ABSTRACT
The Department of Health's End of Life Care Strategy provided the opportunity to develop effective care, regardless of the setting. However, providing end of life care is challenging for all healthcare practitioners, with each care setting experiencing unique challenges. Within one acute NHS trust, the challenges of identifying and diagnosing dying were identified. An electronic resource tool was developed to aid prognostication and provide a single point of reference to assist healthcare practitioners in improving end of life care for patients in an acute hospital setting.
