A retrospective study was undertaken to determine the importance of performing noninvasive blood flow studies on the diabetic patient population prior to undergoing elective foot surgery. Thirty diabetic patients, contemplating elective foot surgery, underwent noninvasive blood flow studies. Eighty-seven percent of these patients, with ankle/brachial indices greater than 0.6, successfully healed after surgery was performed.

Identification and management of the acutely fractured ankle is discussed by the authors. The Lauge Hansen classification system, and in particular, supination-external rotation injuries, is evaluated. Intraoperative technique, perioperative considerations, and generalized management of these pathologic conditions are reviewed, according to the authors' experiences.

The Achilles tendon represents a very important structure in human locomotion. Pathologic afflictions of this tendon may lead to serious problems if not recognized and treated appropriately. Drs. Reinherz, Granoff, and Westerfield briefly review normal tendon anatomy and function. The mechanisms of tendon injury with clinical recognition of pathology, both objective and subjective, are then discussed. Proper management of the compromised tendo Achillis is presented.

Pathologic conditions behind the calcaneus are common, and may initiate during early childhood. The authors discuss osseous and soft tissue conditions involving the posterior-superior surface of the heel. Radiographic and clinical evaluation is considered. Treatment alternatives are also reviewed.
