedures, prefering enterotomy and evacuation of concrement, without simultaneous internal biliary fistula liquidation, especially in cases of increased risk and in old age patients.

ABSTRACT

The authors present a patient at old age with acute superior caval vein syndrome caused by a large benign goitre. Surgical treatment gave a very good early result and a very good late one, inspite of numerous additional burdens subsequent to biological condition of the operated patient.

ABSTRACT

The authors present a retrospective evaluation of the safety of anaesthetizing patients who are above 80 years of age. The level of safety is raised by canal anaesthesia, complex treatment, proper monitoring and intensified perioperative care. In patients operated in emergency routine the coexistence of heart diseases and diabetes is a disadvantage. The risk is not rising parallel to the age, but is difficult to define and demands individual evaluation.
