ABSTRACT

Objective: To relate physical activity, demographic, and clinical characteristics, fit to the prosthesis and health conditions related to the quality of life of adults with lower limb amputations. Method: 70 individuals with lower limb amputations of both sexes participated in the study. Data were collected using a form to characterize the participants, the Physical Activity Scale for Individual with Physical Disabilities (PASIPD), the Trinity Amputation and Prosthesis Experience ScalesRevised (TAPES-R) and the Short Form Health Survey (SF-12). Data were analyzed using descriptive and inferential statistics aiming at correlations, comparisons, and associations. A p lower than or equal to 0.05 was adapted. Results: The results showed that the participants had an energy expenditure of 26.93 MET h/d and higher averages of Health Conditions Related to Quality of Life in the mental health domain, they still had higher averages related to Prosthesis Fit in the Social domain (3.56) with Adjustment Degree above average (6.42). Conclusion: Study participants had a good level of physical activity, good fit to the prosthesis, good physical and mental health, better adaptation to the amputation and participation. In addition, a worse relationship was observed in the adaptation between people with amputations above the knee, when compared to amputations below the knee.

ABSTRACT

ABSTRACT

BACKGROUND AND PURPOSE: Low-and-middle-income countries have resource constraints and waiting lists for radiotherapy (RT). In this context, we sought to determine the survival of inpatients evaluated for palliative RT in a large referral cancer center in Brazil. MATERIAL AND METHODS: From November 2014 through December 2015, we enrolled 333 inpatients with palliative RT evaluation requests in this prospective observational study. We applied Palliative Prognostic Index (PPI) and Survival Prediction Score using Number of Risk Factors (NRF). Primary endpoint was overall survival. Secondary endpoints were survival by PPI and NRF. (ClinicalTrials.gov number, NCT02312791). RESULTS: Median survival (MS) for the entire cohort was 73 days. PPI ≤2 had MS of 120 days; PPI 2.5-4 had MS of 55 days (HR 1.84; 95% CI, 1.07-3.16); PPI >4 had MS of 39 days (HR 3.45; 95% CI, 2.07-5.74) (p < .0001). NRF 0-1 had MS of 129 days; NRF 2 had MS of 73 days (HR 1.74; 95% CI 0.89-3.38); NRF 3 had MS of 40 days (HR 2.95; 95% CI, 1.50-5.78) (p < .0001). CONCLUSION: Inpatients with palliative RT requests seem to have an overall poor survival. PPI and NRF can define subgroups with different prognosis. This could help hospitals and healthcare systems to standardize criteria for prioritization and contribute for fairness.

ABSTRACT

ABSTRACT

Assays in vitro and in vivo were performed on extract from roots and leaves from the Valeriana prionophylla Standl. (VPR and VPF, resp.). In phenylephrine (1 µ M) precontracted rings, VPR (0.01-300 µ g/mL) induced a concentration-dependent relaxation (maximum response (MR) = 75.4 ± 4.0%, EC50 = 5.97 (3.8-9.3) µ g/mL, n = 6]); this effect was significantly modified after removal of the endothelium (EC50 = 39.6 (27.2-57.6) µ g/mL, P < 0.05). However, VPF-induced vasorelaxation was less effective compared to VPR. When rings were preincubated with L-NAME (100 µ M) or indomethacin (10 µ M), the endothelium-dependent relaxation induced by VPR was significantly attenuated (MR = 20.9 ± 2.3%, 34.2 ± 2.9%, resp., P < 0.001). In rings denuded endothelium, precontracted with KCl (80 mM), or in preparations pretreated with KCl (20 mM) or tetraethylammonium (1 or 3 mM), the vasorelaxant activity of VPR was significantly attenuated (MR = 40.0 ± 8.2, n = 5; 50.5 ± 6.0%; 49.3 ± 6.4%; 46.8 ± 6.2%; resp., P < 0.01). In contrast, neither glibenclamide (10 µ M), barium chloride (30 µ M), nor 4-aminopyridine (1 mM) affected VPR-induced relaxation. Taken together, these results demonstrate that hypotension induced by VPR seems to involve, at least in part, a vascular component. Furthermore, endothelium-independent relaxation induced by VPR involves K(+) channels activation, most likely due to BKCa channels, in the rat superior mesenteric artery.
