ABSTRACT

PURPOSE: To describe a rare complication of classical EEC technique with posterior chamber IOL implantation in the sulcus. PATIENTS AND METHODS: This is a retrospective study of 296 eyes which underwent EEC and CP implantation. None was operated with phacoemulsification. The posterior chamber IOL originated from 4 renowned manufacturers. RESULTS: In 25 eyes, at 2 to 7 month after surgery, we noticed a mydriasis with pupillary areflexia, though the majority (20 eyes) presented a miotic pupil in the days following surgery. CONCLUSION: The authors discuss possible causes of such a delayed complication: direct trauma of the iris, difficult haptic introduction in the sulcus, toxic material on the i., o.l. More probably the interaction of more than a cause is necessary for this delayed complication.

ABSTRACT

Three patients with unilateral iridoschisis and glaucoma (1 with bilateral open-angle glaucoma, 1 with unilateral glaucoma) were observed for 3 to 15 years. In none of these patients did iridoschisis progress and a good control of the glaucoma was achieved. The mechanism of this rare condition is yet not clear.

ABSTRACT

In 3 patients under local pilocarpine medication, corneal marginal infiltration and limbic ulcerations were noted that were typical for allergic marginal keratitis. A classical allergic conjunctivitis was present in every case. They healed after pilocarpine instillations were suspended and local corticosteroids together with oral antihistaminic drugs were applied. Marginal allergic erosions of the cornea is another form of secondary complication in patients using pilocarpine.

ABSTRACT

The simplicity of Charleux's technique for peripheral iridectomy is underlined. In a lot of 15 eyes with acute angle closure glaucoma, the i.o.p. after surgery was under 21 mm Hg when the attack lasted less than 48 hours. In a second group of 20 eyes with occludable angle/congener eyes suffered attacks/peripheral iridectomy with Charleux's technique prevented acute angle closure/56 months of postoperative observation.

ABSTRACT

The paper presents the evolution at ten eyes (nine patients) that presented expulsive hemorrhage during surgical act of extracapsular extraction with implant of crystalline lens. There are several cases of extracapsular extraction than intracapsular extraction which were more short like duration. The frequency of female was 90% from cases. The good results show the efficiency of associated treatment: closing of the eyeball with scleral punctura and internal tamponing. Only one eye was lost functionally.

ABSTRACT

Are communicated the good results obtained at four eyes with endophthalmia. The patients are treated with fluoroquinolones and cephalosporins at three eye or ampicillin. The eyeball was saved in all cases and at three eyes from it is obtained a good visual acuity.

ABSTRACT

The superior-temporal subluxation of the crystalline lens at the patients with Marfan syndrome makes optic problems who need a surgical solution. It describes one aspiration technique with concomitant support of crystalline lens, underlining the specifics features what are meet here: exessive hardness of crystalloid and the content of crystalline's sack. At eight operated eyes, the postoperatorial visual acuity restored at values over 5/10.

It described one surgical technique for capsulotomy in the case of secondary cataract at an eye with pseudophakic by posterior chamber. The approaching makes with an simple instrument, through pars plana, the tearing of posterior capsule making under control with operator microscope, in area of visual axie, excluding any possibility to harm the lens, its luxation in vitreous or anothers complications.

ABSTRACT

The authors present the case of a family consisting of eight members (belonging to three generations). Among them there are three cases with increased glaucomatous excavation (C/D 0.7-0.8) but normal visual field and ocular blood pressure. Their clinical tracking, till the age of 16, excepts the possibility of glaucoma in these cases, only a genetically transmitted abnormality being implied.

ABSTRACT

The authors review different methods for quantifying papilae's modifications in glaucoma. For each method the possible errors are discussed. The conclusion is that C/D evaluation based only on biomicroscopical measurements is acceptable in everyday practice, the nervous fiber ring's measurement being not yet accurate and simple enough to be widely applied.

ABSTRACT

Blood presence in anterior chamber in coagulated form raises special therapeutical problems. Even when the blood pressure is normal, it is not allowed to wait too much, because the clot doesn't have the tendency of fast resorption. They are emphasized the surgical difficulties, caused by a too large opening and by adherence of the clot to the iris, capsule of the lens and hyaloid, the necessity of use of a spatula for the detachment of the clot in case the urokinasis may not be used.

ABSTRACT

Are presented the results obtained by artificial lens pupillary implant (Fedorov) at 22 eyes, 18 patients between 48-81 years old, to which the development of cataracta was characterized by a great difference between the eyes. It have been obtained 63% very good visual acuity (5/10 and over) and 31% good (5/15-5/40). The authors Conclusion is the artificial lens implant may be done on diabetic patients if FO doesn't have great modifications.

ABSTRACT

There are discussed the observations that had been done for a period of 10 weeks about, after the YAG-neodymium laser trabeculoplasty for 79 eyes suffering of simple chronic glaucoma. The choosing of the patients has been done because of the tensional uncompensation under the maximum treatment for 42 eyes having ocular tension constantly over 30 mm Hg, or because of the lack tolerating the drugs, the secondary, effects, allergy. The average decreasing was of 6.31 mm Hg, high semnificative from the point of view of statistic mathematics but the decreasing of the tension researches 15 mm Hg for isolated cases, that permitting to decrease or renounce the drugs.

ABSTRACT

It is presented a case of symptomatic separating of the retina which ophthalmoscopic examination, that with a Goldmann glass and that of echographic one have justified the enucleation for. The anatomopathological examination hasn't put in evidence any tumour and the subsequent clinical evolution has evidenced an orbital hydatid cyst.

ABSTRACT

The possibility of improving the aspect of the glaucomatous excavation under the influence of the postoperative ocular hypotonization is shown on 12 eyes (11 patients) observed during the past 13 years. All patients suffered from very advanced chronic simple glaucoma--8 had a C/D 0.9 (postoperatively 0.6 in 3 eyes, 0.4 in 4 eyes and 0.1 in 1 eye), 3 a C/D 0.8 (postoperatively 0.4 in 1 eye and 0.2 in 2 eyes) and 1 a C/D 0.7 (which diminished postoperatively to 0.4). Of the 12 eyes, only one showed an improvement of the visual field, 5 a lowering of the visual acuity (progression of the cataract) and 3 a decrease of the visual field after a new increase of the intraocular pressure.

ABSTRACT

Retrograde transcorneal iridectomy suggested by Charleux and Etienne was used in 27 eyes (20 patients) suffering from glaucoma with closed angle (in 12 eyes with acute attack and in 15 congeneric eyes) (prophylactic). The post- or intrasurgical complications were absent. The operation was simple and had good results. The hospitalization period was of 2-3 days.

ABSTRACT

Comparison of the decrease in intraocular tension in a group of 25 patients show that the collyria of oxprenolol, 0.25%, led to an average decrease of 6.36 mmHg, whereas that of thymolol maleate, 0.25%, to a decrease of 8.76 mmHg. Both decreases were statistically significant versus the initial intraocular tension.

ABSTRACT

The authors operated 20 eyes of secondary glaucoma, the angle being blocked by goniosynechias (obstructive glaucoma). All the eyes had a past history of 1-7 antiglaucoma operations by conventional techniques without lowering intraocular tension. The Benedikt operation was efficient in 3 of 10 eyes (compensation of less than 24 mmHg with medication); the La Roca technique in 3 of 7 eyes, and the Nesterov operation in none of the 3 eyes in which it was applied.
