ABSTRACT

PURPOSE: To assess clinical and radiological outcomes at 2-year follow-up of one-level minimally invasive transforaminal interbody fusion with unilateral pedicle screw fixation (UNILIF) in the treatment of stable lumbar degenerative diseases. METHODS: From January 1, 2012 to January 31, 2013, we prospectively collected clinical and radiological data on patients with stable degenerative lumbar disease managed by UNILIF in a single institution. Preoperatively and at 2 years, we recorded ODI, SF-12, Quebec and VAS. Interbody fusion was analyzed on radiography and on a CT scan, and sagittal balance was tested on full spine radiography. RESULTS: Mean operation time was 74.5 min ± 16.8, mean blood loss was 130.8 ml ± 210.9. At 2 years, ODI, SF-12, Quebec and VAS were significantly improved (p > 0.005).The fusion rate was 96.8% on radiographic analysis and was 87.9% on CT scan analysis. CONCLUSIONS: One-level UNILIF constitutes an effective alternative for management of stable lumbar degenerative diseases. These slides can be retrieved under Electronic Supplementary Material.

ABSTRACT

INTRODUCTION: Interbody fusion is the gold standard treatment for the management of numerous diseases of the spine. Minimally invasive techniques may be more beneficial than conventional techniques. The main goal of this study was to report the one-year postoperative results of a series of posterior lumbar interbody fusions by a minimally invasive technique in relation to improvement in functional outcome, interbody fusion and morbidity. MATERIALS AND METHODS: Between January 2012 and May 2013, 182 patients treated by minimally invasive posterior transforaminal lumbar interbody fusion (TLIF) were included in this prospective multicenter study. Clinical assessment was based on a comparison of the preoperative and one-year postoperative Oswestry (ODI), SF-12 and Quebec Scores and the Visual Analog Scale (VAS). Surgical and postoperative follow-up data were evaluated. Radiological assessment was based preoperative and one-year postoperative full spine teleradiographs. Interbody fusion at one-year was systematically evaluated by CT scan. RESULTS: One hundred and eighty-two patients were included, mean age 58.9 years old. Surgery lasted a mean 101 minutes, mean preoperative bleeding was 143 mL, and mean radiation exposure was 247.4 cGy/cm(2). The rate of postoperative complications was 7.7%. The ODI, the Quebec Score, the SF-12 and the VAS were all significantly improved at one-year (P<0.0001). The rate of fusion was 72.6% at the final follow-up. There was no significant difference in functional outcome between patients with and without fusion. DISCUSSION: The one-year postoperative radiological results and functional outcome of minimally invasive posterior lumbar fusion are satisfactory. The benefits of this minimally invasive approach are mainly found in the first 6 postoperative months. Successful radiological interbody fusion was not correlated to functional outcome at the final follow-up.

ABSTRACT

INTRODUCTION: We conducted a prospective, single-center, continuous study of patients operated for fractures urelated to osteoporosis at the thoracolumbar junction level using percutaneous techniques. The aim of this study was to investigate the clinical and radiological outcomes of percutaneous techniques for these indications. PATIENTS AND METHODS: This study included patients who underwent standalone balloon kyphoplasty surgery or combined with percutaneous posterior osteosynthesis in cases of associated distraction. The fractures were classified according to the Magerl classification. The patients were evaluated clinically (visual analog scale [VAS], the Oswestry Disability Index, and autonomy) and radiologically (vertebral kyphosis and height variations of the vertebral body) for 12 months. RESULTS: Sixty-five patients were included. The mean age at the time of the surgery was 45.4 years (range, 19-72 years). The main indications were A.1 fractures of L1. We noted 22% cement leakages, none having a clinical impact. In the overall series, the VAS at the lesional level improved from 5.5 (range, 3-8) preoperatively to 0.6 (range, 1-3) at 12 months. In all, 95% of the workers resumed their occupation. Traumatic kyphosis improved from 13.3° (range, 5-23°) before the surgery to 8.3° (range, 1-20°). DISCUSSION: The complication rate was low. The radiological results are comparable to those reported in the literature for other series with percutaneous surgery. Only the loss of the correction observed in the group undergoing standalone kyphoplasty with calcium phosphate cement led us to propose another type of treatment for these indications. This study must be continued over the long term to detect the appearance of discopathy related to cement leakage and to answer questions as to how cement evolves. LEVEL OF EVIDENCE: III, prospective study with low statistical power.

ABSTRACT

INTRODUCTION: One objective of surgery in thoracolumbar spine fracture is to restore correct and lasting spinal statics. This may involve vertebral body replacement using an anterior approach. We here report results on a prospective series of 23 trauma patients managed by vertebral body replacement using an expandable cage. PATIENTS AND METHODS: The sex ratio was 2.28. Fifteen cases involved primary treatment of recent fracture and eight secondary surgery for non-union or malunion. In 12 cases, posterior osteosynthesis was associated. Six patients were operated on using a classical approach and 17 using a video-assisted minimally invasive approach. Pre- and perioperative data were recorded, with clinical scores (VAS and Oswestry) at 6 weeks, 3 months, 6 months, 1 year and 2 years. Radiologic follow-up assessed regional traumatic kyphosis (RTK), enabling calculation of regional traumatic angulation (RTA), with control CT to check fusion. RESULTS: Minimum follow-up was 2 years. There were no cases of postoperative neurological deterioration. There were three major postoperative complications: one hemothorax, one adhesive bowel occlusion, and one bilateral pneumothorax at 1 month. Mean Oswestry score at 6 months was 20%, and mean VAS score at 2 years was 0.36. Postoperative RTA showed a mean 7.34° improvement. Mean RTA reduction loss was 1.95° at 3 months, subsequently unchanged. All arthrodeses showed fusion at 6 months. CONCLUSION: Results were satisfactory with this technique, comparable to those reported in the literature. The development of minimally invasive approaches and improved instrumentation procedures optimize surgery and enhance anterior reconstruction tolerance. Lasting restoration of sagittal spinal curvature improves trauma patients' functional recovery.

ABSTRACT

Thoracolumbar fractures are frequent and the functional outcomes are sometimes severe. This multicentric study, including five medical centers, was performed to evaluate the long-term outcomes of the patients. One hundred and thirty six patients with thoracolumbar fracture (T11 to L2) was evaluated with a minimal follow-up of two years. Every one had a clinical exam with a score of Oswestry and an X-Ray study (before and after treatment and at revision). Most of them presented compression fractures, the most often at L1 level. On X-rays, a gain was noted on the vertebral kyphosis immediately after surgery, but there is a loss of correction over time whatever the treatment. The clinical outcomes for the patients were great, with an Oswestry average score of 6,4. A correlation was noted between this functional score and vertebral kyphosis. So, an anterior column strengthening (isolated or performed during the surgery) could improve these functional outcomes. Moreover, the Thoraco Lumbar Injury Severity Score (TLISS) seems to be a simple organigram to determine the most appropriate treatment of these fractures, with particular attention to the distraction mechanism or posterior ligamentous complex lesions. However, RMI before surgery is necessary to evaluate these lesions.
