ABSTRACT

Ischemic hepatitis (IH) is a rare entity, usually associated with hypoperfusion secondary to low cardiac output (congestive heart failure, acute myocardial infarction or dilated cardiomyopathy), followed in frequency by respiratory insufficiency and sepsis. A 49-year-old man presented at the emergency room with progressive dyspnea and hypotension, objectifying a significant increase in transaminases (aspartate transferase 5.550 U / L, alanine transferase 3.826 U / L) and LDH (10.375 U / L). Liver Doppler ultrasound was normal. The echocardiogram confirmed the suspected diagnosis of massive pericardial effusion. Cardiac tamponade is a rare cause for this clinical picture. In the case of our patient, rapid diagnosis and urgent pericardiocentesis led to his prompt recovery.

ABSTRACT

OBJECTIVE: To evaluate the 4 years in operation of the preventive health programme for dental hygiene in children. DESIGN: Two epidemiological crossover and observational studies, retrospective over four years (1990-1994) in order to assess the incidence of dental caries in the target population (n = 800). SETTING: Les Planes Health District, Sant Joan Despí (Barcelona). PATIENTS: Children from 4 to 14 included in the caries prevention programme. INTERVENTIONS: This dental hygiene programme consisted of: school check-ups twice a year, monitoring children at risk of caries each term, education in oral hygiene, directly applied fluoride and sealing cracks at the health centre. MEASUREMENTS AND MAIN RESULTS: To assess the results of the programme, the difference in the incidence of caries between both groups was assessed with the CAO-D index, i.e. the average number of teeth affected by child caries. Overall and for ages, a decrease in the incidence of caries was noted. CONCLUSIONS: The programme has given positive results, which should encourage other health centres in their prevention programmes.

ABSTRACT

The authors have made a systematic study by PRICK-test and RAST, sometimes followed by a rhino-manometric provocation, of the frequency of sensitization to the two moulds Alternaria and Cladosporium in 200 "all comers" children in a paediatric consultation. The frequency was low, only 10% for those consulting in the groups of asthma, rhinitis, pollinosis, repeated respiratory infections and syndromes of obstructed expiration taken together, and 13% for allergic asthma only. This sensitivity is rarely isolated, but is most often accompanied, or complicated by, other pneumoallergens, such as dust, mites, pollens and sometimes cockroaches. It imposes certain particular clinical traits on asthma (summer asthma or recurring asthma) and in others appears to be strongly influenced by the habitat. The skin tests seem to be very weak and RAST insufficient to be sure of real sensitization. Diagnosis, always difficult, is probably best done by a provocation test (preferably rhino-manometry) and this jumps over the bundle of clinical and biological arguments, as well as being, at least in our country, the most useful mimic of atmospheric moulds. The authors insist on the importance of the interrogation which should be concerned with the chronological and environmental circumstances and all respiratory infections that are suspected of having an allergic factor.

ABSTRACT

Two sisters and an unrelated girl presented a distinct intrauterine growth retardation-malformation syndrome with short stature, microcephaly, pectus excavatum, hip dislocation, hypoplastic pubic region and genitalia, camptodactyly, talipes, shortened 2nd toes, hypoplastic patella and skeletal dysplasia probably due to homozygosity from an autosomal recessive gene.

ABSTRACT

A hitherto undescribed inv(2) (p2300q11.2) was found in 2 generations of a family ascertained through a holoprosencephalic liveborn boy with normal karyotype. This inversion, quite probably not related to the child malformations, does not seem neither impair reproductive fitness nor to yield viable recombination aneusomies.
