ABSTRACT

The outors report of two cases of sucsessfully done laparoscopic cholecystectomy in pregnant women with cholelithiasis. The first patient was pregnant with 6 lunar months and suffering of acute cholecystitis, and the second woman is pregnant in 4-th lunar month whith cholelithiasis. During both postoperative periods--no complications. The pregnancies in both cases ended in therm and healthy mature children was born. We make a brief literature review and present whith the opinion that with strong indications and good prepared and experienced surgents, laparoscopic cholecystectomy is a method of choice.

ABSTRACT

It is know that bone marrow micrometastases are an indicator of poor prognosis in patients with epithelial tumors. At the same time little is known if they reflect minimal residual disease or cells distribution and marked metastatic potential. We aimed to perform a clinical study, which though with little gathered clinical material has a direction--investigation of bone marrow in patients with colorectal cancer by the time of surgery, carefully rendering an account of staging and performing curative resection. In intraoperatively done bone marrow biopsy with following investigation for cytoceratin 19 positive cells we found ol presence of metastatic cells in some the studied patients. Finding micrometastases in patients with colorectal cancer is a marker for more agressive course of the disease and for the need to change some of recent methods of treatment.

ABSTRACT

During the last 10 years we have treated 23 patients with acute mediastinitis. The patients' age, sex, medical status, causes of the infection, bacteriology, chest X-ray and computed tomography scan findings, surgical intervention, complications, survival, and other clinical parameters were reviewed.

ABSTRACT

The quick diagnosis is decisive for the outcome of the treatment of patients with esophageal perforation because of the serious complications--mediastinitis, empyema and sepsis. These are the main causes for the high mortality of up to 60 per cent of the patients with esophageal perforation. The difficulties for the particular operative tactics rise from the late diagnosis. These are the most problematic moments of the spontaneous esophageal perforation (Boerhaave's syndrome) management. A review of the clinical signs, the complications, the diagnostics and the successful operative treatment on a patient with Boerhaave's syndrome performed on the 10th day after the incident is presented.

ABSTRACT

Experience with and the impressions of adopting intraoperative abdominal echography in eighteen patients, aged 26 to 73 years, 9 men and 9 women, are shared. The distribution of patients by pattern of diseases is as follows: colorectal carcinoma--4 cases, liver echinococcus--one, gastric carcinoma--one, hepatocellular carcinoma--one, cholangitis acuta purulenta-one, echinococcus of lung and liver--one, calculous cholecystitis with choledocholithiasis--five, and carcinoma of ductus choledochus--one. Fifty intraoperative abdominal echographies and one intrathoracic supradiaphragmatic echography of the liver are performed. Intraoperative abdominal echography of tumors involving organs of the digestive tract contributes to specify the staging of the neoplastic process. In liver echinococcus it determines precisely the number of cysts and diagnoses impalpable cystic formations, while in choledocholithiasis it documents the presence of calculi in the biliary tracts and eventual dilatation of intrahepatic ones. In five patients intraoperative abdominal echographic is done in conjunction with intraoperative fiber choledochoscopy.
