ABSTRACT

La hipotermia periódica espontánea es un trastorno caracterizado por episodios repetidos de hipotermia y diaforesis en ausencia de patología neurológica, endocrina, metabólica o procesos infecciosos intercurrentes. Puede asociar bradicardia, palidez cutánea y sensación de malestar general. La prevalencia es desconocida. Su diagnóstico es de exclusión. Debido a la escasez de casos publicados, no existe una certeza de su mecanismo patogénico. Se han descrito varias hipótesis, como epilepsia, equivalente migrañoso o alteraciones de neurotransmisores. Por todo ello, no existe un tratamiento de elección. Nuestro objetivo es presentar un caso y revisar la literatura médica existente (AU)


Spontaneous periodic hypothermia is a disorder characterized by repeated episodes of hypothermia and sweating in the absence of neurological, endocrine, metabolic injury or intercurrent infections. Bradycardia, pale skin and general malaise feeling can also be associated to this condition. Its prevalence is unknown. It can be diagnosed when other causes have been excluded. Due to the lack of published cases, its pathogenic mechanism is yet unknown. Several hypotheses, such as epilepsy, migraine equivalent or alterations of neurotransmitters, have been described. Therefore, there is not a specific treatment. Our objective is to present a case and to review the literature (AU)

ABSTRACT

El perineal groove, o surco perineal, es una malformación benigna y poco frecuente que tiende a la resolución espontánea. De forma general, se caracteriza por un surco húmedo en la línea media perineal que se extiende desde la horquilla vulvar hasta el ano. El surco perineal se presenta más frecuentemente en niñas, aunque también se ha descrito en varones. Se ignora su incidencia real, debido en parte al desconocimiento del mismo por parte de los profesionales. Presentamos el caso de una recién nacida de 2 días de vida en la que se detecta una lesión lineal en el periné. Es importante su reconocimiento dado que entraña un amplio diagnóstico diferencial de lesiones, incluidas algunas con importantes connotaciones legales por la zona en la que se presenta (AU)


The perineal groove, or perineal sulcus, is an uncommon and benign anomaly which tends to resolve spontaneously. In general, it has been described as a wet sulcus in the midline perineum which extends from the fourchette to the anus. Although the perineal groove appears more frequently in girls, it has been described in males too. The incidence of perineal groove is ignored, it is due to the lack of knowledge of it by health professionals. We report one case of a 2-days-old baby girl. There was found a lineal lesion in the perineum during her clinical examination. Its recognition is important since it involves a wide differential diagnosis of injures, including some of them with important legal implications because of the area in which it is presented (AU)

ABSTRACT

El denominado «juego de la asfixia» es una práctica potencialmente mortal entre los adolescentes, que parece estar en auge en los últimos años. Hasta ahora, se han descrito pocos casos en nuestro país. Se lleva a cabo mediante la aplicación de una fuerte presión en el cuello hasta la pérdida de conciencia durante unos segundos, consiguiendo así una breve euforia. Presentamos el caso de un niño de 10 años que fue encontrado inconsciente ahorcado, mientras jugaba en un parque. En ocasiones, el límite entre la euforia y la muerte cerebral es muy pequeño, pudiendo acarrear graves consecuencias. De ahí la importancia de que los padres, los educadores y los profesionales de la salud se familiaricen con los signos del juego, sean capaces de detectarlo precozmente y ayuden a promover medidas eficaces para su prevención (AU)


The so-called «choking game» is a potentially lethal practice among adolescents, which seems to be booming in the last years. Until now, only a few cases have been reported in our country. It consists in applying strong pressure to the neck until achieving a loss of consciousness for a few seconds, looking for a brief feeling of euphoria. The case of a 10-years-old boy who was found unconscious and hanged in a park while playing is presented. Frequently, the line between euphoria and brain death is very narrow, so it could trigger serious health consequences. Therefore, it is important that parents, educators and health professionals are familiar with the signs of the game, are able to recognize it early and help to promote effective measures for prevention (AU)

ABSTRACT

La enumatosis intestinal en el recién nacido es un signo que típicamente se asocia a la presencia de enterocolitis necrotizante. Presentamos el caso de una recién nacida que debutó con sangre en las deposiciones, aerobilia y neumatosis intestinal en colon izquierdo, sin distensión de asas, diagnosticándose de colitis alérgica en base a la clínica, laboratorio, imágenes y respuesta a tratamiento. La supresión de la proteína de la leche de vaca en la dieta de la madre y el complemento con un sustituto libre de esta proteína, llevó a la resolución del cuadro (AU)


Pneumatosis intestinalis is newly born infants constitutes a clinical manifestation greatly associated with the presence of necrotizing enterocolitis. We provide a newborn´s case presenting with bloody stools, portal venous gas and left colon pneumatosis intestinalis, being diagnosed of allergic colitis based on clinical records, lab-tests, imaging and treatment responses. The suppression of cow´s milk protein in the maternal diet and the replacement of milk by a substitute free form that protein led to the complete resolution (AU)

ABSTRACT

Objetivos : Revisar las comunicaciones científicas presentadas en los congresos de la Asociación Española de Nefrología Pediátrica (AENP) . Material y métodos : En los programas científicos (1988-2007) de los congresos de la AENP se revisaron: número de presentaciones ,centros participantes y con el mayor número de comunicaciones , forma de presentación, estudios experimentales y temas elegidos. Resultados: En los últimos20 años, 91 centros presentaron 1.119 comunicaciones. El Hospital La Paz (Madrid) fue el que más comunicaciones presentó. Desde el año 1995 comenzaron a admitirse comunicaciones tipo póster y 369 de las 815 comunicaciones presentadas tuvieron ese formato. Dieciséis comunicaciones informaron de investigación animal. El tema más frecuente fue la enfermedad glomerular (203). Se presentaron 51 comunicaciones sobre diálisis. Trataron sobre trasplante renal 123 comunicaciones. Sólo una comunicación sobre genética fue presentada antes de 1998. Conclusiones : El formato póster es un método útil para las presentaciones científicas . El tema más habitual fue la enfermedad glomerular. En la última década han aparecido comunicaciones sobre genética, pero sobre experimentación animal son todavía excepcionales (AU)

Ninety-one institutions participated in the meetings, 17/year between 88-92 and 34/year between 03-07. Pediatric Nephrology unit from the H. La Paz (Madrid)contributed the most presentations. Poster presentations were accepted at the ANEP meetings after 1995. Since then,369 of the 815 presentations followed this format. Between88-07 only 16 presentations dealt with experimental nephrology. The most common topics of presentation waere glomerular disease (203) and urinary tract infection/VUR(132). Fifty-one presentations dealt with dialysis (almost2/3 peritoneal). Transplantation was the topic of 123presentations. Of the 21 presentations on molecular genetics only one happened before 1998. Conclusions: The poster is a useful alternative in scientific presentations which has allowed an increase in presentations, authors and institutions participating in the ANEP meetings. The main topic of presentation was glomerular disease. The frequency of presentations dealing with transplantation has increased in the last years. The past decade has seen more presentations on molecular genetics, but presentations dealing with experimental nephrology are still infrequent (AU)

ABSTRACT

Introducción: La presentación de convulsiones benignas febriles durante una gastroenteritis leve sin deshidratación ni fiebre ni alteraciones electrolíticas es una asociación descrita con relativa frecuencia en Asia, siendo más desconocida fuera del continente asiático. Casos clínicos: Se describen las características de cuatro casos de lactantes que ingresaron en nuestro hospital por presentar convulsiones a febriles asociadas a gastroenteritis agudas. En tres de los cuatro casos se identificó la presencia de Rotavirus en heces y en todos los casos la evolución ha sido favorable. Comentarios: Probablemente, esta entidad es infradiagnosticada en nuestro país. Reconocer esta entidad debe servirnos para evitar tratamientos antiepilépticos prolongadose intensos (AU)


Introduction: The presentation of benign seizures afebriles in association a slight gastroenteritis without dehydration or fever or electrolytic imbalance is an association described with relative frequency in Asia, not being more known out of the Asian continent. Case reports: There are described the characteristics off our cases who joined our hospital for presenting convulsions afebriles associated with gastroenteritis acute. In three of four cases Rotavirus’s presence was identified in dregs and in all the cases the evolution has been favorable. Conclusions: Probably this entity is infradiagnosticada in our country. To recognize ourselves this entity must serve to avoid anti-epileptic long and intense (AU)
