ABSTRACT

La fibrosis pulmonar idiopática es una patología sobre la cual se está incidiendo tanto en el diagnóstico, a través de estudios acerca de las alteraciones genéticas que modifican el curso de la enfermedad, como en estudios que nos aportan diferentes alternativas en cuanto al tratamiento. Actualmente disponemos de dos fármacos que han demostrado mejorar tanto el pronóstico de la enfermedad como la calidad de vida de los pacientes que se someten a dichos tratamientos, ellos son Pirfenidona y Nintedanib


Idiopathic pulmonary fibrosis is a pathology that is affecting both the diagnosis, through studies about genetic alterations that modify the course of the disease and studies that provide us with different treatment alternatives. We currently have two drugs that have been shown to improve both the prognosis of the disease and the quality of life of patients who undergo such treatments, such as Pirfenidone and Nintedanib

ABSTRACT

We present three cases of pneumonia by Francisella Tularensis recently diagnosed. We also review this disease with the literature. All the studied patients were adults; two of them had epidemiological antecedents because of being in contact with hares. They present a clinical-radiological symptoms compatible with the pneumonic case described in the literature. The diagnosis was realized through serology in two cases and hemocultive in the other one. All patients had a positive answer to the antibiotic treatment, two cases with gentamicine and the other one with macrolide. There are not references about the subject in the bibliographical research we have realized in Medline. We did not find information in the Spanish base (IME), perhaps that these were the first cases found in the Spanish literature.

ABSTRACT

Presentamos tres casos de neumonía por Francisella tularensis diagnosticados recientemente y revisamos la enfermedad a través de la literatura. Todos los casos eran adultos con antecedente epidemiológico de contacto con liebres en dos de ellos, y presentaron un cuadro clínicoradiológico compatible con la forma neumónica descrita en la literatura. El diagnóstico se realizó mediante serología en dos casos y por hemocultivo en el otro. Todos los pacientes respondieron favorablemente al tratamiento antibiótico instaurado, en dos casos con gentamicina y el tercero con un macrólido. La no existencia de referencias al tema en la búsqueda bibliográfica que hemos realizado mediante Medline y en la base de datos españoles (IME) sugiere que pudiera tratarse de los primeros casos descritos en la literatura española. ABSTRACT

We reviewed the records of 125 patients admitted in our Service of Pneumology from January 1985 to December 1992, which were diagnosed of pleural or pulmonary tuberculosis. Several characteristics are described, with the following findings: 1) Higher incidence among men. 2) Relevant incidence among young people. 3) Frequent association with risk factors or concomitant diseases, mainly tobacco and alcohol consumption. 4) Delay in the diagnosis, probably due to the unspecific symptoms observed; fever, cough, constitutional syndrome. 5) The must frequent radiological findings were pleural leakage and cavitated infiltrations affecting the upper lobes. 6) Sputum microbiological exam still has a high diagnostic profitability.
