ABSTRACT

Adolescent pregnancy is a major public health issue with profound implications for health and socioeconomic factors. The use of long-acting reversible contraception (LARC) could be an interesting strategy to reduce the unintended pregnancy rate. However, the cost of LARC is still a barrier to widespread adoption. This study aimed to analyze the effectiveness and economic impact of LARC compared with non-LARC methods in preventing unintended pregnancy among adolescent girls. This systematic review was registered in PROSPERO (CRD42023387735) and conducted following the PRISMA guidelines. We included articles covering adolescents aged 10-19 years without language restrictions that evaluated the use of LARC compared with non-LARC in terms of effectiveness and the public health costs of unintended pregnancy. The search for articles included the databases MEDLINE/PubMed, Cochrane Library, Embase, and Lilacs, using the entry terms "Adolescent" and "Long-Acting Reversible Contraception." We evaluated the risk of bias and the certainty of the evidence for each outcome of interest. The search retrieved a total of 1,169 articles and, after the title and abstract, we identified 40 articles for full-text analysis. Out of the 40 studies evaluated, 4 articles met the eligibility criteria for cost evaluation, and 1 met the eligibility criteria for effectiveness as an outcome. In conclusion, LARC emerges as the most effective and cost-effective contraceptive method. The cost of utilizing LARC, especially the copper IUD, is significantly lower than the costs attributable to unintended pregnancies in adolescence.

ABSTRACT

Objetivo: avaliar o uso da internet pelo paciente no acesso a informações sobre saúde e possíveis implicações na relação médico-paciente. Método: estudo descritivo, transversal e quantitativo, realizado entre abril e maio de 2022, em Anápolis, Goiás. Utilizou-se questionário validado sobre o uso da internet. Aplicou-se o teste G para associação do uso da internet em relação ao sexo e a faixa etária, com nível de significância de 5% (p<0,05). Resultados: entre 109 pacientes prevaleceu o sexo feminino (80,7%), 18 a 59 anos (83,5%) e ensino médio completo (42,2%). A maioria fica entre 2 e 4 horas por dia na internet (68,0%; p=0,992) e já pesquisou informações de saúde na internet (84,4%; p=0,095), especialmente sobre medicamentos (86,2%; p=0,940). Ambos os sexos usam a internet para uma segunda opinião sobre o diagnóstico médico (homens - 52,4%; mulheres - 68,2%; p=0,185), mas não discutem com os médicos suas pesquisas realizadas na internet (homens ­ 57,1%; mulheres - 86,4%; p=0,005). Conclusão: a internet vem sendo utilizada com bastante frequência para acesso à informação sobre saúde pelos pacientes, porém estes não discutem suas descobertas com o profissional, o que tende a fragilizar a relação médico-paciente.


Objective: to evaluate the use of the internet by patients to access health information and the possible implications for the physician-patient relationship. Method: descriptive, cross-sectional and quantitative study carried out in April and May 2022 in Anápolis, Goiás. A validated questionnaire on internet use was used in the study. The G test was applied to associate internet use with sex and age group, with a significance level of 5% (p<0.05). Results: among the 109 patients, there was a prevalence of females (80.7%), aged 18 to 59 years (83.5%), with complete high school (42.2%). Most spend between two and four hours a day on the Internet (68.0%; p=0.992) and had already searched for health information on the Internet (84.4%; p=0.095), especially about medications (86.2%; p=0.940). Participants of sexes used the internet for a second opinion on the medical diagnosis (men - 52.4%; women - 68.2%; p=0.185), but did not discuss their searches carried out on the internet with the doctors (men - 57, 1%; women - 86.4%; p=0.005). Conclusion: the Internet has been used quite frequently by patients to access health information, but they do not discuss their findings with professionals, which tends to weaken the doctor-patient relationship.

ABSTRACT

We report the sequencing and assembly of the PH8 strain of Leishmania amazonensis one of the etiological agents of leishmaniasis. After combining data from long Pacbio reads, short Illumina reads and synteny with the Leishmania mexicana genome, the sequence of 34 chromosomes with 8317 annotated genes was generated. Multigene families encoding three virulence factors, A2, amastins and the GP63 metalloproteases, were identified and compared to their annotation in other Leishmania species. As they have been recently recognized as virulence factors essential for disease establishment and progression of the infection, we also identified 14 genes encoding proteins involved in parasite iron and heme metabolism and compared to genes from other Trypanosomatids. To follow these studies with a genetic approach to address the role of virulence factors, we tested two CRISPR-Cas9 protocols to generate L. amazonensis knockout cell lines, using the Miltefosine transporter gene as a proof of concept.

ABSTRACT

ntrodução: a própolis é um produto natural que apresenta inúmeras propriedades terapêuticas, dentre elas a ação cicatrizante e anti-inflamatória. Diversos estudos têm sugerido o seu emprego no manejo da mucosite oral (MO) e de lesões ulceradas em mucosa bucal. A MO é uma inflamação da mucosa oral, resultante do tratamento quimio e/ou radioterápico. Já as lesões ulceradas caracterizam-se como um distúrbio ulcerativo inflamatório doloroso. Objetivo: discutir a ação da própolis sobre a prevenção e cicatrização de lesões de origem não infecciosa que acometem a cavidade oral. Metodologia: trata-se de uma revisão integrativa da literatura em que foram utilizadas as bases de dados LILACS, PubMed, SciELO e Cochrane, por meio do cruzamento dos descritores em português: "própolis", "úlceras orais" e "mucosite oral"; e em inglês: "propolis", "oral ulcer" e "mucositis". Os seguintes critérios de inclusão foram estabelecidos: ensaios clínicos e revisões sistemáticas, na íntegra, escritos em inglês ou português, entre 2005 e 2018, que utilizaram a própolis de forma tópica ou sistêmica. Resultados: foram incluídos um total de 10 estudos, onde 2 abordaram o uso da própolis em úlceras orais e 8 tiveram como foco a aplicação deste agente no manejo da MO. Quanto ao desfecho, a aplicação da própolis na mucosite se mostrou eficaz em 7 dos 8 estudos. Já se tratando de úlceras orais, a administração deste agente foi efetiva nos 2 estudos. Conclusão: os estudos analisados demostraram que a própolis apresenta propriedades capazes de favorecer a prevenção e cicatrização de lesões de MO e úlceras orais.


Introduction: propolis is a natural product that has numerous therapeutic properties, including healing and anti-inflammatory action. Several studies have suggested its use in the management of oral mucositis (OM) and ulcerated lesions in the oral mucosa. OM is an inflammation of the oral mucosa resulting from chemotherapy and/or radiotherapy. Whereas ulcerated lesions are characterized as a painful inflammatory ulcerative disorder. Objective: to discuss the action of propolis on the prevention and healing of non-infectious lesions that affect the oral cavity. Methodology: this is an integrative literature review in which LILACS, PubMed, SciELO and Cochrane databases were used, by crossing descriptors in Portuguese: "própolis", "úlceras orais" and "mucosite oral"; and in English: "propolis", "oral ulcer" and "mucositis". The following inclusion criteria were established: clinical trials and systematic reviews, in full, written in English or Portuguese, between 2005 and 2018, which used propolis topically or systemically. Results: a total of 10 studies were included, where 2 addressed the use of propolis in oral ulcers and 8 focused on the application of this agent in the management of OM. As for the outcome, the application of propolis in mucositis proved to be effective in 7 of the 8 studies. As for oral ulcers, the administration of this agent was effective in both studies. Conclusion: the analysed studies demonstrated that propolis has properties capable of help the prevention and healing of OM lesions and oral ulcers.

ABSTRACT

Sodium butyrate-loaded nanoparticles coated chitosan (NaBu-loaded nanoparticles/CS) were developed to treat the choroidal neovascularization in wet age-related macular degeneration (AMD). The nanoparticles were produced by double emulsification and solvent evaporation technique, optimized by experimental statistical design, characterized by analytical methods, investigated in terms of in vitro and in vivo ocular biocompatibility, and evaluated as an antiangiogenic system in vivo. The NaBu-loaded nanoparticles/CS were 311.1 ± 3.1 nm in diameter with a 0.208 ± 0.007 polydispersity index; had a +56.3 ± 2.6 mV zeta potential; showed a 92.3 % NaBu encapsulation efficiency; and sustained the drug release over 35 days. The NaBu-loaded nanoparticles/CS showed no toxicity to human retinal pigment epithelium cells (ARPE-19 cells); was not irritant to the chorioallantoic membrane (CAM); did not interfere in the integrity of the retinal layers of rat's eyes, as detected by the Optical Coherence Tomography and histopathology; and inhibited the angiogenesis in CAM assay. The NaBu-loaded nanoparticles/CS could be a therapeutic alternative to limit the neovascularization in AMD.

ABSTRACT

Scaling and root planing is the gold standard procedure in the periodontal disease treatment. Although this therapy has the ability to reduce or eliminate periodontopathogens mthrough periodontal instrumentation, sometimes the response to treatment may not promote the complete eradication of pathogens, affecting the healing process and leading to the presence of residual periodontal pockets. The aim of this study was to analyze the effectiveness of photodinamic therapy with laser photobiomodulation and as an adjunct to scaling and root planing (having as a conductive agent a fiber optic tip radiating inside the periodontal pocket), compared to scaling and root planing alone in patients with periodontitis on randomized trials. This systematic review was carried out complying with the recommendations of the Preferred Reporting Items for Systematic Reviews and Meta Analyses. The databases used were Pubmed, Lilacs, Biblioteca Virtual em Saúde - Odontologia and Medline. "Periodontal Disease" OR "Periodontal Treatment" AND m"Photodinamic Theraphy" were the descriptors used. A total of five articles were included. Two studies have shown superior results in relation to the referred association; a study indicated benefits in biochemical and/or microbiological aspects; it suggests, however, that clinical relevance needs to be better evaluated; two studies did not reveal benefits that categorize photodynamic therapy + scaling and root planning as superior to conventional treatment. The association in question, when well standardized and performed, can result in promising responses in the control of periodontal disease; however, the clinical benefits that demonstrate its superiority will certainly be subject to new researches. A raspagem e alisamento radicular é o procedimento padrão-ouro no tratamento da doença periodontal. Embora essa terapia seja capaz de reduzir ou eliminar os periodontopatógenos por meio da instrumentação periodontal, algumas vezes a resposta ao tratamento pode não promover completa erradicação dos patógenos, afetando assim, o processo de cicatrização e levando à presença de bolsas periodontais residuais. O objetivo do presente estudo foi analisar a eficácia da terapia fotodinâmica com fotobiomodulação laser adjuvante a raspagem e alisamento radicular (tendo como agente condutor uma ponta de fibra óptica que irradia o interior da bolsa periodontal), em comparação com a raspagem e alisamento radicular isoladamente em pacientes com periodontite, avaliados em estudos clínicos randomizados. Esta revisão sistemática foi realizada de acordo com as recomendações Principais Itens para Relatar Revisões sistemáticas e Meta-análises. As bases de dados utilizadas foram Pubmed, Lilacs, Biblioteca Virtual em Saúde - Odontologia e Medline. "Doença periodontal" OR "Tratamento periodontal" AND "Terapia fotodinâmica" foram os descritores utilizados. Um total de cinco artigos foram incluídos. Dois estudos mostraram resultados superiores em relação à referida associação; um estudo indicou benefícios nos aspectos bioquímicos e/ou microbiológicos; sugere, no entanto, que a relevância clínica precisa ser melhor avaliada; dois estudos não revelaram benefícios que categorizassem a terapia fotodinâmica + raspagem e alisamento radicular como superiores ao tratamento convencional. A associação em questão, quando bem padronizada e realizada, pode resultar em respostas promissoras no controle da doença periodontal; entretanto, os benefícios clínicos que demonstram sua superioridade certamente serão alvo de novas pesquisas. ABSTRACT

Background: Nitazoxanide exerts antiviral activity in vitro and in vivo and anti-inflammatory effects, but its impact on patients hospitalized with COVID-19 pneumonia is uncertain. Methods: A multicentre, randomized, double-blind, placebo-controlled trial was conducted in 19 hospitals in Brazil. Hospitalized adult patients requiring supplemental oxygen, with COVID-19 symptoms and a chest computed tomography scan suggestive of viral pneumonia or positive RT-PCR test for COVID-19 were enrolled. Patients were randomized 1:1 to receive nitazoxanide (500 mg) or placebo, 3 times daily, for 5 days, and were followed for 14 days. The primary outcome was intensive care unit admission due to the need for invasive mechanical ventilation. Secondary outcomes included clinical improvement, hospital discharge, oxygen requirements, death, and adverse events within 14 days. Results: Of the 498 patients, 405 (202 in the nitazoxanide group and 203 in the placebo group) were included in the analyses. Admission to the intensive care unit did not differ between the groups (hazard ratio [95% confidence interval], 0.68 [0.38-1.20], p = 0.179); death rates also did not differ. Nitazoxanide improved the clinical outcome (2.75 [2.21-3.43], p < 0.0001), time to hospital discharge (1.37 [1.11-1.71], p = 0.005), and reduced oxygen requirements (0.77 [0.64-0.94], p = 0.011). C-reactive protein, D-dimer, and ferritin levels were lower in the nitazoxanide group than the placebo group on day 7. No serious adverse events were observed. Conclusions: Nitazoxanide, compared with placebo, did not prevent admission to the intensive care unit for patients hospitalized with COVID-19 pneumonia. INTRODUCTION: Older adults present a higher risk of suicide, and Brazil is experiencing a fast population aging. To understand the impact of demographic transition, we compared Brazilian suicide mortality rates (MR) among adults (50+ years) with global rates, those from one high-income country, and those from one middle-income country. Looking for regional disparities, the MR was analyzed among older adults (60+ years) by Brazilian states. METHODS: This was an ecological study based on estimates from the Global Burden of Disease Study, from 2000 to 2019. Age-standardized MR and age-specific MR per 100,000 inhabitants were described, with 95% uncertainty intervals (UI). RESULTS: During the period, the annual estimates and the declining trend in mortality were higher in the world than in the studied countries. In 2019, global age-standardized MR was 9.39 (95% UI 8.48-10.29), compared to 5.68 (95% UI 5.40-6.19), 6.01 (95% UI 5.10-7.04), and 6.63 (95% UI 6.43-6.95) in Brazil, Mexico, and England, respectively. In Brazil, despite a significant decline in national rates, stability was observed in 15 states. An increase in aging was only found for men, who presented 3-4 times higher MR than women. The states' rates presented large differences: in 2019, the rates among men aged 60-64 years varied from 7.24 (95% UI 5.31; 9.85) to 26.32 (95% UI 20.21; 34.50). CONCLUSIONS: The smaller decline in suicide mortality among older Brazilian adults, the increasing risk with aging, and the higher mortality among men indicate the need for specific prevention policies. The variation within states suggests differences in the data quality or in socio-cultural and historical aspects, which requires further investigation.

ABSTRACT

Bioinformatics is an important tool in several areas of science, and it can be used for the reclassification of organisms from the Archaea and Bacteria kingdoms. Using tools to calculate the average nucleotide identity, gene annotation and pan-genome analysis, it can be identified possible inconsistencies in the classification of bacteria. It was made a massive analysis of genomic data from two bacterial genera of medical interest Brucella and Micrococcus responsible for causing chronic diseases and nosocomial infections, respectively. The methodology used was nucleotide similarity analysis, genomic annotation, pan-genome analysis and annotation of genes of interest. In this study we propose reclassifications for both genera studied and we point out proposals for new poorly classified species and others not yet identified.


A bioinformática é uma importante ferramenta em diversas áreas da ciência, podendo ser usada para a reclassificação de organismos dos reinos Archaea e Bacteria. A partir do uso de ferramentas de cálculo da identidade média de nucleotídeos, anotação de genes, análise pan-genoma podem ser identificadas possíveis inconsistências na classificação de bactérias. Nesse trabalho, foi realizada uma análise massiva de dados genômicos de dois gêneros bacterianos de interesse médico, Burcella e Micrococcus, responsáveis por causar doenças crônicas e infecções hospitalares, respectivamente. A metodologia utilizada foi a de análise de similaridade nucleotídica, anotação genômica, análise de pan-genoma e anotação de genes de interesse. Neste estudo propomos reclassificações para ambos os gêneros estudados e apontamos propostas de novas espécies classificadas erroneamente e de outras ainda não identificadas.

ABSTRACT

ABSTRACT Wound healing consists of the coordinated cascade of molecular, cellular and biochemical events that interact for tissue recomposition to occur. The interruption or failure in this process can trigger the delay in closing the lesion and chronic wounds, in addition to infections. Ozonotherapy proves to be a promising alternative, capable of assisting in the tissue healing process, modulating the inflammatory response and preventing the development of infection. To present, analyze and discuss, through a literature review, the use of Ozonated Oils in the repair of the human body and oral cavity systems. SciELO, LILACS, MEDLINE and PubMed databases were consulted, using the following DeCS / MeSH descriptors: "ozonized oil"; "healing"; "ozone therapy". After crossing, counting and analyzing, 13 studies were selected, according to the theme. Due to the complementary theoretical foundation, 44 articles were included, totaling 57 articles used for the present review. Ozonized Oil therapy has beneficial clinical effects, which include antimicrobial, immunostimulating, analgesic, antipoxic, immunoregulatory and antioxidant action. Topical management of ozone has proven effective for the treatment of foot-and-mouth disease, gingivitis, ulcers and dermatitis, contributing to pain relief and healing. Infectious skin diseases, abscesses, allergic skin diseases, scaly erythema, psoriasis and palmoplantar pustulosis are pathological conditions, the treatment of which benefits from the application of Ozonized Oils. Ozonized Oils have been shown to be effective in the treatment of local infections, when in appropriate formulations and in controlled cases, being a promising therapeutic alternative for tissue repair.


RESUMO A cicatrização de feridas consiste na coordenada cascata de eventos moleculares, celulares e bioquímicos que interagem para que ocorra a recomposição tecidual. A interrupção ou falha nesse processo, pode desencadear o atraso no fechamento da lesão e feridas crônicas, além de infecções. A Ozonioterapia revela-se uma alternativa promissora, capaz de auxiliar no processo de cicatrização tecidual, modular a resposta inflamatória e prevenir o desenvolvimento de infecção. Apresentar, analisar e discutir, através de uma revisão da literatura, o emprego de Óleos Ozonizados no reparo dos sistemas do corpo humano e cavidade oral. Bases de dados SciELO, LILACS, MedLine e PubMed foram consultadas, utilizando os seguintes descritores DeCS/MeSH: "ozonized oil"; "healing"; "ozone therapy". Após cruzamento, apuração e análise, foram selecionados 13 estudos, de acordo com a temática. Devido à fundamentação teórica complementar, 44 artigos foram incluídos, totalizando 57 artigos utilizados para a presente revisão. A terapia com Óleo Ozonizado possui efeitos clínicos benéficos, que incluem ação antimicrobiana, imunoestimulante, analgésica, antipóxica, imunorreguladora e antioxidante. O manejo tópico de ozônio revelou-se eficaz para o tratamento de estomatite aftosa, gengivite, úlceras e dermatite, contribuindo para o alívio da dor e cicatrização. Doenças infecciosas da pele, abscessos, doenças alérgicas da pele, eritema escamoso, psoríase e pustulose palmoplantar são condições patológicas, cujo tratamento é beneficiado pela aplicação de Óleos Ozonizados. Os Óleos Ozonizados têm demonstrado eficácia no tratamento de infecções locais, quando em formulações apropriadas e em casos controlados, sendo uma alternativa terapêutica promissora para o reparo de tecidos.

ABSTRACT

Introduction: The inhibition of osteoclastic activity, associated with different treatment modalities in patients with head and neck cancer, make bones unable to respond to repair processes related to physiological traumas or infection and may result in bone necrosis. The present study aims to report a clinical case of osteoradionecrosis in mandible, and how its sequels were controlled by ozonotherapy. Case report: A 73-year-old female patient with infiltrative ductal breast carcinoma with mandibular metastasis was submitted to chemo and radiotherapy at head and neck region associated with bisphosphonate. Three years later, she was diagnosed with hemimandibular osteoradionecrosis that exhibited communication with oral cavity and with a chronic, suppurative and persistent associated infection. It was applied adjuvant therapy with ozone through the cutaneous fistula and the exposed and necrotic bone. Additionally, non-vital bone debridement was proceeded in two surgical steps. The patient is after a 1-year follow-up non-symptomatic. Conclusion: It is supposed that ozonotherapy, due to its antibacterial and immunoregulatory mechanism of action, was an important therapeutic agent for improving the patient's quality of life.


Introdução: A inibição da atividade osteoclástica, associada a diversas modalidades de tratamento utilizadas em pacientes com câncer de cabeça e pescoço, torna o osso incapaz de responder aos processos de reparo relacionados a traumas fisiológicos ou à infecção, e pode resultar em necrose óssea. O presente estudo tem como objetivo relatar um caso clínico de osteonecrose em mandíbula como consequência do uso de bisfosfonato associado à quimioterapia e à radioterapia para tratamento de câncer de mama com metástase para mandíbula, tendo suas sequelas controladas por meio do uso da ozonioterapia. Relato do caso: Paciente do sexo feminino, 73 anos, com história de carcinoma ductal infiltrante de mama com metástase óssea em mandíbula, a qual foi submetida ao tratamento de quimioterapia e radioterapia em região de cabeça e pescoço; três anos depois, foi diagnosticada com osteonecrose da hemimandíbula direita com exposição completa para a cavidade bucal e infecção crônica, supurativa e persistente. Foi realizada terapia adjuvante com aplicação de ozônio nas fístulas cutâneas e no remanescente ósseo exposto e necrosado, além do desbridamento dos sequestros ósseos em duas etapas cirúrgicas. Paciente encontra-se em acompanhamento há 1 ano, sem sintomatologia associada. Conclusão: Observou-se que a ozonioterapia, em razão da sua ação antibacteriana e cicatrizante, foi um importante agente terapêutico para a melhora da qualidade de vida da paciente.


Introducción: La inhibición de la actividad osteoclástica, asociada con las diversas modalidades de tratamiento utilizadas por los pacientes con cáncer de cabeza y cuello, hace que el hueso no pueda responder a los procesos de reparación relacionados con traumas o infecciones fisiológicas y puede provocar necrosis ósea. El presente estudio tiene como objetivo informar un caso clínico de osteonecrosis mandibular como consecuencia del uso de bisfosfonato asociado con quimioterapia y radioterapia para tartar el cáncer de mama con mandíbula metastásica, controlando sus secuelas mediante el uso de la terapia con ozono. Relato del caso: Paciente de 73 años con antecedentes de carcinoma ductal mamario infiltrante con metástasis en la mandíbula ósea fue sometida a quimioterapia y radioterapia en la región de la cabeza y el cuello. Tres años después, le diagnostica con osteonecrosis hemimandibular derecha con exposición completa a la cavidad oral e infección crónica, supurativa y persistente. La terapia adyuvante se realizó con la aplicación de ozono en las fístulas cutáneas y en el remanente óseo expuesto y necrótico, además del desbridamiento de los secuestros óseos en dos etapas quirúrgicas. El paciente ha estado bajo seguimiento durante 1 años in síntomas asociados. Conclusión: Debido a su acción antibacteriana y curativa, la ozonioterapia fue un importante agente terapéutico para mejorar la calidad de vida del paciente.

OBJECTIVE: The aim of this study was to verify the existence of conscientious objection to comprehensive health care for the victim of sexual violence, as well as to understand the service structure of institutions authorized in the health care system for victims of sexual violence in the state of Minas Gerais. METHODS: This is a quantitative, cross-sectional, descriptive, and analytical field study aiming to collect data from institutions authorized to assist victims of sexual violence in the state. The instrument was handed in to the coordinators of these services. RESULTS: It was found that 11% have no physician in service and that 31% had no training for this type of care. It was revealed that 85% of these institutions have already encountered patients wishing to have a legal abortion, but 83% of them have not had their request granted. There was a 60% presence of conscientious objection by the entire medical team, the main reason being religious (57%). CONCLUSION: The assistance system is not prepared for comprehensive care for victims of sexual violence, especially in terms of legal abortions, with conscientious objection being the main obstacle. A functional referral and counter-referral system is needed to alleviate such a serious and evident problem. It is hoped that the research results will promote dialogues in the state that favor appropriate actions on legal abortion, and respect the medical professional, in case of conscientious objection.

Espera-se que o resultado da pesquisa crie espaços de diálogos dentro do estado que favoreçam ações adequadas sobre o aborto legal, e o profissional médico respeitado, se houver objeção de consciência.

ABSTRACT

BACKGROUND: The Zika virus outbreak in Brazil (2015-2016) affected thousands of children who were born with Congenital Zika Syndrome (CZS). Families play an important role in their care of children with complex needs, yet their knowledge, experience and skills are rarely harnessed in existing interventions to best support these families. OBJECTIVE: This study explores the use of mothers as facilitators for a community-based group intervention for children with CZS and their caregivers in Brazil. METHODS: Four facilitators were trained to deliver the 10-week intervention called "Juntos". Two were mothers of a child with CZS ("expert mothers") and two were therapists (speech therapist and physiotherapist). The intervention was delivered to three groups, generally including 8-10 caregivers. Two researchers, who were psychologists, observed the groups and held focus group discussions at the end of each session. They undertook semi-structured interviews post intervention with a purposive sample of caregivers, and with the facilitators. Observation notes were collated and summarised. Transcripts were transcribed and thematically analysed using five elements to assess feasibility: acceptability, demand, implementation, practicality and adaptation. RESULTS: The use of expert mothers as facilitators was considered to be acceptable and there was demand for their role. Their experiential knowledge was viewed as important for sharing and learning, and supporting and encouraging the group. The intervention was delivered with fidelity by the expert mothers. The practicality of the intervention was facilitated by holding the group sessions in the community, providing transport costs to facilitators and participants, paying expert mothers and therapist facilitators equally and supporting the expert mothers through a mentorship programme. Equal payment with the therapist enabled the expert mothers to better facilitate the groups, through increased confidence in the value of their role. Adaptation of the intervention included development of video resources and mentoring guidelines. CONCLUSION: The use of expert mothers as facilitators of caregiver groups provides a unique approach to harness the knowledge, experience, and skills of families to provide care, and is likely to be feasible in similar contexts.

Pragmatic inferences are one way to study false memories in real-world situations. We aimed to investigate variances in responses to pragmatic implication sentences between Portuguese and American data, presenting, for the first time, normative data of cued recall and recognition for pragmatic implication sentences in Portuguese. In Study 1 we analyzed cued-recall data for Portuguese pragmatic sentences. The proportions of cued- recall for correct and inference responses of each sentence did not significantly correlate with the values of American normative data. In Studies 2a and 2b we analysed forced-recognition data for pragmatic sentences, one with American participants and English sentences (Study 2a) and the other with Portuguese participants and Portuguese sentences (Study 2b). Moreover, two conditions of sentences presentations were applied to eventually detect an influence of the sentences' format, which was not find in both studies. The levels of recognition for correct and inferences were very similar between those two studies but the correlation, sentence by sentence, was low. Together, these results suggest an important recommendation for further studies - normed material for a specific language/culture is a crucial factor to be considered when conducting research on pragmatic inferences

ABSTRACT

Deep uncertainty in future climatic and economic conditions complicates developing infrastructure designed to last several generations, such as water reservoirs. In response, analysts have developed multiple robust decision frameworks to help identify investments and policies that can withstand a wide range of future states. Although these frameworks are adept at supporting decisions where uncertainty cannot be represented probabilistically, analysts necessarily choose probabilistic bounds and distributions for uncertain variables to support exploratory modeling. The implications of these assumptions on the analytical outcomes of robust decision frameworks are rarely evaluated, and little guidance exists in terms of how to select uncertain variable distributions. Here, we evaluate the impact of these choices by following the robust decision-making procedure, using four different assumptions about the probabilistic distribution of exogenous uncertainties in future climatic and economic states. We take a water reservoir system in Ethiopia as our case study, and sample climatic parameters from uniform, normal, extended uniform, and extended normal distributions; we similarly sample two economic parameters. We compute regret and satisficing robustness decision criteria for two performance measures, agricultural water demand coverage and net present value, and perform scenario discovery on the most robust reservoir alternative. We find lower robustness scores resulting from extended parameter distributions and demonstrate that parameter distributions can impact vulnerabilities identified through scenario discovery. Our results suggest that exploratory modeling within robust decision frameworks should sample from extended, uniform parameters distributions.

ABSTRACT

O artigo tem por objetivo problematizar o trabalho realizado nos serviços substitutivos de saúde mental, enfatizando os Centros de Atenção Psicossocial (CAPS), que se baseiam nos preceitos da Reforma Psiquiátrica. Por intermédio de fragmentos de um relato do caso de uma jovem acompanhada pelo serviço, tomaremos a psicanálise como eixo condutor de nossa discussão e, como método de interlocução com a realidade encontrada no serviço, a psicanálise aplicada. Procederemos a uma discussão acerca da escuta do sujeito psicótico, seus impactos no trabalho da equipe, bem como as consequências da proposta de inclusão social, na construção do laço social dos usuários do serviço. Conclui-se que os princípios norteadores da luta antimanicomial, em especial a antinomia exclusão/inclusão, desconsideram a noção de sujeito no contexto da prática nos serviços substitutivos. Propõe-se que o trabalho em equipe, no manejo da clínica da psicose, considere a escuta clínica e a construção singular que cada sujeito encontra para lidar com o sofrimento.


The article aims to problematize the work performed in the substitutive services of mental health, emphasizing the Psychosocial Care Centers (CAPS), which are based on the precepts of the Psychiatric Reform. Through fragments of an account of the case of a young woman accompanied by the service, we will take psychoanalysis as the guiding axis of our discussion and, as a method of interlocution with the reality found in the service, applied psychoanalysis. We will discuss the listening of the psychotic subject, their impact on the work of the team, as well as the consequences of the social inclusion proposal, in the construction of the social bond of the users of the service. It is concluded that the guiding principles of anti-asylum control, especially the exclusion / inclusion antinomy, disregard the notion of subject in the context of practice in substitutive services. It is proposed that the teamwork, in the management of the psychosis clinic, consider the clinical listening and the singular construction that each subject find to deal with the suffering.


El artículo tiene por objetivo problematizar el trabajo realizado en los servicios sustitutivos de salud mental, enfatizando los Centros de Atención Psicosocial (CAPS), que se basan en los preceptos de la Reforma Psiquiátrica. Por intermedio de fragmentos de un relato del caso de una joven acompañada por el servicio, tomaremos el psicoanálisis como eje conductor de nuestra discusión y, como método de interlocución con la realidad encontrada en el servicio, el psicoanálisis aplicado. Procederemos a una discusión acerca de la escucha del sujeto psicótico, sus impactos en el trabajo del equipo, así como las consecuencias de la propuesta de inclusión social, en la construcción del lazo social de los usuarios del servicio. Se concluye que los principios orientadores de la lucha antimanicomial, en particular la antinomia exclusión / inclusión, desconsideran la noción de sujeto en el contexto de la práctica en los servicios sustitutivos. Se propone que el trabajo en equipo, en el manejo de la clínica de la psicosis, considere la escucha clínica y la construcción singular que cada sujeto encuentra para lidiar con el sufrimiento.
