ABSTRACT

OBJECTIVE: To obtain insight into the results of treatment of lagophthalmos by gold weight implantation in the upper eyelid. DESIGN: Retrospective. METHODS: The records of all 37 patients treated in the Netherlands (1986-1998) were reviewed. All traceable patients (n = 33) were sent a questionnaire and invited for clinical evaluation of the treated eye (n = 28). RESULTS: The average follow-up was 44 months (range: 3-150). Twenty-seven patients (82%) reported improved eye closure. After 6 weeks 13 patients (48%) had no eye complaints (dry eye, epiphora, conjunctivitis). At the clinical evaluation 10 patients (36%) had no such complaints. Nine patients (27%) had to undergo a reintervention because of malposition or spontaneous extrusion of the gold weight. It was removed in 4 of them. Upon closure the average residual palpebral aperture was 1.17 mm (range: 0-5) in the patients with gold weight in situ and 6.00 mm (5-10) in the patients in whom the implant had been removed. CONCLUSION: A better eye closure was obtained in 82% of the patients, while eye complaints resolved in 36% of them.

ABSTRACT

Blomstrand osteochondrodysplasia (BOCD) is a rare, autosomal recessive, lethal skeletal dysplasia characterized by generalized osteosclerosis and advanced skeletal maturation. The histopathological characteristics of three novel cases (two isolated cases and a sib-pair) of BOCD are presented and correlated with the clinical and radiographical findings, and the relevant literature is reviewed. The results of our study confirm the existence of two separate types of BOCD, which we propose naming type I: the severe, 'classical' form, and type II: a less severe form.

ABSTRACT

PURPOSE: To investigate the feasibility of using a dual-energy X-ray absorphometry (DEXA) scan to predict long-term force-transmission patterns in wrists. MATERIAL AND METHODS: Both wrists of a man with morbid Kienböck stage IIIa disease of his left wrist (avascular necrosis of the lunate) were examined by a DEXA scan to determine the differences in bone density in the distal radius. RESULTS: In the distal radius of the injured wrist, a shift in bone density was seen toward the scaphoid fossa, which resembles the shift in force-transmission pattern described in force-transmission studies of the wrist. CONCLUSIONS: These differences can be interpreted as a result of an altered force-transmission pattern in the injured wrist.

ABSTRACT

A patient is presented with complaints of a hypermobile right ear due to agenesis of the superior and posterior auricularis muscles and a thin concha cartilage. Suturing the concha cartilage to the mastoid bone and temporal fascia gave sufficient support of the external ear without creating a gross difference in appearance of both ears.

ABSTRACT

A new case of isolated paralysis of the anterior interosseous nerve of a 24-year-old man is described. It is thought that this was caused by a fall on the left arm. This paralysis has a typical clinical picture with a characteristic disturbance of the pinch grip. The patient recovered spontaneously. A case of severe lesion of the hand is reported in which the circulation of the thumb was restored by using a vein graft. The operative procedure and postoperative management are described. The periods of arterial insufficiency in the thumb, likely due to spasms, are discussed.
