ABSTRACT

OBJECTIVE: Aim: To investigate and analyze homeostatic disorders in patients with a combination of Chronic Pancreatitis(CP) and Arterial Hypertension (AH) and to develop correcting ways of the detected changes. PATIENTS AND METHODS: Materials and Methods: General clinical, laboratory-instrumental examination of 121 patients, who were undergoing inpatient treatment with a diagnosis of Chronic Pancreatitis in combination with Arterial Hypertension of the II stage during 2021-2022. RESULTS: Results: In the majority of cases of patients signs the increasing in IL-1,6 and Cortisol levels were found. A decrease in Ca to the lower limit of the norm was observed (2.18 ± 0.26 mmol/l to the data of control group patients (2.32 ± 0.12 mmol/l, p= 0.01 ), the levels of trace elements Zn and Se were determined within the reference values. The Atherogenic Index was increased 1.8 times and was significantly different from the control group date. During the FE-1 study, a decrease in the level of this indicator was revealed by 151.71±13.91 mg/g of feces, both to the values of reference values and a significant difference to the data of the control group (241.28±29.17 mg/g of feces, p<0 .05). CONCLUSION: Conclusions: Based on the multivariate linear regression analysis of the obtained data, formulas have been developed that can be used to predict the dynamics of the dependent variable (FE-1, IL-1, Selenium level, Glutathione Peroxidase, blood pressure) according to changes in the studied influencing factors.

ABSTRACT

A 25-year-old male with known EDMD was referred for the cardiology consultation due to symptoms of heart failure. Echocardiography showed decrease left ventricular ejection fraction (LVEF) and therapy with ramipril, torsemide and rivaroxaban was initiated. Despite initial improvement, the patient later developed presyncope, bradycardia, irregular heartbeat and worsening of dyspnea. Therefore, implantation of resynchronization pacemaker with the function of implantable cardioverter-defibrillator (CRT-D/P) was performed. Ramipril was substituted by sacubitril/valsartan, and mineralocorticoid receptor antagonist and beta-blocker were initiated. Genetic testing found AD mutation in lamin A/C gene LMNA c.746G>A, p.(Arg249Gln). Upon follow-up, the patient demonstrated resolution of dyspnea and reverse remodeling of the left ventricle with complete restoration of the LVEF.

ABSTRACT

BACKGROUNDS: coronary heart disease and hypertension are prominent in the structure of morbidity and mortality in Ukraine, and these indicators have tended to increase. The proportion of patients with myocardial infarction with normal or close to normal coronary arteries according to coronary angiography, according to various sources, ranging from 1 to 12%. AIM: set the clinical course of CHD in the background of arterial hypertension of residents in the mountainous areas of Transcarpathion region. MATERIALS AND METHODS: all patients performed general clinical, laboratory, echocardiographic examination by the standard method and coronary angiography. RESULTS: study groups did not significantly differ in key demographic and clinical data. Mountain residents with myocardial infarction were also almost 9 years younger than lowland residents.Analysis of intracardiac hemodynamics revealed no significant differences except more important dimensions of left atrium and higher indices of diastolic left ventricular function in Highlanders. Concusions: in Highlanders MI occurs at a younger age than in lowland inhabitants. Also, in Highlanders were found trends towards lower values of serum creatinine. Miocardial infarction occurred much less in mountain residents. Hemodynamics in Highlanders characterized by a larger sizes of left atrium and higher rates of diastolic left ventricular function.
