ABSTRACT

Morbidity and mortality caused by tuberculosis are increased in most of the Latin-American indigenous communities. Factors that could explain this situation are poverty and limited health services access due to social conflicts and geographical isolation. We determined the frequency of tuberculosis in Colombian indigenous communities and described their knowledge related to transmission and control. We developed a descriptive study and health survey. Interviews were performed to find ancestral knowledge about tuberculosis. Sputum samples from patients with respiratory symptoms were analyzed. 10 indigenous communities were studied, which tuberculosis incidence was 291/100,000. Communities believe that tuberculosis is a body and spirit disease, which transmission is by direct contact or by witchcraft. Tuberculosis incidence in the studied communities was ninefold higher than that of the general population from Antioquia Department. Knowledge exchange could facilitate the community empowerment and implementation of educational activities which might improve the control of the disease.

Objetivo: cualificar la afectividad del personal de salud frente a pacientes de las UCI pediátricas de la ciudad de Medellín en el primer semestre de 2010. Metodología: se realizó un estudio cualitativo en las unidades de Cuidados Intensivos pediátricas de la ciudad de Medellín; se aplicó muestreo a conveniencia, se aplicaron 16 entrevistas semiestructuradas a médicos, profesionales de enfermería, auxiliares de enfermería y terapeutas respiratorias. Se analizaron categorías relacionadas con seguridad, confianza, tensión, desilusión, tristeza, fatiga, apego y compasión durante la atención. esultados: las experiencias y relatos personales de los profesionales de estas áreas revelan algunos temas que merecen atención: pérdidas sin duelo, expectativas irreales, abandono familiar, maltrato físico, incertidumbre al relacionarse con el paciente y sus familias, preguntas no resueltas y un sufrimiento y muerte inexplicada. Conclusiones: aunque la cantidad de preocupaciones que aqueja al personal de la salud, se infiere que tienen interés por la conservación de la vida y que su juicio no se ve alterado bajo ninguna circunstancia.


Objective: To appraise the affective state of healthcare personnel at Medellin pediatric ICU, 2010. Methods: A qualitative study was conducted in pediatric intensive care units in the city of Medellin. The sample was made at the convenience of the researchers and 16 semi-structured surveys were administered to doctors, nurses, nursing assistants and respiratory therapists. The surveys analyzed categories related to safety, confidence, stress, disappointment, sadness, fatigue, attachment and compassion while seeing patients.Results: The experiences and personal stories of professionals in these areas reveal some important issues that deserve attention: loss of patients without grief, unreal expectations, family abandonment, physical abuse, uncertainty when interacting with patients and their families, unanswered questions, pain, and unexplained death. Conclusions: Although the health personnel's worries reflect a concern for the preservation of life, their good judgment is not jeopardized under such circumstances.

Resultados: as experiências e relatos pessoais dos profissionais destas áreas revelam alguns temas importantes que merecem atendimento: perdas sem duelo, expectativas irreais, abandono familiar, maltrato físico, incerteza ao relacionar-se com o paciente e suas famílias, perguntas não resolvidas e um sofrimento e morte inexplicada. Conclusões: ainda que a quantidade de preocupações que arqueja ao pessoal da saúde, reflete que têm um interesse para a conservação da vida e que seu juízo não se vê alterado sob nenhuma circunstância.
