ABSTRACT

Breast cancer is the most incidental and deadly neoplasm worldwide; in Mexico, very few epidemiologic reports have analyzed the pathological features and its impact on their clinical outcome. Here, we studied the relation between pathological features and the clinical presentation at diagnosis and their impact on the overall and progression-free survival of patients with breast cancer. For this purpose, we collected 199 clinical records of female patients, aged at least 18 years old (y/o), with breast cancer diagnosis confirmed by biopsy. We excluded patients with incomplete or conflicting clinical records. Afterward, we performed an analysis of overall and progression-free survival and associated risks. Our results showed an average age at diagnosis of 52 y/o (24-85), the most common features were: upper outer quadrant tumor (32%), invasive ductal carcinoma (76.8%), moderately differentiated (44.3%), early clinical stages (40.8%), asymptomatic patients (47.8%), luminal A subtype (47.8%). Median overall survival was not reached, but median progression-free survival was 32.2 months (29.75-34.64, CI 95%) associated risk were: clinical stage (p < 0.0001) symptomatic presentation (p = 0.009) and histologic grade (p = 0.02). Therefore, we concluded that symptom presence at diagnosis impacts progression-free survival, and palpable symptoms are related to an increased risk for mortality.

ABSTRACT

PURPOSE: Fetuses with a diagnosis of congenital lung malformations (CLM) on prenatal imaging are commonly referred to a multi-disciplinary specialty team for prenatal assessment and postnatal management. The net effect of such services is broadly stated to improve the outcomes of affected newborns. However, these claims are relatively unsubstantiated. METHODS: After IRB approval, a retrospective review of children diagnosed with CLM from 2008 to 2018 and referred to a large urban children's hospital was performed. A comparison was performed between prenatally diagnosed patients having a multi-disciplinary fetal center evaluation (FC) and prenatally diagnosed patients who did not receive a referral or were seen prior to the establishment of the center (NON-FC). RESULTS: Eighty-eight live-born patients with a prenatal diagnosis of CLM were identified, with 49 in the FC group and 39 NON-FC. Thirty-four (63%) and 23 (59%) patients underwent operative resection of CLM, respectively. FC patients presented earlier at first postnatal follow-up (42 vs. 145 days, p = .03), had fewer preoperative office visits (2.1 vs. 3.4, p = .0003), received fewer preoperative chest radiographs (0.5 vs. 1.3; p = .002) and chest computed tomography (0.9 vs. 1.4; p = .001), and had fewer preoperative pneumonias (0 vs. 17.4%; p = .02) compared to their NON-FC counterparts. FC patients were also more likely to undergo resection at an earlier age (217 vs. 481 days, p = .003) and were more likely to undergo a minimally invasive resection (75% vs. 39.1%, p = .015). There were no differences in post-operative outcomes between the two groups. CONCLUSION: Children with a prenatal diagnosis of CLM appear to benefit from an organized multi-specialty team approach in several impactful parameters. Hospital systems and providers that invest in similar strategies are likely to achieve improved outcomes in the care of newborns prenatally diagnosed with a CLM.

ABSTRACT

RESUMEN Se presentó el caso de una actinomicosis ósea, que se manifestó como una lesión de la piel en la zona correspondiente al segundo metatarsiano izquierdo. Por el antecedente de ser la paciente operada de un tumor de células gigantes, se pensó en una recidiva tumoral. Fue intervenida quirúrgicamente y se realizó exéresis del segundo metatarsiano y de la piel afectada. La biopsia informó actinomicosis. Se trató a la paciente con amoxicilina; evolucionando satisfactoriamente, y dando seguimiento en consulta (AU).


ABSTRACT The authors presented the case of a bone actinomycosis expressed as a skin lesion in the area of the second left metatarsals. Due to the antecedent of having undergone a surgery of a giant cell tumor, a tumor recurrence was thought. Surgical intervention was performed and the excision of the second metatarsals and affected skin was performed. The biopsy reported Actinomycosis. The patient was treated with amoxicillin. She had a satisfactory evolution and is still followed up in consultation (AU).

ABSTRACT

Long-term climate change and periodic environmental extremes threaten food and fuel security1 and global crop productivity2-4. Although molecular and adaptive breeding strategies can buffer the effects of climatic stress and improve crop resilience5, these approaches require sufficient knowledge of the genes that underlie productivity and adaptation6-knowledge that has been limited to a small number of well-studied model systems. Here we present the assembly and annotation of the large and complex genome of the polyploid bioenergy crop switchgrass (Panicum virgatum). Analysis of biomass and survival among 732 resequenced genotypes, which were grown across 10 common gardens that span 1,800 km of latitude, jointly revealed extensive genomic evidence of climate adaptation. Climate-gene-biomass associations were abundant but varied considerably among deeply diverged gene pools. Furthermore, we found that gene flow accelerated climate adaptation during the postglacial colonization of northern habitats through introgression of alleles from a pre-adapted northern gene pool. The polyploid nature of switchgrass also enhanced adaptive potential through the fractionation of gene function, as there was an increased level of heritable genetic diversity on the nondominant subgenome. In addition to investigating patterns of climate adaptation, the genome resources and gene-trait associations developed here provide breeders with the necessary tools to increase switchgrass yield for the sustainable production of bioenergy.

BACKGROUND: Approximately 20% of fetuses diagnosed with congenital lung malformations (CLMs) are found to have additional anomalies. We aim to determine if additional anomalies have an impact on postnatal outcomes for patients with CLMs. METHODS: After institutional review board approval, we performed a retrospective review of live-born patients with CLMs from 2008 to 2018. All patients were prenatally diagnosed with CLMs. Clinical information pertaining to additional congenital anomalies and outcomes was collected from the electronic health record and analyzed. RESULTS: Of the 88 patients who had a prenatal diagnosis of CLMs, 20.5% had additional anomalies. Ten of the 18 patients (56%) were considered to have a major anomaly in addition to CLMs. Outcomes for patients electing nonoperative management of CLMs were similar between those with and without an additional anomaly. Although patients with an additional anomaly were more likely to have perinatal respiratory complications (44% versus 17%, P = 0.03), the number of preoperative clinic and emergency department visits, age at surgery, minimally invasive approach to surgical resection of CLM, estimated blood loss, length of hospital stay, intubation, duration of intubation, 30-day postoperative complications, and long term sequelae were not statistically different. This held true when stratified for major versus minor anomalies. CONCLUSIONS: Twenty percent of fetuses diagnosed with CLM in our population have additional anomalies. Newborns with additional anomalies have a higher risk of pre-excision pulmonary complications. However, the overall outcomes of all patients with CLMs are similar.

ABSTRACT

Algeciras Bay is an important industrial and port zone in the south of Spain whose pollution by particulate matter surpasses the threshold levels recommended by the World Health Organization (WHO) in its 2005 Guide on Air Quality. This study analyses the mortality avoided and the economic benefit which would be derived from a reduction of the pollution of PM2.5 and PM10 to the levels recommended by the WHO in Algeciras Bay in the period 2005-2015. The analysis carried out shows that the industrial zones, such as Los Barrios and San Roque, are those which have greater levels of pollution and in which the relative risk is greater. The calculations for Algeciras Bay between 2000 and 2015 show 182 deaths which would be avoided if the particulate matter pollution were reduced to the levels recommended by the WHO. Likewise, the economic valuation which this impact has on health is carried out through two concepts: the cost of illness and the Value of Statistical Life (VSL). The result shows that the economic benefit that would come out with the cost of illness valuation is 5,329,110€ and from the VSL is 414,787,113€. Implications: PM2.5 has a greater concentration in industrial localities and is linked to the industrial activity. When the particulate matter pollution is reduced to the levels recommended by the WHO in an industrialised area such as Algeciras (Spain), 182 deaths which would be avoided. The result shows that the economic benefit that would come out with the cost of illness valuation is 5,329,110€ and from the value of statistical life is 414,787,113€.

Los tumores del cuerpo carotídeo son relativamente infrecuentes y raramente se diagnostican antes de la exposición quirúrgica, su diagnóstico es benigno, pero suelen ser muy vasculares por lo que su extirpación quirúrgica muchas veces resulta difícil. Estos tumores son de crecimiento lento y pueden evolucionar durante años. El objetivo de este estudio es presentar un caso con un tumor del cuerpo carotídeo en región lateral derecha del cuello. Clínicamente se observó un aumento de volumen de aproximadamente 4 cm, no doloroso a la palpación, de tipo gomoso y adherido a planos profundos, asintomático, tratado en nuestra institución durante el año 2014. Se le realizó exéresis simple de la lesión, y el departamento de Anatomía Patológica reportó el diagnóstico de referencia. La evolución de la paciente después de 17 meses ha sido satisfactoria (AU).


The tumors of the carotid body are relatively uncommon and rarely diagnosed before the surgical exeresis, their diagnosis is benign, but they are usually very vascular and its extirpation is very difficult. These tumors have a slow growth and it can evolve during years. The aim of this study is to present a case with a tumor of the carotid body in the right lateral region of the neck. An increase of volume was observed of approximately 4 cm, not painful, of gummy type and stuck to deep, asymptomatic plans, treaty in our institution during the year 2014. It was carried out exeresis of the lesion, and the department of Pathological Anatomy reported the reference diagnosis. The patient's evolution after 17 months has been satisfactory (AU).

Fundamento: la atención a la paciente obstétrica, con coagulación intravascular diseminada, requiere de los servicios de cuidados intensivos para lograr reducir la mortalidad que puede producirse.Objetivo: caracterizar las pacientes obstétricas con coagulación intravascular diseminada, ingresadas en la unidad de cuidados intensivos del Hospital General Docente Dr. Ernesto Guevara de la Serna, entre el 10 de enero de 2006 y el 31 de diciembre de 2015.Métodos: se realizó un estudio descriptivo longitudinal retrospectivo, en una muestra de 28 pacientes obstétricas y puérperas, ingresadas en la referida unidad de cuidados intensivos en el período de tiempo declarado.Resultados: según el diagnóstico etiológico, la mayor incidencia estuvo dada por los trastornos hipertensivos con los 35,7 por ciento, seguidos del shock hemorrágico, 21,42 por ciento, la sepsis y las maniobras abortivas con el 14,3 por ciento. Las complicaciones más frecuentes se dieron en la falla multiorgánica, el shock séptico y la infección respiratoria aguda, con un 53,6 por ciento en cada caso, seguida de la disfunción ventricular izquierda (39,3 por ciento) y la disfunción hepática (35,7 por ciento). El 96,43 por ciento permanecieron internadas por más de 72 horas, con una mortalidad del 53,6 por ciento y en el 100 por ciento de las fallecidas existieron comorbilidades, mientras éstas solo se presentaron en el 23,08 por ciento de las sobrevivientes.Conclusiones: se caracterizó a las pacientes obstétricas que ingresaron en la unidad de cuidados intensivos con coagulación intravascular diseminada, las que en su gran mayoría permanecieron en esta unidad por más de 72 horas y evidenciaron la existencia de un importante número de comorbilidades en ellas (AU)


Background: medical attention to the obstetric patient with disseminated intravascular coagulation requires intensive care services to reduce the mortality this condition may bring about.Objective: to characterize the obstetric patients with disseminated intravascular coagulation admitted to the intensive care unit of Dr. Ernesto Guevara de la Serna General Teaching Hospital, from January 1st 2006 to December 31st 2015.Methods: a retrospective, longitudinal and descriptive study was carried out, in a sample of 28 obstetric and puerperal patients admitted to the intensive care unit and during the period herein mentioned.Results: according to the etiological diagnosis, the highest incidence was caused by hypertensive disorders with 35,7 percent, followed by hemorrhagic shock with 21,42 percent and sepsis as well as abortive procedures with 14,3 percent. The most frequent complications were found in the multi-organic failure, septic shock and acute respiratory infection with 53,6 percent in each case, followed by left ventricular dysfunction (39,3 percent) and hepatic dysfunction (35,7 percent). 96,43 percent of the patients remained admitted for more than 72 hours, with a mortality of 53,6 percent. There were comorbidities in all those who died for 100 percent, while they were only present in 23,08 percent of those who survived.Conclusions: a characterization of the obstetric patients admitted to the intensive care unit with disseminated intravascular coagulation was made. Most of the patients stayed in the unit for more than 72 hours and presented an important number of comorbidities (AU)

Introducción: El Consejo de Acreditación en Educación Médica para Graduados (ACGME) requiere que los residentes sean educados en seis competencias que se consideran importantes para la práctica médica independiente. Objetivos: Entender cómo los residentes perciben su aprendizaje de las competencias ACGME e identificar qué actividades educativas fueron útiles en el aprendizaje. Sujetos y métodos: Estudio descriptivo transversal de tipo encuesta a 93 residentes (72 de primer año y 21 de segundo año), 32 tutores y 10 directivos del programa de segunda especialización de la Universidad Privada Antenor Orrego de Trujillo, Perú. Cada ítem se clasificó en una escala de Likert de 5 puntos y se consideró relevante cuando los encuestados calificaron un ítem con una puntuación promedio igual o mayor que 4 (lo que indicó una actitud positiva). Resultados: En total, 135 de los 136 participantes (99%) respondieron a la encuesta: 93 (100%) residentes, 32 (99%) tutores y 10 (100%) directivos. La mayoría de los residentes declaró que su aprendizaje de las seis competencias ACGME era ‘adecuada’. No se encontró una diferencia significativa entre residentes, tutores y directivos con relación a la importancia de las competencias. Actividades de atención al paciente y observación de los médicos asistentes y compañeros fueron catalogadas por los residentes como las dos actividades de aprendizaje más útiles en adquirir estas seis competencias. Conclusión: Se enfatiza la importancia de aprender de modelos que se deben seguir durante las actividades de atención al paciente y de la heterogeneidad de las actividades de aprendizaje necesarias para la adquisición de estas seis competencias


Introduction: The Accreditation Council for Graduate Medical Education (ACGME) requires physicians in training to be educated in six competencies considered important for independent medical practice. Aims: To understand how residents perceive their learning of the ACGME competencies and to identify which educational activities were most helpful in acquiring these competencies. Subjects and methods: Cross-sectional study in which a survey was to send to 93 residents (72 first year, 21 second year), 32 mentors and 10 executives of the Program of Second Specialization at the Antenor Orrego University, Trujillo, Peru. Each item was rated on a Likert scale of 5 points, and was considered relevant when respondents rated an item with an equal or higher average score of 4 (which indicated a positive attitude). Results: A total 135 of the 136 participants (99%) responded to the survey: 93 (100%) residents, 32 (99%) mentors and 10 (100%) executives. Most residents stated that their learning of the six ACGME competencies was 'adequate'. No significant difference was found between residents, mentors and executives regarding the importance of skills. Patient care activities and observation of attending physicians and peers were listed by residents as the two most helpful learning activities for acquiring the six competencies. Conclusion: Our findings reinforce the importance of learning from role models during patient care activities and the heterogeneity of learning activities needed for acquiring all 6 competencies

Acute kidney injury (AKI) has been recognized as a risk factor for the development of chronic kidney disease (CKD). Aldosterone has a critical role in promoting renal injury induced by ischemia. Here, we evaluated whether spironolactone administered before or after AKI caused by ischemia protects against CKD. In the first set of experiments, Wistar rats underwent a sham operation without or with prior spironolactone treatment, or underwent 45 minutes of bilateral renal ischemia without or with spironolactone treatment before ischemia and assessed over 270 days. The second set of rats received low (20 mg/kg) or high (80 mg/kg) doses of spironolactone at three different times after the sham operation or bilateral renal ischemia and were assessed after 90 days. Untreated animals developed CKD following ischemia-induced AKI as characterized by a progressive increase in proteinuria, renal dysfunction, podocyte injury, glomerular hypertrophy, and focal sclerosis. This was associated with increased oxidative stress, an upregulation of tumor growth factor (TGF)-ß, followed by upregulation of the TGF-ß downstream effectors phospho-Smad3, collagen I, fibronectin, and proinflammatory cytokines. Treatment with spironolactone either before or after ischemia prevented subsequent CKD by avoiding the activation of fibrotic and inflammatory pathways. Thus, spironolactone may be a promising treatment for the prevention of AKI-induced CKD.

ABSTRACT

The acquisition of an (18)F-FDG PET-CT scan in patients with suspected paraneoplastic cerebellar syndrome can be helpful in determining the origin of a neoplasm because of its high sensitivity and also helps guide the neurological development course depending on the degree of incorporation of (18)F-FDG to the cerebellar parenchyma when compared with the rest of the brain.

Se realizó una presentación de casos, de dos pacientes con lesiones dermatológicas, donde existe correlación clínico-histopatológica, de Pityriasis rubra pilaris. La intención de este trabajo es que en la práctica dermatológica se piense en esta dermatosis. Para ello se tuvieron como objetivos: viabilizar la comprensión de esta entidad, contribuir a realizar el diagnóstico diferencial con otros cuadros como son dermatitis seborreica, queratodermia palmo-plantar, psoriasis, entre otros. En la dermatosis objeto de estudio, su cuadro histológico es característico, y debe introducirse alimentos en la dieta ricos en vitamina A, beta carotenos y azufre, pues el factor alimenticio juega un papel importante en aquellas afecciones donde la etiología por déficit de las vitaminas u otro oligoelemento está presente. Se revisan aspectos clínicos, asociaciones con otras enfermedades, el tratamiento de esta dermatosis. Se utilizó el método aleatorio, al azar, longitudinal, corroborando el diagnóstico por biopsia de piel de las pacientes. Las pacientes estudiados fueron: ARG, de 5 años, femenina, blanca remitida por el pediatra por lesiones en placas máculo amarillo-hipocrómicas–papulosas, distribuidas en superficie de extensión de brazos y piernas; LSG, de 19 años, femenina, blanca, que es remitida por presentar lesiones en placas máculo-hipocrómico pápulo-escamosas bordes difusos, localizadas en muslo y pierna derechos, se constata alopecia difusa del pelo del cuero cabelludo. El tratamiento local indicado fue una pomada con reductor, ácido salicílico y vaselina, la orientación alimenticia fue dirigida a ingerir alimentos ricos en vitamina A o beta carotenos, y alimentos ricos en azufre, insistiéndoles en esta parte del tratamiento por los antecedentes de ambas pacientes a la no ingestión de estos alimentos. A lo largo de 10 años hemos revisado anualmente la evolución de las pacientes, no existiendo recidivas hasta el momento, manteniendo ambas el régimen dietético orientado


We presented the cases of two patients with dermatologic lesions, where there it is clinic- histopathologic correlation, of Pityriasis rubra pilaris. The intention of this work is taking into account this dermatosis in the dermatologic practice. To achieve that our objectives were: allowing the comprehension of this entity, contributing to the differential diagnosis with other conditions like seborrheic dermatitis, palmoplantar keratoderma, psoriasis and others. The histological picture of the studied dermatosis is characteristic, and there should be introduced in the diet foods rich in vitamin A, beta carotenes and sulfur, because the alimentary fact plays an important role in those conditions where the etiology by vitamins or other oligo-element deficit is present. We reviewed clinical aspects, the association with other diseases, the treatment of this dermatosis. We used the randomized, longitudinal method, corroborating the diagnostic by patient's skin biopsy. The studied patients were: ARG, white, female, 5-years-old patient send by the podiatrist because she had lesions in yellow macula hypochromic- papular plates, located in extension surfaces of arms and legs; LSG, white, female, 19-years-old patient send presenting lesions in macula- hypochromic papular- squamous plaques with diffuse rims, located in the right thigh and leg, stating diffuse alopecia of the scalp. The indicated treatment was an ointment with reductive, salicylic acid and vaseline. They were advised to eat foods rich in vitamin A or beta carotenes and sulfur, insisting in this part of the treatment because they had antecedents of not eating these foods. We have followed the evolution of these patients for ten years without relapses, having the patients kept the oriented dietetic regime

Se realizó una presentación de casos, de dos pacientes con lesiones dermatológicas, donde existe correlación clínico-histopatológica, de Pityriasis rubra pilaris. La intención de este trabajo es que en la práctica dermatológica se piense en esta dermatosis. Para ello se tuvieron como objetivos: viabilizar la comprensión de esta entidad, contribuir a realizar el diagnóstico diferencial con otros cuadros como son dermatitis seborreica, queratodermia palmo-plantar, psoriasis, entre otros. En la dermatosis objeto de estudio, su cuadro histológico es característico, y debe introducirse alimentos en la dieta ricos en vitamina A, beta carotenos y azufre, pues el factor alimenticio juega un papel importante en aquellas afecciones donde la etiología por déficit de las vitaminas u otro oligoelemento está presente. Se revisan aspectos clínicos, asociaciones con otras enfermedades, el tratamiento de esta dermatosis. Se trata de la presentación de dos casos de acretismo placentario que ocurrieron en el Hospital Territorial Docente de Cárdenas, con menos de 48 horas de transcurridos de uno a otro, con confirmación de Anatomía Patológica y buena evolución de los pacientes y sus recién nacidos. En un caso se efectuó histerectomía subtotal post cesárea abdominal cuya indicación fue: embarazo a término, trabajo de parto, presentación pelviana, presunta placenta previa y cesárea anterior. En otro caso se efectuó histerectomía total post parto vaginal eutócico ante la imposibilidad de extracción manual de la placenta y hemorragia aguda.


We present two cases of placenta accreta attended at the Teaching Territorial Hospital of Cardenas, with less than 48 hours of difference, confirmed by Pathologic Anatomy and a good evolution of the patients and their newborns. In one case we made a subtotal hysterectomy after abdominal cesarean indicated as: term pregnancy, labor, pelvic presentation, presumed previous placenta and anterior cesarean. In the other, we made a total hysterectomy after eutocic vaginal delivery because of the impossibility of manually extracting placenta and acute hemorrhage.

ABSTRACT

This study examined histories of past emotional, physical, and sexual abuse as correlates of current psychological distress using data from 916 female sex workers (FSWs) who were enrolled in a safer-sex behavioral intervention in Tijuana and Ciudad (Cd.) Juarez, Mexico. We hypothesized that histories of abuse would be associated with higher symptom levels of depression and somatization and that social support would moderate the relationship. Nonparametric correlations and a series of hierarchical regression analyses revealed that all forms of past abuse predicted higher levels of depressive symptoms, and physical and sexual abuse were significantly associated with higher levels of somatic symptoms. Social support was also significantly associated with fewer symptoms of distress; however, it was not shown to moderate the relationship between abuse history and distress.
