ABSTRACT

PURPOSE: Since symptomatic, non-antibiotic therapy has become an alternative approach to treat acute cystitis (AC) in women, suitable patient-reported outcome measures (PROM) are urgently needed. The aim of this part II of a larger non-interventional, case-control study was the additional assessment of the ACSS as a suitable PROM. METHODS: Data from 134 female patients with diagnosed acute uncomplicated cystitis were included in the current analysis with (1) a summary score of "Typical" domain of 6 and more; (2) at least one follow-up evaluation after the baseline visit; (3) no missing values in the ACSS questionnaire data. Six different predefined thresholds based on the scoring of the ACSS items were evaluated to define "clinical cure", also considering the draft FDA and EMA guidelines. RESULTS: Of the six different thresholds tested, a summary score of the five typical symptoms of 5 and lower with no symptom more than 1 (mild), without visible blood in urine, with or without including QoL issues was favoured, which partially also could be adapted to the draft FDA and EMA guidelines. The overall patient's clinical assessment ("Dynamic" domain) alone was not sensitive enough for a suitable PROM. CONCLUSIONS: Scoring of the severity of symptoms is needed not only for diagnosis, but also for PROM to define "clinical cure" of any intervention, which could be combined with QoL issues. Results of the study demonstrated that the ACSS questionnaire has the potential to be used as a suitable PROM and should further be tested in prospective clinical studies.

ABSTRACT

The regenerative capacity of articular cartilage was studied in animals in which its defects were replaced by biocomposite materials based on polycaprolactone in combination with hydroxyapatite. Six specimens of the material were used, which consisted of different proportions of these polymers. In the experiment on sheep (n = 6) it was found that these biocomposite materials were replaced by hyaline-like cartilage during healing of artificially created defects in the articular cartilage of the knee joint, while the ratio of composite components had no effect on the quality of the regenerates formed. These results support the view of a possible application of biocomposite materials in the treatment of degenerative and traumatic lesions of hyaline cartilage.

ABSTRACT

The aim of this study was to analyse the results of the treatment of extraperitoneal rectal and perineal injuries for 153 patients. All the patients were examined according to the scheme. Therapeutic-diagnostic algorithm was developed for perineal, anal, rectal injuries. Wide opening and drainage of the wound was used. The method allows to avoid colostomy and its subsequent operative closing in extraperitoneal injury of the rectum. The method of surgical treatment is recommended in extraperitoneal rectal and perineal injuries.

ABSTRACT

The study is based on the results of conservative and operative surgical treatment of 184 victims with open and closed, isolated and combined injuries of the liver. The patients' age was from 15 to 80 years. All patients were examined and treated in 1991 to 1996 in Moscow clinical hospital No 67. Conservative treatment for superficial injuries of the liver were carried out in 72 patients. 112 patients underwent urgent operations. Conservative treatment included: examination, control diagnostic laparoscopy and introduction of control drainage followed by dynamic laparoscopy. Operative treatment included: laparotomy, revision of the abdominal cavity, sutures or resection of the liver, surgical treatment of the other abdominal organs. Comparative analysis of two methods of treatment demonstrated possibility and necessity of the use of conservative treatment for superficial injuries of the liver, especially in combined traumas.

ABSTRACT

Comparative evaluation of immediate and long-term results of gastric resection was made in 89 patients operated upon by a traditional method and in 69 patients after laser operations. It was found that laser scalpel used in gastric resection gives reliable hemostasis, reduces the intraoperative blood loss and the time of creation of gastroenteric anastomosis. At the same time 2 times greater amount of inflammatory complications of the anastomosis was noted. The cause of the development of anastomosis after laser resection of the stomach is discussed.
