ABSTRACT

Case report of a 38-year-old man who, 14 months after resection of a malignant melanoma from the right shoulder in September 1990, presented with a tumour of the right adrenal gland. Resection confirmed metastatic melanoma. Chemotherapy was administered for 3 months. The patient was followed until his death in March 1995, i.e. three years after resection of the secondary adrenal tumour.

ABSTRACT

The authors review results of 20 years of cryosurgery for benign prostatic hypertrophy based on 992 cases treated by this method. In particular, they compare the 500 more recent cases of cryocauterisation with the new minimally invasive techniques proposed:balloon dilatation, prostatic stent, thermotherapy, hyperthermia, medical treatment with a 5-alpha-reductase. A study of the cost of these treatments with similar performances places cryosurgery in a good position. It covers the largest range of therapeutic indications with the fewest contraindications in high-risk patients. Good long-term objective results were observed. In their current state of application, thermotherapy and hyperthermia are essentially palliative modalities.

ABSTRACT

The development of urinary lithiasis as a result of parathyroid lesions is not an exceptional occurrence and should be routinely looked for. Among the 30 cases of parathyroid adenoma identified during a 25-year period in a department of urology in a medium-sized town, 18 were detected following the occurrence of urinary lithiasis. Clinical features in these 18 cases, as well as the investigations that led to the discovery of the adenoma are reported. The surgical cervicotomy technique used by the same operators is briefly described. In other countries (Germany, Switzerland, Italy), the detection and management of parathyroid adenomas is often considered as within the field of urology.

ABSTRACT

The authors present a case which is essentially interesting because of its rarity. A metastasis of the left pelvic ureter was discovered following an episode of renal colic in a woman treated for known breast cancer. The patient's age, 49 years, encouraged conservative treatment. The functional urological and radiological outcome were satisfactory with a follow-up of 3 years.

ABSTRACT

Research on 880 cases. Cryosurgery was chosen for high-risk patients when other methods were contra-indicated. The operation is performed with no anesthetics. There is no risk of bleeding even in patients presenting with a high hypocoagulation. The operative technique is explained in detail, and precise indications are given for prostatic adenoma as well as for malignant tumors. The allied techniques of cryocautery and cryo-TUR are also described. Resumption of urination is assured in 87% of cases. Complications are rare if the protocol is scrupulously respected. The complications are discussed. There are other indications for cryosurgery in urology, such as for bladder tumors, but in rare cases only, and this technique should be considered as experimental.

ABSTRACT

This article reports a case of stress incontinence associated with ureteral ectopia, which was successfully treated by periurethral injection of teflon paste. The results were good twenty months later.

ABSTRACT

Four bladder cancers were treated by cryo-tumorectomy under impedance-metry. In 3 very advanced cases, the result was favourable for several months, without any local recurrence, but there was generalization of the cancer. In the 4 th case, the excellent result has been maintained for 3 1/2 years, with a normal life-style, but the treatment was completed with gamma therapy. This technique is proposed as an exceptional measure in very fragile patients as an alternative to cystectomy. In the absence of any proven cryo-immunological effect, complementary gamma therapy is recommended.
