ABSTRACT
We report a case of spontaneous dissection of the left main coronary artery in a 32-year-old healthy woman that was unrelated with childbirth or other known risk factors. The clinical presentation was an extensive acute anterior myocardial infarction. She was treated by thrombolysis, but did not meet reperfusion criteria. Coronary angiography revealed dissection of the left main coronary artery that extended into the anterior descending and circumflex coronary arteries. Rescue angioplasty with stenting was performed. The clinical, diagnostic, and therapeutic features of this rare entity are discussed.

Describimos un caso de disección espontánea del tronco coronario izquierdo en una mujer sana de 32 años sin relación con el embarazo ni otros factores desencadenantes. La presentación clínica fue un cuadro de infarto agudo de miocardio anterior extenso. Inicialmente se trató con trombólisis sin obtener criterios de reperfusión. La coronariografía reveló una disección del tronco coronario izquierdo que se extendía a la arteria descendente anterior y circunfleja. Se realizó angioplastia de rescate mediante implantación de stents. Exponemos y discutimos las consideraciones clínicas, diagnósticas y terapéuticas de esta rara afección (AU)
