ABSTRACT

Os números de adoecimento oncológico são crescentes, e o câncer é o principal problema de saúde pública no mundo. O trabalho é uma das dimensões da vida que se alteram no processo de adoecimento e tratamento oncológico. O servidor público municipal com diagnóstico oncológico passa por perícia e pode ter restrições médicas, readaptação profissional e até mesmo aposentadoria por incapacidade permanente. Este artigo é um estudo de caso com abordagem qualitativa e seu objetivo é descrever e problematizar o Programa Ressignificar, criado e implantado em um Departamento de Gestão de um município da Baixada Santista, que proporciona acompanhamento interdisciplinar dos servidores com diagnóstico de neoplasia em tratamento. Foram realizadas análise documental e construção de narrativas baseadas nas experiências de seis servidores participantes do Programa. As estatísticas apresentam alta prevalência do câncer de mama na população feminina. Dentre os acompanhados, 66,07% realizam o monitoramento por meio de exames periódicos, tornando possível o retorno ao trabalho, e 33,92% seguem em tratamento, afastados das atividades laborais. Os afastamentos são longos. As narrativas revelam vivências do adoecimento, relação com o trabalho, impacto do afastamento e apreciação do Ressignificar. Os resultados evidenciam a necessidade de dedicação ao tratamento, a centralidade do trabalho (identidade, socialização e sustento) e o medo da inutilidade. A readaptação representa desafio que pode ser oportunidade. O programa tem sido bem aceito, se mostrado eficiente e pertinente, proporcionando aos servidores um tratamento ao mesmo tempo institucional e personalizado. Espera-se que outras municipalidades, instituições públicas e privadas criem programas semelhantes.


The numbers on oncological illnesses are growing, and cancer is the main public health problem in the world. Work is one of the dimensions of life that change in the process of illness and cancer treatment. The municipal public servant with cancer diagnosis undergoes forensic medical evaluation and may have medical restrictions, professional rehabilitation and even retirement due to permanent disability. This article is a case study with a qualitative approach and its objective is to describe and problematize the Ressignificar Program, created and implemented in a Management Department of a municipality in Baixada Santista, which provides interdisciplinary monitoring of civil servants diagnosed with neoplasia under treatment. Documentary analysis and construction of narratives based on the experiences of six civil servants participating in the Program were carried out. Statistics show a high prevalence of breast cancer in the female population. Among those monitored, 66,07% perform monitoring through periodic examinations, making it possible to return to work, and 33,92% remain in treatment, away from work activities. The medical leaves are long. The narratives reveal experiences of illness, relationship with work, impact of medical leaves and appreciation of the Ressignificar Project. The results show the need for dedication to treatment, the centrality of work (identity, socialization and sustenance) and the fear of uselessness. Readaptation represents a challenge that can be an opportunity. The program has been well accepted, shown to be efficient and relevant, providing civil servants with both institutional and personalized treatment. Other municipalities, public and private institutions are expected to create similar programs.

ABSTRACT

Objective: This study evaluated the effects of aerobic, resistance, and combined exercise training on cardiac function and autonomic modulation in female ob/ob mice. Methods: Four-week-old female wild type and obese (ob/ob) mice were divided into five groups (n = 8): control (WT), obese (OB) obese + aerobic training (OBA), obese + resistance training (OBR), and obese + combined training (OBC). The exercise training was performed on treadmill and/or ladder at 40-60% maximum test during 8 weeks. Cardiac function was measured using echo machine. Heart rate variability (HRV) was evaluated in the time and frequency domain. Results: OB group presented higher body weight gain (~600%), glycemia (~44%) and glucose intolerance (~150%), reduction of cardiac vagal modulation, evidenced by a lower RMMSD (~56%), total power and high frequency band, and a higher isovolumic relaxation time (IVRT) (~24%) in relation to the WT group. Aerobic and combined training led to a lower IVRT (OBA: ~14%; OBC: ~14%) and myocardial global index (OBA: ~37%; OBC: ~44%). The OBA group presented an increased in vagal indexes of HRV than the other ob/ob groups. A negative correlation was observed between the delta of aerobic exercise capacity and MPI (r = 0.45; p = 0.002) and exercise capacity and body weight gain (r = 0.39; p = 0.002). Conclusion: Only the obese females underwent to aerobic exercise training showed improvement in cardiac function and HRV. Moreover, the aerobic exercise capacity as well as a greater responsivity to aerobic exercise training is intimately associated with these improvements, reinforcing the importance of aerobic exercise training to this population.

ABSTRACT

Background: Tonin, a serine-protease that forms Angiotensin II (AngII) from angiotensinogen, is increased in failing human heart samples. Increased blood pressure (BP) and decreased heart rate (HR) variabilities are associated with higher risk of cardiovascular morbidity. Losartan has been used to reduce hypertension and, therefore, lowers the risk of fatal and non-fatal cardiovascular events. Determination of tonin's impact on BP and HR variabilities as well as the impact of losartan remain questions to be elucidated. Aim: Evaluation of cardiovascular autonomic profile in transgenic mice overexpressing the rat tonin enzyme TGM'(rton) and the impact of AT1 receptor blocker, losartan. Methods: Male C57BL/6 (WT) and TGM'(rTon) mice were cannulated for recording BP (Windaq, 4 MHz) for 30 min at baseline and 30 min after losartan injection (20 mg/kg). BP and HR variabilities were analyzed in time and frequency domain method. Low-frequency (LF) and high-frequency (HF) components were identified for sympathetic and parasympathetic modulations analysis. Ang I, AngII, and Ang1-7 were quantified by high performance liquid chromatography method. The total enzymatic activity for AngI, AngII, and Ang1-7 formation was evaluated in the heart and plasma by Liquid chromatography mass spectrometry (LC-MS/MS). Results: At the baseline TGM'(rTon) exhibited higher BP, lower cardiac LF, higher cardiac HF, lower LF/HF, and lower alpha index than wild type (WT). After losartan injection, TGM'(rTon) mice presented an additional decrease in cardiac LF and increase in HF in relation to baseline and WT. In the vasculature, losartan caused decreased in BP and LF of systolic BP in WT mice in relation to its baseline. A similar effect was observed in the BP of TGM'(rTon) mice; however, LF of systolic BP increased compared to baseline. Our data also indicates that AT1R receptor signaling has been altered in TGM'(rTon)mice. Interestingly, the dynamics of the renin-angiotensin system kinetics change, favoring production of Ang1-7. Conclusion: Autonomic evaluation of TGM'(rTon) mice indicates an unclear prognosis for diseases that affect the heart. HR variability in TGM'(rTon) mice indicates high risk of morbidity, and sympathetic and parasympathetic modulation indicate low risk of morbidity. The low risk of morbidity could be the biased production of Ang1-7 in the heart and circulation; however, the altered response of AT1R in the TGM'(rTon) remains to be elucidated, as well aswhether that signaling is pro-protection or pro-pathology.

Sepsis is an uncontrolled systemic inflammatory response against an infection and a major public health issue worldwide. This condition affects several organs, and, when caused by Gram-negative bacteria, kidneys are particularly damaged. Due to the importance of renin-angiotensin system (RAS) in regulating renal function, in the present study, we aimed to investigate the effects of endotoxemia over the renal RAS. Wistar rats were injected with Escherichia coli lipopolysaccharide (LPS) (4 mg/kg), mimicking the endotoxemia induced by Gram-negative bacteria. Three days after treatment, body mass, blood pressure, and plasma nitric oxide (NO) were reduced, indicating that endotoxemia triggered cardiovascular and metabolic consequences and that hypotension was maintained by NO-independent mechanisms. Regarding the effects in renal tissue, inducible NO synthase (iNOS) was diminished, but no changes in the renal level of NO were detected. RAS was also highly affected by endotoxemia, since renin, angiotensin-converting enzyme (ACE), and ACE2 activities were altered in renal tissue. Although these enzymes were modulated, only angiotensin (ANG) II was augmented in kidneys; ANG I and ANG 1-7 levels were not influenced by LPS. Cathepsin G and chymase activities were increased in the endotoxemia group, suggesting alternative pathways for ANG II formation. Taken together, our data suggest the activation of noncanonical pathways for ANG II production and the presence of renal vasoconstriction and tissue damage in our animal model. In summary, the systemic administration of LPS affects renal RAS, what may contribute for several deleterious effects of endotoxemia over kidneys.
