Les auteurs rapportent une observation exceptionnelle de syndrome de la jonction pyelo-ureterale congenital sur rein unique. Il s'agit d'un cas decouvert chez un jeune homme de 21 ans apres une lombalgie chronique unilaterale droite evoluant depuis trois ans. La fonction renale etait toujours conservee. Le diagnostic a ete confirme par les examens d'imagerie. Une pyeloplastie a ciel ouvert a donne un excellent resultat

Pregnancy brought to term is rare in patients undergoing haemodialysis. Both mother and foetus are exposed to lethal from complications frequently caused by hypertension and anaemia. We are reporting the first case of pregnancy observed in an haemodialysis centre in Antananarivo, Madagascar. In spite of the modest means of the centre (use of acetate, lack of recombinant erythropoietin, limited biological controls) the foetus could be kept by intensification of the dialysis. No complication occurred to the mother when in the literature its frequency is 16%. The baby was premature and low weighted. His mental and physical development was normal. This success is the consequence of heavy management especially for the patient.

We report a case of impure nephrotic syndrome occurring during cysticercosis. Renal biopsy showed membranous glomerulonephritis, indicating that there were immune complexes involved in reaction to this parasite. The patient had previously received prednisolone treatment without any success. Subsequently, treatment with praziquantel was followed by the disappearance of subcutaneous nodules with a decrease of proteinuria. This led to a remission of the nephrotic syndrome and, after an immediate post-therapeutic peak, lower serologic levels. Also, after an immediate post-therapeutic peak, creatinemia reached the proper value, which was lower than before the treatment, within twenty months of evolution. Because nephropathy complicated the cysticercosis and the antiparasitic treatment was favorable, research on this type of infection is interesting. Also, the serology of cysticercosis should be added to the list of antigen tests which could identify the cause of membranous glomerulonephritis.

Des lesions cutanees et sous-cutanees a type de nodules sous une peau erythemateuse et douloureuse sont les caracteristiques de l'erytheme noueux. Il est souvent la manifestation de la maladie systematique. Plusieurs etiologies sont relevees; en particulier la tuberculose; bien que dans 50 pour cent des cas on ne trouve pas de cause. Les auteurs rapportent le cas d'une patiente porteuse de lesions d'erytheme noueux et une tuberculose ganglionnaire sus-claviculaire dont le diagnostic a ete evoque a l'examen histopathologique et la presence de Mycobacterium tuberculosis confirmee par la culture. En cette periode de recrudescence de la maladie tuberculeuse; il est opportun de penser a cette association

Following the study done in 1990 concerning the existence of glomerular lesions in falciparum malaria, this study, pursued on large scale, included 110 persons (30 healthy persons, 11 other than falciparum malaria, and 69 cases of falciparum malaria). 30.4% among these 69 cases of falciparum malaria presented glomerular affect signs. A complete clinical and biological recovery was obtained with quinine injection.

The dissolution of uric acid calculi can be obtained by per os or parenteral alkalinization, but this method cannot be used in case of obstructive calculi. With 3 patients, whose urinary tract was obstructed by a radio-transparent calculi at the pyelo-ureteral junction, the ureteral probe setting by endoscopic way permitted to realize the in situ alkalinization. After 5 to 11 days of treatment, the calculi have been completely dissolved. A patient also presented a slightly radio-opaque lithiasis, but it remained still. The in situ alkalinization is an effective and non invasive treatment in case of obstructive uric acid calculi, and completes the release of the urinary tract logically by ureteral probe setting.
