ABSTRACT

Myriad policy, ethical and legal considerations underpin the sharing of biological resources, implying the need for standardised and yet flexible ways to digitally represent diverse 'use conditions'. We report a core lexicon of terms that are atomic, non-directional 'concepts of use', called Common Conditions of use Elements. This work engaged biobanks and registries relevant to the European Joint Programme for Rare Diseases and aimed to produce a lexicon that would have generalised utility. Seventy-six concepts were initially identified from diverse real-world settings, and via iterative rounds of deliberation and user-testing these were optimised and condensed down to 20 items. To validate utility, support software and training information was provided to biobanks and registries who were asked to create Sharing Policy Profiles. This succeeded and involved adding standardised directionality and scope annotations to the employed terms. The addition of free-text parameters was also explored. The approach is now being adopted by several real-world projects, enabling this standard to evolve progressively into a universal basis for representing and managing conditions of use.

ABSTRACT

BACKGROUND AND STUDY AIMS: Post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis (PEP) is a complication associated with important morbidity, occasional mortality and high costs. Preventive strategies are suboptimal as PEP continues to affect 4% to 9% of patients. Spraying epinephrine on the papilla may decrease oedema and prevent PEP. This study aimed to compare rectal indomethacin plus epinephrine (EI) versus rectal indomethacin plus sterile water (WI) for the prevention of PEP. PATIENTS AND METHODS: This multicentre randomised controlled trial included patients aged >18 years with an indication for ERCP and naive major papilla. All patients received 100 mg of rectal indomethacin and 10 mL of sterile water or a 1:10 000 epinephrine dilution. Patients were asked about PEP symptoms via telephone 24 hours and 7 days after the procedure. The trial was stopped half way through after a new publication reported an increased incidence of PEP among patients receiving epinephrine. RESULTS: Of the 3602 patients deemed eligible, 3054 were excluded after screening. The remaining 548 patients were randomised to EI group (n=275) or WI group (n=273). The EI and WI groups had similar baseline characteristics. Patients in the EI group had a similar incidence of PEP to those in the WI group (3.6% (10/275) vs 5.12% (14/273), p=0.41). Pancreatic duct guidewire insertion was identified as a risk factor for PEP (OR 4.38, 95% CI (1.44 to 13.29), p=0.009). CONCLUSION: Spraying epinephrine on the papilla was no more effective than rectal indomethacin alone for the prevention of PEP. ABSTRACT

Programmed death-ligand 1 (PD-L1) and ICOS-L (also referred to as B7 homolog 1 and 2, respectively) modulate the immune inflammatory response. The aim of the present study was to examine the expression levels of these inflammatory mediators in two groups of patients with an Helicobacter pylori (H. pylori) infection; patients with and without gastric cancer. The association between bacterial virulence factors, CagA and VacA, was also examined, as well as their correlation with the inflammatory profile. Endoscopy analysis indicated that 18 patients suffered from cancer and 28 patients suffered from other gastric pathologies. PCR and reverse transcription-quantitative PCR were used to analyze gastric biopsies and determine the expression levels of the inflammatory modulators PD-L1 and ICOS-L, transcription factors, cytokines and other genes associated with inflammation and pathogenicity. All 46 patients were determined positive for markers of H. pylori. Patients with stomach cancer had lower levels of ICOS-L (P<0.05) and GATA3 (P<0.01), a negative correlation between CagA and IL-17 (P<0.05), a positive correlation between CagA and IL-10 (P<0.05), a negative correlation between vacA-m1 and retinoid orphan receptor γt (RORγt) (P<0.001), and a positive correlation between RORγt and ICOS-L (P<0.001). The reduced levels of ICOS-L and GATA3 along with the negative correlation between CagA and IL-17, and between vacA-m1 and RORγt were all associated with an increased risk of gastric cancer in the present cohort.

ABSTRACT

Polyamines (PAs) are small aliphatic amines with important regulatory activities in plants. Biotic stress results in changes in PA levels due to de novo synthesis and PA oxidation. In Arabidopsis thaliana five FAD-dependent polyamine oxidase enzymes (AtPAO1-5) participate in PA back-conversion and degradation. PAO activity generates H2O2, an important molecule involved in cell signaling, elongation, programmed cell death, and defense responses. In this work we analyzed the role of AtPAO genes in the Arabidopsis thaliana-Pseudomonas syringae pathosystem. AtPAO1 and AtPAO2 genes were transcriptionally up-regulated in infected plants. Atpao1-1 and Atpao2-1 single mutant lines displayed altered responses to Pseudomonas, and an increased susceptibility was found in the double mutant Atpao1-1 x Atpao2-1. These polyamine oxidases mutant lines showed disturbed contents of ROS (H2O2 and O2-) and altered activities of RBOH, CAT and SOD enzymes both in infected and control plants. In addition, changes in the expression levels of AtRBOHD, AtRBOHF, AtPRX33, and AtPRX34 genes were also noticed. Our data indicate an important role for polyamine oxidases in plant defense and ROS homeostasis.

ABSTRACT

El cáncer colorrectal (CCR) constituye el segundo tipo de cáncer más frecuente en la población europea. Actualmente no existe biomarcadores moleculares que se pueden utilizar para la detección temprana del cáncer de CCR. KLF6 es un supresor tumoral relacionado con varios tipos de cánceres. Nuestra hipótesis plantea que KLF6 puede ser un excelente marcador en el diagnóstico precoz de CCR. Para estudiar la implicancia de KLF6 en el CCR, se seleccionaron 15 biopsias de cada estadio (T1,T2 y T3) de los archivos del Servicio de Anatomía Patológica del Hospital Central de la Defensa Gómez-Ulla, las cuales presentaban áreas de tejido afectado (tumor) y áreas sin afectación (no tumorales). Para ello se realizó un estudio histológico, inmunohistoquímico y RT-PCR, basada en la expresión de 3 genes, Ki67 y p53 como marcadores positivos y KLF6 como marcador en estudio. Los resultados mostraron que la expresión de KLF6 está directamente relacionada con el aumento de la malignidad celular en los adenocarcinomas, corroboradas por las RT-PCR, observándose la aparición progresiva de formas de procesado alternativo, no correspondiente a KLF6. Esta proteína, se expresó tanto a nivel citoplasmático como nuclear en los primeros estadios T1 y T2, para desaparecer a nivel nuclear en el estadio más avanzado (T3). Concluimos que KLF6 es un buen marcador tumoral de CCR, debido a que muestra patrones crecientes de expresión a nivel citoplasmático y decrecientes a nivel nuclear...


Colorectal cancer (CRC) is the second most common type of cancer in the European population. Currently there molecular biomarkers that can be used for early detection of cancer of CRC. Is a tumor suppressor KLF6 associated with several types of cancers. Our hypothesis is that KLF6 can be an excellent marker for the early diagnosis of CRC. To study the implication of KLF6 in CRC, we selected 15 biopsies of each stage (T1, T2 and T3) from the archives of the Pathology Department of Defense Central Hospital Gómez-Ulla, which had areas of affected tissue (tumor) and unaffected areas (non-tumor). This study was performed histological, immunohistochemical and RT-PCR, based on the expression of three genes, markers Ki67 and p53 as positive and as a marker in KLF6 study. The results showed that the expression of KLF6 is directly related to increased malignancy cell adenocarcinomas, corroborated by RT-PCR, showing the gradual emergence of alternative forms processing, corresponding to no KLF6. This protein was expressed both cytoplasmic and nuclear in the early stages T1 and T2, disappearing at the nuclear level in the most advanced stage (T3). En el presente artículo, comentamos ciertos aspectos inéditos acerca del Gabinete de Radiología del Hospital Naval de San Carlos, centrados en el periodo comprendido entre 1896 y 1921. El escrito está basado principalmente en los documentos encontrados como consecuencia de la investigación bibliográfica y documental llevada a cabo en diversas bibliotecas de la Armada, del Archivo Nacional Don Alvaro de Bazán de El Viso del Marqués (Ciudad Real), de la Biblioteca Nacional y del Archivo General Militar de Segovia. El trabajo ha sido efectuado con motivo del Centenario de la creación de los Gabinetes de Radiología en los hospitales de la Armada, que tuvo lugar en Noviembre de 1897 (AU)

ABSTRACT

El presente estudio fue realizado con el objeto de evaluar la seguridad y eficacia de una Sedación o Anestesia en plano superficial con propofol en bolos por vía endovenosa. Se estudiaron 50 pacientes neurológicos, en donde la sedación se indujo con atropina a dosis de 0.01 mg/kg, fentanyl 1.0 µg/kg seguido de un bolo de propofol a 2.41 ñ 0.23 mg/kg promedio, para producir una adecuada inmovilización. El mantenimiento anestésico se realizó con bolos de propofol a 0.6 ñ 0.02 mg/kg en un tiempo promedio de 9.25 minutos por bolo. La calidad de la anestesia fue excelente en 43 y regular en 7 pacientes. Después de la inducción, la presión arterial media disminuyó transitoria pero significativamente, la frecuencia cardiaca, y la respiratoria no tuvieron modificaciones significativas. Consideramos la presente técnica como adecuada para la inducción y mantenimiento de la anestesia de pacientes que requieren estudios de Resonancia Magnética
