ABSTRACT

A case of urachal signet-ring cell carcinoma is reported and compared with the 18 previously reported cases. This tumor appears aggressive, with a five-year survival rate of 27 percent. Only 1 patient achieved a five-year disease-free survival.

ABSTRACT

The stoma is the weak link of external urinary diversion. Improper stomal position or configuration produces the unnecessary burden of leakage for the patient with diversion. Stomal bleeding, incrustation and peristomal dermatitis cause inconvenience and expense. Stomal stenosis is the most common reason for repeat operation of an ileal conduit. The Turnbull loop stoma obviates many of these problems. We have performed 100 urinary diversions using this stoma with minimal stomal complications and no episodes of stomal stenosis from 1 to 7 years postoperatively. The significant complication has been parastomal hernia. Minor modifications of the technique have lowered the incidence of hernia. Loop stomas are our principal form of stomal construction.

ABSTRACT

Information regarding the sensitivity of the Minnesota Multiphasic Personality Inventory (MMPI) to psychopathology and the meaning of MMPI elevations in neurological patients, particularly in those with epilepsy, is of both theoretical and clinical concern. Such information has a particular relevance to the temporal lobe epilepsy-psychopathology controversy. In order to investigate evidence for increased psychopathology as a function of complex partial seizures (temporal lobe epilepsy in the old nomenclature), three groups of epilepsy patients differing in seizure type were compared on the MMPI. No supportive evidence was present for such an association. Second, the sensitivity of the MMPI to psychopathology was examined by comparing three groups of epilepsy patients differing in history of psychiatric problems. The results clearly indicated that the MMPI is sensitive to psychopathology in patients with epilepsy. Finally, the Sc scale was examined in relation to items descriptive of "disease-related experiences" and the psychiatric implications of endorsing such items. About one third of the items were identified as descriptive of "real experiences." Nevertheless, patients with psychiatric histories endorsed more of such items. In conclusion, the MMPI appears to be sensitive to psychopathology in epilepsy. Therefore, insensitivity of this instrument cannot be advanced as an explanation for the results of many studies that have not supported the temporal lobe epilepsy-psychopathology controversy.

ABSTRACT

Chronic pain patients typically display reduced activity level attributed to pain and implying a positive correlation between exercise or activity and pain complaints. This study correlated observed pain complaints with amount of prescribed exercise performed by chronic pain patients when exercising to tolerance. Patients were in evaluation of earliest stages of multi-modal treatment. Exercises were physician prescribed to assess use of involved body parts and to promote general activity level. Patients were instructed to do exercise repetitions until pain, weakness or fatigue caused them to stop. Patients decided when to stop. Observations of amount of exercise performed were correlated with observed visible or audible indications of pain or suffering (pain behaviors). Results indicate a consistent negative relationship, i.e., the more exercise performed, the fewer the pain behaviors. This finding is contrary to the frequently observed physician prescription with chronic pain to limit exercise when pain increases.

ABSTRACT

Simultaneous disappearances of both rivalry images were reported by 28 women who reported on the rivalry 3.2 degree diagonal rivalry contours for 10 min. of continuous viewing. The data support the notion that suppression of images during binocular rivalry is independent in both eyes. In a second study, 48 women reported simultaneous disappearances during the rivalry of 3.2 degree vertical and horizontal contours for 10 min. of continuous viewing under one of two blink-rate instruction conditions. Fewer simultaneous disappearances were reported by Ss instructed to blink every 5 sec. than by Ss instructed to blink every 15 sec. Blinking may reduce the probability of simultaneous disappearances in rivalry. In both studies, the fusion squares surrounding the rivalry stimuli were continuously perceived.
