ABSTRACT
The rarity of genital tuberculosis and the non-specificity of its manifestations lead to delayed diagnosis and the need to treat this disease at an advanced stage. We report two cases observed and treated at the Fianarantsoa Teaching Hospital: two young women (aged 22 and 33 years) discovered during work-ups for primary infertility and secondary amenorrhea. The diagnosis was established by histological examination of granulomatous tissue with caseous necrosis, pathognomonic for tuberculous. Isolation of mycobacterium by inoculation on Lowenstein-Jensen medium and culture are not available in Fianarantsoa. Chemotherapy against tuberculosis is most often effective for the disease, but fertility is definitely compromised, even though in vitro fertilization is possible in some cases.

Post-term pregnancy in the broad ligament is rarely described in the literature. A 33-year-old woman, grava 2 para 1, not followed-up during gestation was admitted at 44 weeks of gestation. The fetus was dead and in transversal position, as shown by ultrasound. A cesarean section was performed and the fetus was extracted from the right broad ligament. The placenta was inserted on the surface of the posterior wall of the uterus. Blood loss was estimated at 1200 mL and the patient received isogroup total blood. There was no major complication.

La survenue d'un Syndrome Catastrophique des Antiphospholipides (SCAPL) contre-indique toute grossesse ulterieure. L'evolution spontanee d'une grossesse chez un terrain ayant un syndrome des antiphospholipides (SAPL) presente un taux d'echec entre 75et 90. La mortalite materno-foetale par SCAPL est de 50. La contraception definitive par ligature des trompes est alors obligatoire. Pourtant; les auteurs rapportent le cas d'une patiente qui a encore pu bien mener trois grossesses apres la survenue d'un SCAPL. Le suivi d'un protocole d'anticoagulation a ete exceptionnellement benefique

L'herpes genital est une infection virale due a Herpes Simplex 1 ou 2. Chez la femme enceinte; elle peut avoir des consequences redoutables chez l'enfant a naitre. Nous rapportons 2 observations qui ont souleve le probleme de diagnostic et de prise en charge. La premiere concernait une primipare de 29 ans dont le diagnostic est etabli vers 38 semaines d'amenorrhee devant une notion d'eruptions peniennes chez le conjoint et des leucorrhees rebelles. La deuxieme concernait une primipare de 22 ans de diagnostic etabli vers 36 semaines d'amenorrhee pour des eruptions vesiculo-suintantes vulvo-vaginales. Les particularites de la prise en charge sont discutees pour prevenir l'herpes neonatal. L'evolution favorable confirme que la cesarienne en urgence associee a un traitement anti-viral pour la mere et l'enfant reste le traitement de choix
