ABSTRACT

We review patients operated of parotidectomy in our ENT department of Dr. Peset Hospital from Valencia during the period 1975-2001. We shelted an amount of 145 cases. The pathologic distribution between benign and malign tumors was as following: 119 benign tumors (83%) and 26 malign tumors (17%). Here we study benign parotid tumors, which represent 83% of total parotid tumors. We performed suprafacial parotidectomies in 72% and total parotidectomy in 19% of cases. Nodulectomy was realized in 9% of patients in those tumors that arise in the lower part of the parotid gland and usualy they were Warthin tumors. The main complications of surgery were facial nerve dysfunction and Frey syndrome. We didn't have any case of permanent facial nerve dysfunction but transitory facial nerve disfunction occurred in 26% of patients. Frey syndrome appeared in 10% of patients operated of parotidectomy. Benign mixed tumor was the only one benign tumor in recurring and the frequency was 5% of patients.

ABSTRACT

Realizamos un estudio de los pacientes operados de parotidectomía en el Servicio de Otorrinolaringología del Hospital Dr. Peset de Valencia durante los años 1975-2001. Se recogieron un total de 145 casos. La distribución según patologías fue: 119 tumores benignos (83 por ciento) y 26 tumores malignos (17 por ciento). En el presente trabajo estudiamos los tumores benignos, que representan el 83 por ciento de los tumores parotídeos en nuestra casuística. Las técnicas quirúrgicas practicadas fueron la parotidectomía suprafacial (72 por ciento) y la parotidectomía total (19 por ciento). La nodulectomía está indicada en aquellos casos en los que el tumor está localizado en la cola de la parótida por debajo del ángulo de la mandíbula, la realizamos en el 9 por ciento de los casos y generalmente se trataba de cistadenoma papilar linfomatoso (tumor de Warthin). En cuanto a las complicaciones de la cirugía, en las lesiones del nervio facial, no tuvimos ningún caso de parálisis facial permanente en los tumores beningnos, en cambio las lesiones transitorias ocurrieron en el 26 por ciento de los casos. La aparición del síndrome de Frey fue del 10 por ciento en el total de pacientes operados de parotidectomía. El adenoma pleomorfo es el único tumor beningno que recidivó y la frecuencia de aparición de recidivas fue del 5 por ciento (AU)

ABSTRACT

In our milieu, Dr. Peset Hospital, Valencia, we have collected between 1979 and 1998, a total amount of 683 thyroidectomies. Between them 101 carcinomata and 582 bening thyroid pathology. In this work are studied the bening pathology treated surgically. 86.4 percent were women and 13.6% men. Middle age 45 years. By complementary explorations, echography the solid single node by 41.9 percent of cases and solid numerous nodes by 34.7% were the more important findings found. By gammagraphy with greater frequency discovered a cold single node by 44.2% of cases. Anatomopathological types more frequently found were single hyperplastic node (13.3%), multiple hyperplastic nodes (27.6%), spreading hyperplasy (9.1%) and follicular adenoma (37.4%). The aim of this work is the comparative study of findings of complementary explorations with the anatomopathological results gained through the surgical pieces.

ABSTRACT

En nuestro medio del Hospital Dr. PEset de Valencia hemos recogido, en el período comprendido entre 1979-1998 un total de 683 tiroidectomías. En ellas se detectaron 101 carcinomas y 582 patologías benignas de tiroides. En el presente trabajo vamos a estudiar la patología benigna tratada quirurgicamente en nuestro Servicio. De los pacientes con patología benigna el 86,4 por ciento fueron mujeres y el 13,6 por ciento varones, con una edad media de 45 años. En las exploraciones complementarias en la ecografía los principales hallazgos encontrados fue el nódulo sólido único en el 41,9 por ciento y nódulos sólidos múltiples en el 31,7 por ciento. En la gammagrafía el hallazgo observado con mayor frecuencia fue el nódulo frío único en el 44,2 por ciento de los casos. Los tipos anatomopatológicos hallados con mayor frecuencia fueron los siguientes: hiperplasia nodular única (13,3 por ciento), hiperplasia nodular múltiple (27,6 por ciento), hiperplasia difusa (9,1 por ciento) y adenoma folicular (37,4 por ciento). El objetivo del presente trabajo es realizar un estudio estadístico comparando los hallazgos de las exploraciones complementarias (ecografía y gammagrafía) con los resultados anatomopatológicos definitivos de las piezas quirúrgicas (AU)

ABSTRACT

We review patients operated of parotidectomy in our ENT department of Dr. Peset Hospital from Valencia during the period 1975-2001. We sheltered an amount of 147 cases. The pathologic distribution between benign and malign tumors was as following: 121 benign tumors (83%) and 26 malign tumors (17%). Here we study malign parotid tumors, which represent 17% of total parotid tumors. Sensibility and specificity of FNA was 54% and 96.5% respectively. We performed almost with similar frequencies total and suprafacial parotidectomies in 46% and 42% of cases respectively. Total parotidectomy with facial resection is indicated in cases with tumor nerve infiltration and has been done in 8% of cases. The main complications of surgery were facial nerve dysfunction and Frey syndrome. Permanent facial nerve dysfunction and and transitory facial nerve disfunction occurred in 3.8% and 30.8% of patients respectively. Frey syndrome appeared in 10% of patients operated of parotidectomy. Recurrences occurred in 15.4% of patients.

ABSTRACT

Reporting the results of a retrospective study on 101 cases of carcinoma of the thyroid gland, treated in our Service between 1979 and 1998. Analysis of the several anatomopathological variables found, the surgical treatment and also with radioactive isotopes, the complications and the condition evolution. Epidemiologically 82 percent of patients were women and men the 18 percent. Middle age 51 years. 12 percent of patients had familiar antecedents of thyroid pathology. The histological types found were: papillary carcinoma (74.26%), follicular carcinoma (14.85%), Hürthle's cellules carcinoma (3.96%) and anaplastic carcinoma (2.97%). In our Service elective treatment of thyroid gland carcinoma is total thyroidectomy with neck dissection in 18.3 percent of affected. The survival rate amount for 81.3 percent at five years. The aim of this article is to display the therapeutics and evolution of thyroid gland cancer, based in our own 18 years of experience acquired in the Service.

ABSTRACT

We compile our experience about the association between benign thyroid disease and thyroid carcinoma, and perform a review on the literature about this theme. We conclude on the importance of knowing this association, even not statistically significative, when it is detected modifies totally our therapeutic attitude and the vigilance of these patients.

ABSTRACT

We present the results of a retrospective study about 101 cases of thyroid carcinoma treated in our ENT-department during the period 1979-1998. We consider the epidemiologic, diagnostic and therapeutic aspects. Epidemiologically 82% of patients were female and 18% male, with a mean age of 51 year. About 12 percent of patients had familiar antecedents of thyroid pathology. The thyroid pathology affected preferently the right thyroid lobe in 40 percent of cases, the left lobe in 28.9 percent, the isthmus in the 6.7 percent and in 24.4 percent it was bilateral. Hormonal 93.8 percent were in euthyroid state and the 6.2 percent of the whole were tested and show hyperparathyroidism. Complementary examinations showed, in gammagraphy, a cold solitary nodule in 48 percent of patients. The echography more frequently discovered a solitary nodule in 36.7 percent or multiple solid nodules in 34.7 percent. Fine needle biopsy sensitivity was in 85.7 percent. Histologic types encountered were: papillary carcinoma (74.26), follicular carcinoma (14.85), Hürthle cells carcinoma (3.96), medullary carcinoma (3.96) and anaplastic carcinoma (2.97 percent). Choice treatment of thyroid carcinoma, in our Department, was total thyroidectomy.

ABSTRACT

We performed a prospective study over 19 patients to evaluate the efficacy of the Epley's particle repositioning maneuver for treatment of the benign paroxysmal positional vertigo (BPPV), 78.9% were cured through this procedure, so we find this method as very useful for improving the BPPV.

ABSTRACT

Most important complications of thyroid gland surgery are recurrent paralysis (which usually results from section, stretching or devascularization of this neural branch) and parathyroid lesions resulting from unnoticed removal of more than two of these glands or through necrosis because deficient blood supply. We present a study about the complications of thyroid surgery based on the thyroid surgery done in our ENT Service during 1979-1998 term. An amount of 683 cases make the series. The 86% were female and 14% male, with a mean age of 40-50. Surgical techniques realized were: hemithyroidectomy (15.3%) and isthmectomy (2.9%). About 7.4% of patients were reoperated. Stated complications were: local (serohematoma 4.1%, infection 1.2%, postoperative hemorrhage 1.2%) and recurrent paralysis (unilateral transitory 2.6% and unilateral definitive 1.6%; there wasn't any case of bilateral palsy) and superior laryngeal nerve paralysis (1.4% as well as hypoparathyroidism (transitory 22.7% and definitive 5.3%). These complications were most frequent in reoperated cases and in total and subtotal thyroidectomies. In most of these series the incidence of recurrent palsy range between 1.5% and increases more than 9.5% in surgical check and the hypoparathyroidism incidence ranges over 25%.

ABSTRACT

From the age of sixty, vertigo is mainly due to vertebro-basilar insufficiency. It has been described that the association of Dihydroergocristine-Piracetam (D-P) is a useful treatment for vertebro-basilar insufficiency. That is why we have designed a comparative study between D-P an a Placebo, so that to prove if this association can be usefull in the treatment of vertigo occasioned by cerebrovascular insufficiency. Fifty patients complaining of vertigo were included in the study after an untreated term. 19 received a daily capsule of Placebo, and the other 31, treated with D-P, were divided in two groups: 16 patients received a dose of 3 mg Dihydroergocristine + 1.6 g Piracetam every 12 hours per os; and 15 other were treated with 1.5 mg Dihydroergocristine + 0.8 g Piracetam every 8 hours during 90 days. The patients were evaluated at the beginning of the study and 90 days later, with anamnesis and vestibular tests. In the last consultation the patients autoevaluated themselves the effect and the tolerance to the drugs received. In the Placebo group it was observed an improvement or disappearance of vertiginous symptoms in the 68.5% of the cases, while with D-P was 93.7% at the dose of 3 mg Dihydroergocristine + 1.6 g Piracetam each 12 hours and 100% with the dose 1.5 mg Dihydroergocristine + 0.8 g Piracetam each 8 hours. None of the treated patients with D-P worsened their symptoms. We observe a considerable decrease in the number of patients with vegetative symptoms in the group treated with D-P related to the Placebo group, though the symptoms persisted more time in the group treated with D-P that in the Placebo group. The group treated with D-P get a higher percentage of improvements and disappearance of auditive and cervical symptoms that the groups treated with Placebo. In the vestibulo-spinal and cerebellous tests it was observed a better improvement with D-P at the dose of 1.5 mg of Dihydroergocristine + 0.8 g Piracetam each hours compared with the other two groups. It can be concluded that the association D-P is an effective treatment for vertigo, getting also a higher normalization of the vestibular tests than Placebo.

We also present a statistics about the symptomatology, the results of the vestibular tests and the diagnose, in a range of patients complaining of vertigo, aged over 65, studied in our ENT-Department.

ABSTRACT

This paper deals with a retrospective analysis of 162 patients with glottic cancer treated in our ENT-Department using vertical partial laryngectomies (cordectomy and partial surgery with cartilage resection. PSCR). The term of study comprise from 1975 till 1991. In this interval were performed 118 cordectomies and 44 partial laryngectomies including cartilage resection. The parameters considered were: clinical onset, tumor sitting, TNM and pTNM classification, histopathological findings and treatment and survival and recurrence rates at 5 years, as well.

ABSTRACT

A case report of epithelial carcinoma of the inferior lip which spread in cervical lymph nodes previously affected by a chronic lymphocitic leukaemia, is presented. The coexistence of both histological findings in some of the nodes (collision tumor) is remarked. We have made a review of the scanty literature about metastases on second tumors and compared and commented those features.

ABSTRACT

We report a retrospective study about 80 myringoplasties performed on 75 patients. The single occlusive myringoplasty technique was undergone in 33.7 percent of cases, while the tympanomeatal flap procedure was used in 66,2 percent. The grafted material was temporal muscle fascia (63.7%) or tragus perichondrium (36.2%). In 30 percent of instances some retouches were achieved on the ossicular chain. The anatomic success rate accounted for 67.5% and a good functional issues for 59.7% of cases.

ABSTRACT

We present the follow-up and end results of 98 horizontal supraglottic laryngectomies done for treatment of supraglottic carcinomata. Both functional and oncologic outcome as well the complications and 5-year survival of these laryngectomies are considered. We conclude that this surgical technique is a suitable one for the larger part of supraglottic carcinomas.
