ABSTRACT

As of now, a dental surgeon and a maxillofacial surgeon face a problem in choosing the optimal treatment of extensive cavitary masses in the maxillary bones. A detailed study of the characteristic x-ray manifestations of large jaw bone cavities makes it possible to give an insight into the pattern and extension rate of a destructive process, to choose the most optimal surgical treatment, and to yield more predictable results. The aim of this investigation was to analyze the morphological characteristics of large odontogenic cysts in the jaw bones and to reveal the main specific features of their x-ray picture in relation to their histological structure. An x-ray study determines the location, sizes, shape, and structure of the shadow of a cavity, its relation to the adjacent anatomic masses, as well as bone swelling, and preserved cortical laminae and facilitates the most correct presumptive diagnosis.

ABSTRACT

The experimental work has been done on big biomodels (laboratory mini-pigs) for establishing of optimal timing (1, 3, 6.5 months) for functional loads intraosseous implantation. It was shown in morphological dynamic studies in the period from 1 to 6.5 months that connective tissue capsule was formed around implant which is maturing quickly, packing and thinwing by reparation osteogenesis. The osseous mass growth was noted on implant surface and alveolar process thickening in the implantation zone under alveolar process due to the influence of early functional loads.

ABSTRACT

Patients with temporo-mandibular joint (TMJ) dysfunction need complex treatment that includes prosthetic treatment in intrajoint relation stabilization. In cases of TMJ pathology it is necessary to examine patients and make axiography, function analysis, MPI-analysis, magnetic resonance tomography and zonography of TMJ, electromyography of the masticatory muscles. The authors examined 47 patients with TMJ dysfunction, 43 of them had occlusion pathology. We managed to eliminate the dysfunction symptoms and to receive stable result of the treatment in all the patients.

ABSTRACT

In this study authors demonstrate description and analysis of clinical and roentgenological presentations of early synostosis of cranio-cerebral sutures in children. Roentgenological description of cranio-cerebral sutures in infants showed up their significant narrowing and the increase in shadow density, early appearance of ossifications zones in the peripheral parts of the sutures, diffuse increase of cranial bones thickness because of apposition osteogenesis disorder. Results of the study show that many alterations in the cerebral cranium are accompanied with malformations of facial skeleton.

ABSTRACT

The paper describes the evolution and consequences of different modes of eliminating orbital bottom defects and deformities that develop in posttraumatic deformities due to bone fractures of the facial mid-skeleton. Spiral computed tomography of this portion of the facial skeleton has demonstrated that the optimum results of restoration of the orbital bottom and those of elimination of eyeball displacement give the use of osteoplasty with a split parietal bone graft.

The paper deals with X-ray parameters in indirect implantation and with the impact of early functional load on osteointegration in the experimental and clinical settings. Analysis of cases suggests that implant installation just after tooth extraction accelerates implant osteointegration and causes no complications.

ABSTRACT

The possibilities of the use of carbon implants in cases of any defects of any part of facial skeleton and soft tissues of both inherent and acquired nature were studied. Taking into account X-ray transparency of carbon implants the experiment was carried out to make the material X-ray contrast when using the addition of powdered metallic boron in the range of concentrations 8-12%. It was successful and the samples were visualized. In these cases spiral computed tomography is the optimal tool to make all deformation volume clear, to plan operative interventions and to observe the treatment results.

ABSTRACT

Questions of diagnostics and treatment of odontogenic cysts with contiguous invasion of sinuses are discussed. It was shown that the use of computer tomography increased considerably its diagnostic value in determination of size and localization of the cyst, its relationship with sinus membranes. It helps to plan the operation correctly and prevents complications. The results of clinical-roentgenological examination and treatment of 10 patients with odontogenic cysts with contiguous invasion of sinuses are offered.

A rare case with osteochondroma located near the condylar process of the mandible is described. Surgical treatment was carried out (removal of the formation through the periauricular approach). Orthopedic treatment was carried out during the postoperative period. The proportions of the facial skull, occlusion, and articulation were completely restored.

ABSTRACT

The paper considers whether carbonic materials whose contrasting is achieved when they contain 8-12% of metallic boron power may be applied to restore bone defects of the facial skull. Implants of this composition were employed to operate on 4 patients with different defects of the facial skull. A three-projection of SCT image obtained in the "SOFT" mode was used as a way of registration. The results of the studies have indicated that the materials under study have X-ray contrast, a complete biological compatibility, and non-toxicity when they are used in clinical practice.

ABSTRACT

Results of using different types of endoprostheses for repair of the temporomandibular joint (TMJ) are discussed. The main advantages and drawbacks of the method of endoprosthetic treatment of the mandibular condylar process, detected during the immediate and remote postoperative periods, are described. The biomechanics of mandibular movements in prostheses of the joint is presented. Results of using complete endoprosthesis of the TMJ, designed with consideration for mandibular movements biomechanics and experience gained in the use of various types of the joint endoprostheses are presented.

ABSTRACT

The paper discusses the most rational algorithm of an X-ray study and its results obtained while analyzing X-ray findings in 320 patients with severe congenital deformities of the visceral cranium. The malformations are shown to involve not only the visceral cranium, but also the cerebral cranium and its base and they can be rarely assigned to any of the known syndromes as they have frequently something in common. The most rational scheme of a X-ray study is presented.

ABSTRACT

A method for simultaneous surgical treatment of various types of injuries of bone and soft-tissue structures of the zygomatico-naso-orbital area is presented. Special attention is paid to repair of posttraumatic eno- and hypophthalmos. A total of 42 patients were operated on by the proposed methods at Central Institute of Dentistry in 1998-2002. The injuries were diagnosed using computer-aided tomography of the facial skeleton. Bone tissue defects were repaired by free bone autotransplants of the parietal bone, ileac bone crest, tibial bone, autotransplants of the rib and auricular cartilage. Coronary, oral, subciliary accesses are recommended; endonasal access can be used if necessary. The complex approach presented in this report is recommended for repair of posttraumatic deformations and defects and for the treatment of fresh injuries of this location.
