ABSTRACT

BACKGROUND: Fibrin glue has shown to be a pioneering remedy to overcome the inconvenience of suturing. Autologous products reduce the risk of contamination and immunological responses compared to commercial fibrin glue. Hence using autologous fibrin glue (AFG) as a tissue sealant in periodontal flap closure would be of added value for a predictable outcome. OBJECTIVE: To evaluate and compare the efficacy of AFG with 4-0 silk sutures in periodontal flap closure and healing. MATERIAL AND METHODS: Total of 17 patients diagnosed with generalized periodontitis in the age group of 18 to 60 years with probing pocket depth of ≥ 6 mm and clinical attachment level of ≥5 mm in at least two quadrants were categorized into test group (AFG) and control group (4-0 silk sutures). Primary outcome measures [modified gingival index (MGI), early wound index (EHI) and interleukin 1 beta (IL-1ß) and secondary outcome measures (oral hygiene index, plaque index and visual analog scale (VAS) were recorded at baseline, 1, 2, 3 and 4 weeks. RESULTS: A statistically significant reduction of IL-1ß value (p =.005); MGI (p = .02); EHI (p = .004), VAS scores (p < .001) and a positive Spearman's correlation (rho = 0.15) was seen between EHI scores & IL-1ß levels in test group at 1week period in comparison to control group. CONCLUSION: AFG showed promising results as an alternative to sutures. The use of AFG significantly improved clinical parameters and reduced the levels of pro-inflammatory cytokines making it an effective adhesive with healing properties in the era of suture-less periodontal flap surgery.

ABSTRACT

Background: Platelet-rich fibrin matrix (PRFM) has not been extensively studied as other platelet concentrates such as Choukron's platelet-rich fibrin (PRF). This randomized controlled trial aimed to evaluate PRFM regenerative ability in human periodontal intrabony defects. Materials and Methods: Fifteen patients (age: 30-55 years) having probing pocket depth (PPD) ≥6 mm, and radiographic evidence of bilateral vertical intrabony defects were recruited. A split-mouth design was used in each patient; one quadrant of the arch was treated with open flap debridement (OFD) alone (control group) and the other quadrant with OFD + PRFM (test group). The PRFM was prepared by dual-spin technology using a patented thixotropic separator gel. Outcome measures comprising plaque index, gingival index (GI), PPD, clinical attachment level (CAL), depth of the defect, defect fill (DF), and percentage of DF (PDF) were recorded at baseline, after 3 months and 6 months. The parameters were applicably analyzed using the Friedman test, Fisher's exact test, t-test, paired t-test, repeated measures ANOVA, and Post Hoc-Bonferroni correction. Results: The GI, a net reduction in CAL, and PPD of the test group were significantly better than the control group at 3 months and 6 months (P < 0.05), while DF and PDF showed significant results at 6 months (P < 0.05). A consistent early wound healing index of 1 at 1 week was displayed in the test group (66.7%) in comparison to the control group (33.7%). Conclusion: PRFM can be a clinically significant periodontal regenerative material in the treatment of vertical intraosseous defects.

ABSTRACT

Technological advances in the field of implantology have led to the concept of surface modifications to enhance implant stability by utilization of current concepts of tissue engineering and materials such as platelet concentrates and stem cells. The purpose of the present randomized controlled clinical trial was to evaluate and compare the effect of platelet rich fibrin matrix (PRFM) with and without peripheral blood mesenchymal stem cells (PBMSCs) on implant stability; by assessing the bone to implant contact (BIC) using resonance frequency analysis (RFA), insertion torque and also to establish and correlate the same with implant stability quotient (ISQ). A total of 15 patients with 30 sites ensuring a minimum of two dental implants adjacently placed in an edentulous area; with the age group of 25-50 years of both the sexes were categorized into Group 1 (dental implant with PRFM) and Group 2 (dental implant with PBMSCs embedded in PRFM). Insertion torque values at the time of dental implant placement and ISQ using RFA was recorded at 1 week, 1 month, and 3 months post operatively. There was no significant difference (p = 0.81) in Insertion torque values between both the groups (G1 and G2). Platelet rich fibrin matrix along with PBMSCs enhanced implant stability as higher and statistically significant ISQ values were noted at 1 week (p = 0.18), 1 month (p ≤ 0.001), and 3 months (p ≤ 0.001) intervals in the G2 group. Platelet rich fibrin matrix and PBMSCs showed promising results as a potential regenerative material for increasing and enhancing BIC and hence implant stability.

ABSTRACT

INTRODUCTION: Dentinal hypersensitivity (DH), is a commonly encountered clinical problem, the management of which is by two main approaches that involve blockage of nerve activity and tubular occlusion. Desensitizing agents containing arginine and calcium carbonate have emerged effective in occluding tubules. Ozone removes the smear layer and opens up the dentinal tubules for easy penetration of these desensitizing agents. OBJECTIVE: To comparatively evaluate the effect of ozonated oil on the patency and occlusion of dentinal tubules with and without adjunctive application of a desensitizing agent containing arginine. MATERIALS AND METHODS: 80 dentin specimens that were distributed into 4 groups as Group 1 (control), Group 2 (ozonated oil), Group 3 (desensitizing agent) and Group 4 (desensitizing agent following the application of ozonated oil), underwent an acid challenge. Scanning electron microscopic (SEM) images of the dentin specimens were obtained prior to any treatment, after the application of the therapeutic agents and after acid challenge and mean tubule occlusion scores were recorded in each case employing a suitable scoring system. The statistical analysis employed One-way ANOVA test followed by Tukey's Post hoc test and Student Paired t-test for intergroup and intragroup comparisons of the mean tubule occlusion scores respectively. RESULTS: Group 2, treated with ozonated oil alone showed a statistically significant increase in the number of open dentinal tubules. Group 4 showed more compact deposits of desensitizing agent and more densely occluded tubules as compared to Group 3 which was more retained in the former even after acid challenge. CONCLUSION: Adjunctive application of the desensitizing agent containing arginine, with ozonated oil has a synergistic effect, where the latter causes opening of dentinal tubules allowing more compact penetration of the former and thus may be a potentially more effective treatment approach in the management of DH.

ABSTRACT

A 3 year old female patient with history of congenital hydrocephalus and epileptic episodes since a week was referred to the department from Dhiraj general hospital for dental opinion. The patient was non ambulatory. On initial clinical examination grade II mobility was present with lower anterior teeth along with lower lip laceration and an extra oral swelling on the left side of the face. Pharmacological management was opted for this patient as regular chair side management was not feasible. The necessary investigations were completed. When the patient was taken under general anaesthesia for treatment, a radical symptomatic and preventive treatment was opted. Such cases pose a challenge to the Pedodontist's decision making skills and must be dealt considering the long term health benefits.

ABSTRACT

Enamel microabrasion is a noninvasive method that removes intrinsic and superficial defects from teeth aimed to improve dental esthetic with minimal loss of dental tissue. This case presentation describes the attempt for teeth color correction utilizing that conservative technique in a young girl whose maxillary anterior teeth presented an opaque white/brown stain. Along with microabrasion, an innovative approach of application of casein phosphopeptide-amorphous calcium phosphate crème on the tooth, and remineralization was carried out thereby reducing postoperative sensitivity of the treated tooth. Based on the results of this case report, it can be concluded that this technique is efficient and can be considered a minimally invasive procedure.

ABSTRACT

Plastic in any form is a nuisance to the well-being of the environment. The 'pestilence' caused by it is mainly due to its non-degradable nature. With the industrial boom and the population explosion, the usage of plastic products has increased. A steady increase has been observed in the use of plastic products, and this has accelerated the pollution. Several attempts have been made to curb the problem at large by resorting to both chemical and biological methods. Chemical methods have only resulted in furthering the pollution by releasing toxic gases into the atmosphere; whereas; biological methods have been found to be eco-friendly however they are not cost effective. This paves the way for the current study where fungal isolates have been used to degrade polyethylene sheets (HDPE, LDPE). Two potential fungal strains, namely, Penicillium oxalicum NS4 (KU559906) and Penicillium chrysogenum NS10 (KU559907) had been isolated and identified to have plastic degrading abilities. Further, the growth medium for the strains was optimized with the help of RSM. The plastic sheets were subjected to treatment with microbial culture for 90 days. The extent of degradation was analyzed by, FE-SEM, AFM and FTIR. Morphological changes in the plastic sheet were determined.

ABSTRACT

Nanotechnology has proven its competence in almost all possible fields we are aware of. However, today nanotechnology has evolved in true sense by contributing to a very large extent to the food industry. With the growing number of mouths to feed, production of food is not adequate. It has to be preserved in order to reach to the masses on a global scale. Nanotechnology made the idea a reality by increasing the shelf life of different kinds of food materials. It is not an entirely full-proof measure; however it has brought down the extent of wastage of food due to microbial infestation. Not only fresh food but also healthier food is being designed with the help of nano-delivery systems which act as a carrier for the food supplements. There are regulations to follow however as several of them pose serious threats to the wellbeing of the population. In coming days, newer modes of safeguarding food are going to be developed with the help of nanotechnology. In this paper, an overview has been given of the different methods of food processing, packaging, and preservation techniques and the role nanotechnology plays in the food processing, packaging, and preservation industry.
