ABSTRACT

We report a case of difficult-to-control mycosis fungoides (MF), where the role of the dental surgeon was crucial for the control and prognosis of the disease. A 62-year-old female patient diagnosed with MF had a previous record of red patches and small raised bumps on the face, along with a cancerous growth in the cervical and vulvar region. The patient was initially treated with methotrexate and local radiotherapy without resolution. Chemotherapy with cyclophosphamide, doxorubicin, vincristine, and prednisone was then started (CHOP protocol). The dental team of a reference hospital was consulted to evaluate swelling in the anterior region of the palate, which had been developing for two months, reporting discomfort when eating. The role of the dentistry team was fundamental in the differential diagnosis of oral lesions with dental infections, second neoplasia, or even a new site of disease manifestation, in addition to controlling mucosal changes resulting from chemotherapy. After ruling out dental infection, the dentistry team performed a lesion biopsy to confirm the diagnosis. The histopathological and immunohistochemical analysis showed atypical lymphoid infiltration of T cells (CD3+/CD4+/CD7-/CD8-), coexpression of CD25, and presence of CD30 cells, corresponding to the finding for MF. Identifying CD30 + allowed for a new chemotherapy protocol with brentuximab vedotin (BV) combined with gemcitabine. This protocol effectively controlled MF, which previous protocols had failed to do. The diagnosis by the dental team was essential for therapeutic change and improvement of the patient's clinical condition without the need for invasive medical procedures.

OBJECTIVE: To assess the impact of enhancement filters on the formation of halo artifacts in radiographs of dental implants obtained with a complementary metal oxide semiconductor (CMOS) system. METHODS: Digital radiographs of dental implants placed in dry human mandibles were processed with the Noise Reduction smoothing filter, as well as the Sharpen 1, Sharpen 4, and Sharpen UM high-pass filters available in the CLINIVIEW™ software (Instrumentarium Dental, Tuusula, Finland). Subjective analysis involved evaluating the left, right, and apical surfaces of each implant for the presence of much, few, or no halo. The objective analysis involved measurement of the halo area using the Trainable Weka Segmentation plugin (ImageJ, National Institutes of Health, Bethesda, MD, USA). Data were analyzed using Friedman's test (subjective analysis) and ANOVA (objective analysis) (α = 5%). RESULTS: In the subjective evaluation, the Sharpen 4 filter produced more radiographs with much halo present, and in the objective evaluation, a bigger halo area when compared to the original images and the Noise Reduction filter for all surfaces (p < 0.05). CONCLUSIONS: When evaluating dental implants, priority should be given to original images and those enhanced with smoothing filters since they exhibit fewer halo artifacts. CLINICAL RELEVANCE: Post-processing tools, such as enhancement filters, may improve the image quality and assist some diagnostic tasks. However, little is known regarding the impact of enhancement filters in halo formation on CMOS systems, which have been increasingly used in dental offices.

ABSTRACT

Surface cleaning of plastic materials of historical value can be challenging due to the high risk of inducing detrimental effects and visual alterations. As a result, recent studies have focused on researching new approaches that might reduce the associated hazards and, at the same time, minimize the environmental impact by employing biodegradable and green materials. In this context, the present work investigates the effects and potential suitability of dense carbon dioxide (CO2) as an alternative and green solvent for cleaning plastic materials of historical value. The results of extensive trials with CO2 in different phases (supercritical, liquid, and vapor) and under various conditions (pressure, temperature, exposure, and depressurization time) are reported for new, transparent, thick poly(methyl methacrylate) (PMMA) samples. The impact of CO2 on the weight, the appearance of the samples (dimensions, color, gloss, and surface texture), and modifications to their physicochemical and mechanical properties were monitored via a multi-analytical approach that included optical microscopy, Raman and attenuated total reflection Fourier transform infrared (ATR-FTIR) spectroscopies, and micro-indentation (Vickers hardness). Results showed that CO2 induced undesirable and irreversible changes in PMMA samples (i.e., formation of fractures and stress-induced cracking, drastic decrease in the surface hardness of the samples), independent of the conditions used (i.e., temperature, pressure, CO2 phase, and exposure time).

ABSTRACT

Vitamin D deficiency is highly prevalent in pregnant women and has been related to a higher risk of gestational diabetes mellitus (GDM). The aim of this study is to analyze vitamin D status evolution in a population of pregnant women with and without GDM. Two-hundred women were included from January 2019 to February 2022 as follows: Control group -CG-, Lifestyle group -LG- (GDM not requiring insulin), and Insulin group -IG- (GDM requiring insulin). Visits were carried out at baseline, antenatal, postpartum, and 1 year after birth. Vitamin D levels, weight, and insulin resistance were measured at every visit. Data about the season, vitamin D supplementation, Mediterranean diet adherence, and physical activity were included. In the three groups, 134 women were included in the CG, 43 in the LG, and 23 in the IG. Vitamin D levels were similar among the groups at baseline, but they were significantly higher in the LG and IG in comparison with the CG at the antenatal visit and significantly higher in the IG vs. CG and LG at the postpartum and 1 year after birth visits. Vitamin D levels were independently related to vitamin D supplementation and the season at baseline, to the season and belonging to the LG or IG at the antenatal visit, and were only independently associated with belonging to the IG at postpartum and 1 year after birth visits. In conclusion, in our population, women with GDM requiring insulin had higher levels of vitamin D in comparison with those not requiring insulin and healthy controls at postpartum and 1 year after pregnancy. Requiring insulin during pregnancy seems to be a factor that independently determines the levels of vitamin D until 1 year after birth. More studies are required to reproduce these data in other populations and to elucidate the mechanisms underlying these findings.

BACKGROUND: Reliable mortality data are essential for the development of public health policies. In Brazil, although there is a well-consolidated universal system for mortality data, the quality of information on causes of death (CoD) is not even among Brazilian regions, with a high proportion of ill-defined CoD. Verbal autopsy (VA) is an alternative to improve mortality data. This study aimed to evaluate the performance of an adapted and reduced version of VA in identifying the underlying causes of non-forensic deaths, in São Paulo, Brazil. This is the first time that a version of the questionnaire has been validated considering the autopsy as the gold standard. METHODS: The performance of a physician-certified verbal autopsy (PCVA) was evaluated considering conventional autopsy (macroscopy plus microscopy) as gold standard, based on a sample of 2060 decedents that were sent to the Post-Mortem Verification Service (SVOC-USP). All CoD, from the underlying to the immediate, were listed by both parties, and ICD-10 attributed by a senior coder. For each cause, sensitivity and chance corrected concordance (CCC) were computed considering first the underlying causes attributed by the pathologist and PCVA, and then any CoD listed in the death certificate given by PCVA. Cause specific mortality fraction accuracy (CSMF-accuracy) and chance corrected CSMF-accuracy were computed to evaluate the PCVA performance at the populational level. RESULTS: There was substantial variability of the sensitivities and CCC across the causes. Well-known chronic diseases with accurate diagnoses that had been informed by physicians to family members, such as various cancers, had sensitivities above 40% or 50%. However, PCVA was not effective in attributing Pneumonia, Cardiomyopathy and Leukemia/Lymphoma as underlying CoD. At populational level, the PCVA estimated cause specific mortality fractions (CSMF) may be considered close to the fractions pointed by the gold standard. The CSMF-accuracy was 0.81 and the chance corrected CSMF-accuracy was 0.49. CONCLUSIONS: The PCVA was efficient in attributing some causes individually and proved effective in estimating the CSMF, which indicates that the method is useful to establish public health priorities.

ABSTRACT

Background: Accurate cause of death data are essential to guide health policy. However, mortality surveillance is limited in many low-income countries. In such settings, verbal autopsy (VA) is increasingly used to provide population-level cause of death data. VAs are now widely interpreted using the automated algorithms SmartVA and InterVA. Here we use conventional autopsy as the gold standard to validate SmartVA methodology. Methods: This study included adult deaths from natural causes in São Paulo and Recife for which conventional autopsy was indicated. VA was conducted with a relative of the deceased using an amended version of the SmartVA instrument to suit the local context. Causes of death from VA were produced using the SmartVA-Analyze program. Physician coded verbal autopsy (PCVA), conducted on the same questionnaires, and Global Burden of Disease Study data were used as additional comparators. Cause of death data were grouped into 10 broad causes for the validation due to the real-world utility of VA lying in identifying broad population cause of death patterns. Findings: The study included 2,060 deaths in São Paulo and 1,079 in Recife. The cause specific mortality fractions (CSMFs) estimated using SmartVA were broadly similar to conventional autopsy for: cardiovascular diseases (46.8% vs 54.0%, respectively), cancers (10.6% vs 11.4%), infections (7.0% vs 10.4%) and chronic respiratory disease (4.1% vs 3.7%), causes accounting for 76.1% of the autopsy dataset. The SmartVA CSMF estimates were lower than autopsy for "Other NCDs" (7.8% vs 14.6%) and higher for diabetes (13.0% vs 6.6%). CSMF accuracy of SmartVA compared to autopsy was 84.5%. CSMF accuracy for PCVA was 93.0%. Interpretation: The results suggest that SmartVA can, with reasonable accuracy, predict the broad cause of death groups important to assess a population's epidemiological transition. VA remains a useful tool for understanding causes of death where medical certification is not possible.

ABSTRACT

La disfunción neurogénica del tracto urinario inferior es una consecuencia frecuente en pacientes con lesión de la médula espinal y es secundaria a la hiperactividad del músculo detrusor, que compromete dos funciones principales: el almacenamiento y el vaciamiento de la orina por daño en los centros de control neurológico de la micción. Se asocia con una morbimortalidad significativa, altos costos en la atención médica y disminución en la calidad de vida. Por ello se debe detectar y tratar de manera temprana, al igual que realizar un seguimiento estrecho que reduzca el riesgo de deterioro de las vías urinarias superiores. El presente artículo tiene como finalidad revisar algunos conceptos básicos en relación con la definición, epidemiología, fisiopatología, presentación clínica, complicaciones y, en especial, el tratamiento de esta condición, siendo este último el objetivo de esta revisión.


Neurogenic lower urinary tract dysfunction is a frequent consequence in patients with spinal cord injury and is secondary to detrusor muscle hyperactivity, which compromises two main functions: storage and emptying of urine due to damage to the neurological control centers of micturition. It is associated with significant morbidity and mortality, high health care costs and decreased quality of life. Therefore, it should be detected and treated early, as well as closely monitored to reduce the risk of upper urinary tract impairment. The purpose of this article is to review some basic concepts in relation to the definition, epidemiology, pathophysiology, clinical presentation, complications and, especially, the treatment of this condition, the latter being the objective of this review.

ABSTRACT

Cellulose nitrate (CN) is an intrinsically unstable material that puts at risk the preservation of a great variety of objects in heritage collections, also posing threats to human health. For this reason, a detailed investigation of its degradation mechanisms is necessary to develop sustainable conservation strategies. To investigate novel probes of degradation, we implemented deep UV photoluminescence micro spectral-imaging, for the first time, to characterize a corpus of historical systems composed of cellulose nitrate. The analysis of cinematographic films and everyday objects dated from the nineteenth c./early twentieth c. (Perlov's collection), as well as of photo-aged CN and celluloid references allowed the identification of novel markers that correlate with different stages of CN degradation in artworks, providing insight into the role played by plasticizers, fillers, and other additives in stability. By comparison with photoaged references of CN and celluloid (70% CN and 30% camphor), it was possible to correlate camphor concentration with a higher rate of degradation of the cinematographic films. Furthermore, the present study investigates, at the sub-microscale, materials heterogeneity that correlates to the artworks' history, associating the different emission profiles of zinc oxide to specific color formulations used in the late nineteenth and early twentieth centuries.

ABSTRACT

Preserving culture heritage cellulose acetate-based historical films is a challenge due to the long-term instability of these complex materials and a lack of prediction models that can guide conservation strategies for each particular film. In this work, a cellulose acetate degradation model is proposed as the basis for the selection of appropriate strategies for storage and conservation for each specimen, considering its specific information. Due to the formulation complexity and diversity of cellulose acetate-based films produced over the decades, we hereby propose a hybrid modeling approach to describe the films degradation process. The problem is addressed by a hybrid model that uses as a backbone a first-principles based model to describe the degradation kinetics of the pure cellulose diacetate polymer. The mechanistic model was successfully adapted to fit experimental data from accelerated aging of plasticized films. The hybrid model considers then the specificity of each historical film via the development of two chemometric models. These models resource on gas release data, namely acetic acid, and descriptors of the films (manufacturing date, AD-strip value and film type) to assess the current polymer degradation state and estimate the increase in the degradation rate. These estimations are then conjugated with storage conditions (e.g., temperature and relative humidity, presence of adsorbent in the film's box) and used to feed the mechanistic model to provide the required time degradation simulations. The developed chemometric models provided predictions with accuracy more than 87%. We have found that the storage archive as well as the manufacturing company are not determining factors for conservation but rather the manufacturing date, off gas data as well as the film type. In summary, this hybrid modeling was able to develop a practical tool for conservators to assess films conservation state and to design storage and conservation policies that are best suited for each cultural heritage film.

A deep understanding of the degradation of cellulose diacetate (CDA) polymer is crucial in finding the appropriate long-term stability solution. This work presents an investigation of the reaction mechanism of hydrolysis using electronic density functional theory calculations with the B3LYP/6-31++G** level of theory to determine the energetics of the degradation reactions. This information was coupled with the transition-state theory to establish the kinetics of degradation for both the acid-catalyzed and noncatalyzed degradation pathways. In this model, the dependence on water concentration of the polymer as a function of pH and the evaporation of acetic acid from the polymer is explicitly accounted for. For the latter, the dependence of the concentration of acetic acid inside the films with the partial pressure on the surrounding environment was measured by sorption isotherms, where Henry's law constant was measured as a function of temperature. The accuracy of this approach was validated through comparison with experimental results of CDA-accelerated aging experiments. This model provides a step forward for the estimation of CDA degradation dependence on environmental conditions. From a broader perspective, this method can be translated to establish degradation models to predict the aging of other types of polymeric materials from first-principles calculations.

PURPOSE: To measure the in vivo longitudinal chromatic aberration (LCA) from the chromatic difference of focus (480 to 700 nm) using psychophysical methods in patients bilaterally implanted with a hydrophobic trifocal intraocular lens (IOL). METHODS: Psychophysical best focus was measured in both eyes at different wavelengths (480 to 700 nm) and at three different viewing distances (0.00, +1.75, and +3.50 diopters [D]) using a custom-developed polychromatic adaptive optics set-up provided with a supercontinuum laser, a Hartmann-Shack wavefront sensor, a deformable mirror, a motorized Badal system, a pupil monitoring system, and a psychophysical channel with monochromatically illuminated stimuli. Measurements were performed on 10 patients (20 eyes) bilaterally implanted with hydrophobic trifocal diffractive IOLs (FineVisionHP POD F GF; PhysIOL). LCA was computed from the chromatic difference of focus curves as the difference between 480 and 700 nm at near, intermediate, and far. RESULTS: The LCA from psychophysical measurements was significantly higher for far vision (0.99 ± 0.06 diopters [D]), than for intermediate (0.67 ± 0.10 D) and near (0.23 ± 0.08 D) vision (one-way analysis of variance, P < .05). CONCLUSIONS: LCA for far vision was significantly higher than for intermediate and near vision in hydrophobic trifocal diffractive IOLs, in agreement with a previous study with the same optical design but hydrophilic material IOLs. The LCA for the hydro-phobic IOL is slightly higher than for the hydrophilic IOL at far. Different combinations of refractive and diffractive LCA will allow optimizing IOL designs to improve polychromatic image quality. [J Refract Surg. 2020;36(12):804-810.].

ABSTRACT

Purpose: As multifocal contact lenses (MCLs) expand as a solution for presbyopia correction, a better understanding of their optical and visual performance becomes essential. Also, providing subjects with the experience of multifocal vision before contact lens fitting becomes critical, both to systematically test different multifocal designs and to optimize selection in the clinic. In this study, we evaluated the ability of a simultaneous vision visual simulator (SimVis) to represent MCLs. Methods: Through focus (TF) optical and visual quality with a center-near aspheric MCL (low, medium and high near adds) were measured using a multichannel polychromatic Adaptive Optics visual simulator equipped with double-pass, SimVis (temporal multiplexing), and psychophysical channels to allow measurements on-bench and in vivo. On bench TF optical quality of SimVis-simulated MCLs was obtained from double-pass (DP) images and images of an E-stimulus using artificial eyes. Ten presbyopic subjects were fitted with the MCL. Visual acuity (VA) and DP retinal images were measured TF in a 4.00 D range with the MCL on eye, and through SimVis simulations of the same MCLs on the same subjects. Results: TF optical (on bench and in vivo) and visual (in vivo) quality measurements captured the expected broadening of the curves with increasing add. Root mean square difference between real and SimVis-simulated lens was 0.031/0.025 (low add), 0.025/0.015 (medium add), 0.019/0.011 (high add), for TF DP and TF LogMAR VA, respectively. A shape similarity metric shows high statistical values (lag κ = 0), rho = 0.811/0.895 (low add), 0.792/0.944 (medium add), and 0.861/0.915 (high add) for TF DP/LogMAR VA, respectively. Conclusions: MCLs theoretically and effectively expand the depth of focus. A novel simulator, SimVis, captured the through-focus optical and visual performance of the MCL in most of the subjects. Visual simulators allow subjects to experience vision with multifocal lenses prior to testing them on-eye. Translational Relevance: Simultaneous visual simulators allow subjects to experience multifocal vision non-invasively. We demonstrated equivalency between real multifocal contact lenses and SimVis-simulated lenses. The results suggest that SimVis is a suitable technique to aid selection of presbyopic corrections in the contactology practice.

This work aims at understanding the influence of the production processes and materials in the properties and long term behavior of acrylic sheet, i.e., poly(methyl methacrylate) (PMMA), a material generally considered very stable in museum collections. A comparative study was conducted in samples from cast acrylic sheets produced in the early 2000s, from which manufacturing details were known, and samples provided by the artist Lourdes Castro from acrylic sheets she had bought in the 1960s. Transparent and red opaque cast acrylic samples, containing cadmium red pigment, were used. All samples were artificially aged in a solarbox with irradiation λ > 300 nm for a total period of 8000 h, and alterations were followed by a multi-analytical approach which included Raman, infrared (FTIR-ATR) and UV-Vis spectroscopies; gravimetry; size exclusion chromatography (SEC); thermogravimetry (TGA); micro-indentation; colorimetry; and optical microscopy. Not all cast PMMA sheets presented similar stabilities. We have concluded that the production processes (which may include the polymerization conditions, the organic additives and the origin of the monomer) play a more important role in the properties and long-term behavior of these acrylic sheets than the presence of cadmium red and/or the age of the material.

Desde su notificación a finales del 2019, la infección secundaria a coronavirus se ha difundido a nivel global hasta ser calificada como una pandemia. La respuesta inflamatoria generalizada que produce la infección por Sars-CoV-2, compromete múltiples órganos y sistemas, siendo el sistema respiratorio uno de los más afectados y con desenlace fatal, sin embargo, la afectación del sistema nervioso periférico puede implicar el desarrollo de déficits permanentes, con limitaciones en las actividades de la vida diaria y restricción en la participación, llevando a la pérdida de independencia y reducción de la calidad de vida. El médico rehabilitador debe estar enfocado al reconocimiento y detección temprana de la patología nerviosa periférica, así como a determinar la intervención de la cual se beneficia más el paciente, liderar el equipo interdisciplinario de salud y mantener un trabajo fluido con otras especialidades médicas.


Since its notification at the end of 2019, the infection secondary to coronavirus has spread globally to the point of being qualified as a pandemic. The generalized inflammatory response produced by Sars-CoV-2 infection involves multiple organs and systems, being the respiratory system one of the most affected and with fatal outcome, however, the involvement of the peripheral nervous system may involve the development of permanent deficits, with limitations in activities of daily living and restriction in participation, leading to loss of independence and reduced quality of life. The rehabilitation physician should be focused on the early recognition and detection of peripheral nerve pathology, as well as to determine the intervention from which the patient will benefit the most, lead the interdisciplinary health team and maintain a fluid work with other medical specialties.

ABSTRACT

La infección por coronavirus Sars-CoV-2 desde su aparición en 2019, se ha manifestado con el compromiso multiorgánico y con la consecuente necesidad de un abordaje multidisciplinario que incluye a varios profesionales de la salud para la intervención aguda, la estabilización del paciente, la exploración neurológica y el proceso de rehabilitación. En conjunto las manifestaciones neurológicas, respiratorias y cardiovasculares son causa de discapacidad, alteración en la funcionalidad, de la participación y del entorno social en los pacientes con infección por Sars-CoV-2, por lo que requieren de una rehabilitación temprana como también de programas diseñados en torno a la pandemia y al compromiso neurológico, con el fin de prevenir complicaciones tempranas. Estas son razones por las que la presente revisión narrativa tiene como fin describir los principales compromisos del sistema nervioso central así como las secuelas neurológicas, basados en la búsqueda y referencia de diferentes tipos de artículos publicados hasta la fecha.


Sars-CoV-2 coronavirus infection since its emergence in 2019, has manifested with multiorgan involvement and the consequent need for a multidisciplinary approach that includes several health professionals for acute intervention, patient stabilization, neurological examination and rehabilitation process. As a whole, neurological, respiratory and cardiovascular manifestations are the cause of disability, alteration in functionality, participation and social environment in patients with Sars-CoV-2 infection, thus requiring early rehabilitation as well as programs designed around the pandemic and neurological involvement, in order to prevent early complications. These are the reasons why the present narrative review aims to describe the main central nervous system compromises as well as neurological sequelae, based on the search and reference of different types of articles published to date.

INTRODUCTION: Unspecified stroke (UnST) is of great importance in mortality statistics, as it is the fourth leading cause of death in Brazil. The objective of this study was to identify the profile of reclassified causes of death after investigation of deaths caused by UnST in Brazil. METHODS: All deaths registered as UnST in 2017 in the Mortality Information System (SIM) were considered as garbage codes. The specific causes, detected after investigation in 60 selected cities, were analyzed by age and sex. RESULTS: Of the total deaths due to UnST identified in these 60 cities (n = 11,289), 25.8% were investigated. Of these, 56.3% were reclassified to ischemic stroke, 12.7% to hemorrhagic stroke, and 23.3% to other specific causes, such as diabetes and chronic kidney disease, in both sexes. DISCUSSION: The higher proportion of deaths due to ischemic stroke in comparison to hemorrhagic stroke was expected. However, the detection of other specific causes outside the stroke group indicates possible quality problems in the filling of death certificate (DC). CONCLUSION: The investigations allowed the identification of subgroups of deaths due to stroke. In addition to the research, however, it is important to conduct physician training in the adequate filling in of the DC, in order to improve estimates of specific stroke mortality, and to enable appropriate targeting of health actions and services.
