ABSTRACT

Context: As female workers face a higher risk of exposure to workplace hazards than women in general, it is imperative that measures be put in place in the workplace to ensure safe and healthy pregnancies. Objectives: This study aimed to analyze the effects of occupational hazards in the workplace environment on the potential for pregnancy disorders experienced by female workers. Methods: This was an analytical, observational study with a case-control design. The participants were female workers who worked in industrial environments in both formal and nonformal industries. The inclusion criteria used in this study were female workers who had been or were pregnant at the time of the study, had worked in the industrial sector for at least one year, and did not smoke or consume alcohol. The samples were collected from 144 female workers. Midwives assisted in collecting data and conducting examinations of female workers. This study was conducted in Surabaya and Sidoarjo between June and December 2020. Data were analyzed descriptively, and a multivariable logistic regression test was performed. The study was conducted in accordance with the health protocols and prevention of COVID-19. Results: The results showed that occupational hazards in the workplace environment that affect pregnancy disorders include workload (p = 0.004, OR = 28.676 (2.979-276.076); hot working environment (p = 0.014, OR = 3.077 (1.254-7.552); strong odors (p = 0.017, OR = 7.640 (1.436-40.656); shift work (p = 0.023, OR = 8.063 (1.337-48.623); irregular shift work (p = 0.018, OR = 7.371 (1.409-38.557); and night shift work (p = 0.015, OR = 11.780 (1.605-86.450). Conclusions: Companies are expected to pay special attention to female workers regarding various workplace controls to prevent potential pregnancy-related disorders.

ABSTRACT

Background: Currently, human immunodeficiency virus and acquired immunodeficiency syndrome (HIV/AIDS) has become one of the major health problems worldwide, including Indonesia. East Java is one of the provinces in Indonesia with the highest prevalence of HIV infection. One of the causes of HIV infection transmission is lesbian, gay, bisexual, and transgender (LGBT) practice. Furthermore, the treatment using antiretroviral (ARV) drugs in HIV-1 patients can fail due to the presence of HIV drug resistance. Objective: The aim of this study is to identify the behavior at risk of HIV transmission among LGBT, patterns of genetic variation and antiretroviral (ARV) resistance. Methods: A systematic review and meta-analysis based on the PRISMA guidelines was conducted. We searched three databases including PubMed, ScienceDirect, and Google scholar for studies investigating the non-heterosexual behavior as risk factor of HIV infection and antiretroviral resistance. Only studies published in English are considered. The adjusted estimates of the risk were carried out using best-adjusted OR with 95% confidence interval (CI) and significant p value < 0.05. Results: In the quantitative analysis of HIV infection risk factors, a total of 13 studies were included, which investigated non-heterosexual behavior as a potential factor. The studies involved a total of 37,129 participants, comprising 10,449 individuals in the non-heterosexual behavior group (LGBTQ+) and 26,680 individuals in the heterosexual group. The majority of the participants in this study were from the USA, Japan, China, and Brazil, and the main HIV subgenotypes were B and CRF. Additionally, the antiretroviral resistance of HIV patients was examined, involving a total of 3062 individuals, with 1296 individuals in the non-heterosexual behavior group and 1766 individuals in the heterosexual group. Our calculation showed that non-heterosexual behavior was significant as risk factor of HIV infection (OR = 2.17, 95% CI = 1.94-2.43, p < 0.001) and antiretroviral resistance (OR = 1.31, 95% CI = 1.00-1.71, p = 0.05). Conclusion: This study concludes that non heterosexual behavior is significant risk factor of HIV infection. A quite prevalent of antiretroviral resistance were found among non heterosexual behavior. The main subgenotype of HIV are B and CRF.

Backgrounds: The government's strategy in reducing the high infant mortality rate (IMR) in Indonesia is to place midwives in the village and use the Integrated Management of Young Infants (MTBM) guidelines that integrate all steps through early detection and effective treatment. Midwives have not shown maximum performance in neonatal coverage and neonatal complications, each of which has not reached the target. This study aims to analyze the effect of self-efficacy and work engagement on task performance in the Pamekasan Public Health Center Work Area. Designs and methods: This research was conducted as an analytic study with a cross-sectional study. A sample of 151 village midwives in each Puskesmas' working area, was then analyzed and interpreted to test the model with SEMPLS. Results: Directly self-efficacy has no effect on performance (t statistic 0.315 < 1.96; p-value 0.753 > 0.05), self-efficacy affects work engagement (t statistic 13.98 > 1.96; p-value 0.000 < 0.05), while work engagement has an effect on performance (t statistic 11.426 > 1.96; p-value 0.000 > 0.05). Indirectly, self-efficacy will affect performance if it is through work engagement (t statistic 7.392 > 1.96; p-value 0.000 > 0.05). Conclusion: The findings show that self-efficacy and work engagement can help improve the performance of village midwives in detecting neonatal emergencies.

Background: Older adults experience progressive decline in various organs and changes in pharmacokinetics and pharmacodynamics of the drugs in the body which lead to an increased risk of medication-related problems. Potentially inappropriate medications (PIMs) and medication complexity are key factors contributing to adverse drug events in the emergency department (ED). Objective: To estimate the prevalence and investigate the risk factors of PIMs and medication complexity among older adults admitted to the ED. Methods: A retrospective observational study was conducted among patients aged > 60 years admitted to the ED of Universitas Airlangga Teaching Hospital in January - June 2020. PIMs and medication complexity were measured using the 2019 American Geriatrics Society Beers Criteria® and Medication Regimen Complexity Index (MRCI), respectively. Results: A total of 1005 patients were included and 55.0% (95% confidence interval [CI]: 52 – 58%) of them received at least one PIM. Whereas, the pharmacological therapy prescribed to older adults had a high complexity index (mean MRCI 17.23 + 11.15). Multivariate analysis showed that those with polypharmacy (OR= 6.954; 95% CI: 4.617 – 10.476), diseases of the circulatory system (OR= 2.126; 95% CI: 1.166 – 3.876), endocrine, nutritional, and metabolic diseases (OR= 1.924; 95% CI: 1.087 – 3.405), and diseases of the digestive system (OR= 1.858; 95% CI: 1.214 – 2.842) had an increased risk of receiving PIM prescriptions. Meanwhile, disease of the respiratory system (OR = 7.621; 95% CI: 2.833 – 15.150), endocrine, nutritional and metabolic diseases (OR = 6.601; 95% CI: 2.935 – 14.847), and polypharmacy (OR = 4.373; 95% CI: 3.540 – 5.401) were associated with higher medication complexity. Background: The number of female workers in Indonesia has increased and female workers have been discovered to be very vulnerable to several hazards and health problems in the workplace environment linked to their menstrual cycle and pregnancy. Therefore, this study was conducted to analyze risk factors associated with pregnancy disorders in female workers. Design and Methods: The research was conducted through the use of a cross-sectional design with 307 female workers using simple random sampling. Furthermore, a descriptive analysis was conducted to describe the conditions of the respondents during pregnancy to childbirth process. Results: The results showed 45% of respondents had pregnancy disorders, 16% had miscarriages, and despite the fact most of the prenatal care processes was assisted by doctors or midwives, 2.6% of the respondents consulted Traditional Birth Attendants, who were also recorded and was found to account for 2.9% of the deliveries. Moreover, the risk factors associated with pregnancy disorders include vibrations (P=0.004), irritants (P=0.002) and repetitive works (P=0.009). Conclusions: It is recommended that companies should provide maternal and child health protection and control for any risk associated with female workers.
