ABSTRACT

Purpose: To translate, culturally adapt, evaluate the measurement properties, and propose a new scoring system of the Cincinnati Knee Rating System for the Brazilian population.Materials and Methods: One hundred fifty people with anterior cruciate ligament injury completed Cincinnati Knee Rating System and Lysholm Knee Scoring Scale on three occasions: 1-week prior to surgery, 90-days after and 95-days after reconstruction. The measurement properties of the Brazilian-Portuguese Cincinnati Knee Rating System, internal consistency, construct validity, reproducibility, ceiling and floor effect and responsiveness, were tested.Results: Using the original scoring system (multiple scores), the internal consistency (Cronbach's α) varied between 0.54-0.79 (if item deleted = 0.07-0.73); the construct validity (Pearson's r) varied between 0.19-0.82 (related to Lysholm); the reliability (intraclass correlation coefficient) varied between 0.96-0.99; the standard error of measurement varied between 0.2-1.3 points; the minimum detectable change varied between 0.4-3.5 points; no ceiling or floor effect was detected, and responsiveness (effect size) varied between -0.3-2.7. Using the new proposed scoring system (single score), Cronbach's α was 0.78 (if item deleted = 0.65-0.71); the Pearson's r was 0.79 (related to Lysholm); the intraclass correlation coefficient was 0.99; the standard error of measurement was 0.5 points, the minimum detectable change was 1.3 points; no ceiling or floor effect was detected, and effect size was 1.4.Conclusions: The Brazilian-Portuguese Cincinnati Knee Rating System has adequate property measurement and can be used in a Brazilian population. The new proposed scoring system is appropriate.Implications for rehabilitationThe Brazilian-Portuguese Cincinnati Knee Rating System is a valid and reliable instrument which can identify consistent clinical changes over the time.The Brazilian-Portuguese Cincinnati Knee Rating System can be used to evaluate and to follow-up a Brazilian population with anterior cruciate ligament injuries and postoperative reconstruction.The new proposed score for Cincinnati Knee Rating System is appropriate and did not compromise the quality of the measurement properties.The new scoring system generates a unique score, creating a potentially simpler and faster clinical understanding of the patient's condition.

ABSTRACT

INTRODUCTION: The objective of this study was to adapt the Brazilian version, and verify the validity, reliability and internal consistency of the Oliveira questionnaire on low back pain in young people. MATERIAL AND METHODS: The questionnaire was translated from European Portuguese into Brazilian Portuguese by means of translation and re-translation. The validity of the contents was determined by experts who analyzed the clarity and pertinence of the questions. Fifteen young people aged 15 to 18 took part in the pre-test step (qualitative analysis), 40 in the test-retest (reliability) and 679 in the evaluation of internal consistency. The intra-class correlation coefficient and Spearman's correlation coefficient were used in the reliability analysis (test-retest), and Cronbach's alpha to determine the internal consistency (stability). RESULTS: In the translation phase the questionnaire was modified and considered suitable, observing similarity and equivalence of the two versions. After being corrected by the experts in the validation of the contents, the instrument was considered suitable and valid, and in the pre-test, the young people suggested some modifications to make the questionnaire more succinct. With respect to reliability, the values for the intra-class correlation coefficient were between 0.512 - acceptable and 1 - excellent and Spearman's correlation coefficient varied between 0.525 and 1, classifying the instrument as reproducible. The internal consistency was considered acceptable with a 0.757 Cronbach's alpha. DISCUSSION: The Oliveira questionnaire was choosen since it has been used in several Portuguese studies; moreover, it addresses the need to raise data regarding low back pain and associated risk factors. CONCLUSIONS: The Brazilian version of the Oliveira questionnaire on low back pain in young people showed valid and reliable cultural adaptation, with good reliability and stability.


Introdução: O objectivo deste estudo foi adaptar a versão brasileira, verificar a validade, a fiabilidade e a consistência interna do questionário de Oliveira para dor lombar em jovens. Material e Métodos: A tradução do questionário do português de Portugal para o português do Brasil foi realizado através de tradução e re-tradução. A validade de conteúdo foi realizada por experts que analisaram a clareza e pertinência das questões. Participaram das etapas de pré-teste 15 jovens (análise qualitativa), teste-reteste 40 jovens (fiabilidade) e avaliação da consistência interna 679 jovens, com idade de 15 a 18 anos. Na análise da fiabilidade (teste-reteste) usou-se o coeficiente de correlação intra-classe e o coeficiente de correlação de Spearman, na consistência interna (estabilidade) o alpha de Cronbach. Resultados: Na fase de tradução o questionário foi modificado e considerado adequado, observando-se similaridade e equivalência nas duas versões. Na validação de conteúdo, após as correções dos peritos, o instrumento foi considerado adequado e válido. Os jovens no pré-teste sugeriram modificações tornando o questionário mais sucinto. Quanto a fiabilidade, os valores para o coeficiente de correlação intraclasse foram de 0,512 - aceitável a 1 - excelente, o coeficiente de correlação de Spearman variou entre 0,525 a 1; estes valores classificam o instrumento como reprodutível. A consistência interna foi considerada aceitável alpha de Cronbach de 0,757. Discussão: O questionário Oliveira foi escolhido por ter sido utilizado em vários estudos em Portugal; além disso, atende a necessidade de levantar dados sobre dor lombar e fatores de risco associados. Conclusão: A versão brasileira do questionário de Oliveira para dor lombar em jovens tem adaptação cultural válida e fiável, com boa confiabilidade e estabilidade.

ABSTRACT

Antecedentes: la articulación de la rodilla es una de las más afectadas y lesionadas, siendo la lesión del ligamento cruzado anterior (ACL) la más frecuente entre ellas, alcanzando aproximadamente el 79% de todas las lesiones de rodilla. Esta lesión, cuando involucra atletas, causa compromisos físicos, fisiológicos y especialmente psicológicos, considerando la alta tasa de incidencia o lesión contralateral después de regresar al deporte. Este compromiso a veces aleja al atleta del deporte, o disminuye su rendimiento. Toda esta limitación o deterioro afecta directamente la calidad de vida de los atletas. Caso clínico: en el presente estudio se realizó seguimiento de una atleta femenina de balonmano durante 15 años, que fue sometida a 3 reconstrucciones del LCA, para evaluar el deterioro físico, funcional y psicológico que estas cirugías causaron a lo largo de los años. Con-clusión: se comprobó que el paciente se alejó por completo de las actividades posteriores a las cirugías, además de haber desarrollado un severo proceso de artrosis como consecuencia de un importante déficit muscular.


Background information: the knee joint is one of the most affected and injured joints in the body, with the Anterior Cruciate Ligament (ACL) injury being the most common (about 79%). Howe-ver, when this injury involves athletes, not only does it cause physical damage, but physiological and, particularly, psychological as well, as it has a high recurrence rate or provokes a contralateral injury after returning to sports. This damage sometimes steers the athlete away from sports al-together, or performance decreases. Hence, these limitations or impairments directly affect the athletesÍ´ quality of life. Case report: in this study, monitoring or a follow up of a female handball athlete who had to undergo three reconstructions of the ACL was carried for 15 years in order to evaluate the physical, functional and psychological impairment that these surgeries caused over the years. Conclusion: it can be verified that the patient completely steered away from sports activities after the surgeries and developed a severe arthrosis process as a consequence of serious muscular deficit.
