ABSTRACT

PIK3CA-related overgrowth spectrum (PROS) encompasses different clinical entities caused by somatic activating mutations in PIK3CA. Among PROS, CLOVES syndrome represents a severe phenotype with poor survival rate. We present the case of a 4-month-old girl with CLOVES syndrome successfully treated with alpelisib, a PIKC3A inhibitor.

ABSTRACT

Missed clinic appointments or no-shows burden health care systems through inefficient use of staff time and resources. Scheduling software with automatic appointment reminders shows promise to improve clinics' management through timely cancellations and re-scheduling, but at-scale evidence is missing. We study a nationwide text message appointment reminder program in Chile implemented at primary care clinics for patients with chronic disease. Using longitudinal clinic-level data, we find that the program did not change the number of visits by chronic patients eligible to receive the reminder but visits from other patients ineligible to receive reminders increased by 5.0% in the first year and 7.4% in the second. Clinics treating more chronic patients and those with a relatively younger patient population benefited more from the program. Scheduling systems with automatic appointment reminders were effective in increasing clinics' ability to care for more patients, likely due to timely cancellations and re-scheduling.

ABSTRACT

Esta investigación que presentamos pretende profundizar en las señales de alarma más frecuentes de los niños y las niñas diagnosticados con TEA, atendiendo a su nivel de desarrollo lingüístico. Para ello, analizamos los resultados obtenidos de una muestra de 142 niños y niñas evaluados en los cinco últimos años en el Servicio de Psicomotricidad de la Universidad de La Laguna. El instrumento utilizado es el ADOS-2 y para realizar el análisis estadístico hacemos uso del SPSS. Las conclusiones obtenidas hacen referencia a la ausencia de lenguaje durante los 3 primeros años de vida y a un aumento del porcentaje de sujetos con lenguaje adquirido a partir de esta edad; al escaso uso de la mirada, poco uso de la expresividad facial y gestualidad limitada en la interacción social; o la falta de respuesta cuando se les llama por su nombre solo en sujetos carentes de lenguaje, como algunas de las características más evidentes en las personas con TEA. En general, las variables en interacción social están más estrechamente relacionadas entre sí que en el resto de los ámbitos evaluados. (AU)


This research aims to deepen the most frequent warning signs of children diagnosed with ASD, taking into account their level of linguistic development. We ana-lyze the results obtained from a sample of 142 children evaluated in the last five years in the Servicio de Psicomotricidad, Universidad de La Laguna. The instrument we use is the ADOS-2, and to perform the statistical analysis, we make use of the SPSS. The conclusions obtained refer to the absence of language during the first 3 years and an increase in the percentage of subjects with language acquired from this age; the limited use of the gaze, little use of facial expressiveness and limited gesture in social interaction; or the lack of response when called by name only in subjects lacking language, such as some of the most obvious characteristics in people with ASD. In general, variables in social interaction are more closely related to each other than in the other evaluated areas. (AU)

Objective: the exact prevalence of obesity in Mexico is not well known and varies between sources, but more than 30% of Mexico's population are obese. Obesity is associated with several diseases such as metabolic syndrome; the latter, along with cancer, have become public health concerns worldwide, and their association has been widely studied in developed countries. The aim of this study was to identify the overall prevalence of metabolic syndrome and to describe its characteristics among first-time cancer patients at a referral center in Mexico. Methods: a prospective, observational, cohort study of first-time patients of the National Cancer Institute of Mexico in the period of September 2016-2017. We identified 1,165 first-time patients, and 316 patients with known or recently diagnosed metabolic syndrome were included. Results: median age was 55 years old and most were female (81%). The most frequent tumors were breast, gynecological, and hematological growths. Obesity (class 1-3) and abnormal glucose and/or previous diabetes mellitus diagnosis were mostly observed in patients with skin and soft tissue tumors; dyslipidemia, high triglycerides, and/or low HDL-cholesterol were mostly observed in patients with gastrointestinal tumors. Conclusion: the prevalence of metabolic syndrome among first-time cancer patients was 27%. As obesity and cancer are of public concern in Mexico, the implementation of preventive strategies for metabolic syndrome patients, focusing on the first level of care during early stages in order to reduce the risk of cancer, is needed

El objetivo de este estudio fue identificar la prevalencia del síndrome metabólico y describir las características entre pacientes oncológicos de primera vez en un hospital de tercer nivel en México. Métodos: estudio prospectivo, observacional y de cohortes que incluye a pacientes oncológicos atendidos por primera vez en el Instituto Nacional de Cancerología durante el periodo de septiembre 2016 a 2017. Identificamos 1165 pacientes; 316 tenían el diagnóstico de síndrome metabólico y fueron incluidos en el presente estudio. Resultados: la mediana de edad fue de 55 años y la mayoría de los pacientes eran del sexo femenino (81%). Las neoplasias más frecuentes fueron las de mama, ginecológicas y hematológicas. Conclusiones: la prevalencia del síndrome metabólico en nuestros pacientes oncológicos fue de 27%. ABSTRACT

Only a small percentage of children with Autism Spectrum Disorders are diagnosed before they are three years old, although earlier detection and intervention would reduce the disabilities associated with the disorder. In addition, as children get older, treatments are more costly and difficult and the results less satisfactory. Considering the importance of detecting autism early, the objective of this study is to identify the instruments that can be used to detect signs of autism before children are 2 years old, and which of these instruments have been validated in the Spanish population. By searching in several research databases, we compared the existing instruments and their main characteristics. We found that some instruments can be used to assess a possible autism spectrum disorder before children are 2 years old, with acceptable sensitivity, specificity and reliability indexes. However, only a few instruments have been validated for the Spanish population, some of which have not been specifically designed for early detection. For this reason, a tool needs to be developed to detect the warning signs of autism spectrum disorders before the age of 2 which can be applied as part of the protocol for pediatric check-ups

ABSTRACT

Introducción Desde los trabajos presentados por Giuliano et al . en 1994 y sus posteriores validaciones, la biopsia del ganglio centinela se ha convertido en el standard de la evaluación axilar en estadios tempranos del cáncer de mama. Es de vital importancia respaldar esta técnica con altas tasas de identificación y de correlación anatomopatológica intraoperatoria/diferida. Objetivos 1) Determinar nuestra tasa de identificación usando solo azul patente. 2) Determinar la correlación entre el estudio intraoperatorio y diferido. 3) Determinar la tasa de recurrencia axilar. Material y método Se reclutaron 100 pacientes con cáncer de mama T1 T2 N0 o cdis extenso de alto grado diagnosticadas por punción con aguja fina (paf) o Core Biopsy. Se utilizó solo Azul Patente al 1%, inyección subareolar, masaje circular por 10 minutos, incisión axilar. Se reconoce como Ganglio Centinela al ganglio o a los ganglios teñidos de azul o con su linfático aferente con colorante. El estudio intraoperatorio se realizó por sección en fresco e impronta citológica. El diferido por coloración con Hematoxilina y Eosina. Las pacientes fueron seguidas por el cirujano actuante y/u oncología clínica. Resultados Se evaluaron 100 pacientes. Nuestra tasa de detección fue del 98%. Los falsos negativos intraoperatorios por impronta citológica fueron del 3%. Luego de un seguimiento promedio de 63,8 meses, no se detectaron recidivas axilares. El número total de Ganglios Centinela positivos fue del 26,5%. El promedio de Ganglios hallados fue de 1,2. Conclusiones La Biopsia de Ganglio Centinela usando Azul Patente como único método es una operación confiable, de bajo costo y al alcance de todo centro con interés de desarrollar dicha técnica.


Introduction Since Giuliano et al. publications in 1994 and forward validations, sentinel node biopsy (snp) has become the standard procedure for staging the axilla in early breast cancer. Therefore, it is of vital importance to back this technique up by a high rate of identification and high intraoperative/final pathology correlation. Objectives 1) To determine our identification rate by only using Patent Blue dye. 2) To determine the correlation between intraoperative and final pathology results. 3) To determine axillary recurrence. Materials and method A hundred patients have been recruited. Each was diagnosed with T1 T2 N0 Breast Cancer or extense high grade Ductal Carcinoma In situ (dcis) by fine needle aspiration (fna) and/or Core Biopsy. Patent Blue 1% was injected subareolary. In addition, circular massage was performed during 10 minutes and an axillary incision was made. The node or nodes dyed in blue or with coloured lymphatic afferent have been acknowledged as sentinel nodes. The node was analyzed intraoperatively by touch imprint citology. The final study was done with Hematoxilin Eosin coloration. Further follow up was in charge of the surgeon and/or clinical oncologist. Results A hundred patients were assesed. Our identification rate was 98%. The pathology intraoperative false negative rate was 3%. Afer an average of 63,8 months follow up, no axillary recurrence was detected. 26,5% of positive sentinel nodes was found. The average of sentinel nodes found was 1,2. Conclusions snb by using only patent blue dye is a low cost reliable technique, and available for every institution interested in its development.

Introducción La biopsia percutánea con guía estereotáxica digital (bed) ha permitido disminuir el número de cirugías innecesarias y planificar con mejores resultados las biopsias y cirugías radioquirúrgicas. Objetivos El objetivo de este trabajo fue analizar la tasa de subestimación de la punción biopsia percutánea con guía estereotáxica digital y correlacionar la imagen mamográfica con la histopatología. Material y método Se evaluaron 247 pacientes con lesiones mamarias subclínicas diagnosticadas por mamografía y sometidas a una bed (suros) durante el período mayo 2013 a diciembre 2015. Se registró de cada paciente la edad, clasificación de bi-rads, la morfología radiológica de las lesiones, los informes histopatológicos de las bed y de posteriores biopsias y/o cirugías radioquirúrgicas (bp/cxrdq). Se analizaron 237 biopsias (Bp). La edad promedio fue de 52 años. De 237 Bp realizadas, obtuvimos 168 lesiones benignas, 25 lesiones de alto riesgo histológico (larh) y 44 malignas. La sensibilidad de las bed fue del 97,7% y la especificidad del 100%; la tasa de subestimación del método fue del 4% para larh y del 26% para cdis. Las lesiones radiológicas predominantes fueron las microcalcificaciones. Conclusiones La bed ha demostrado ser un excelente método de diagnóstico, con múltiples ventajas respecto de la bprdq, con una buena correlación histopatológica y muy baja tasa de subestimación en larh


Introduction Vacuum-assisted stereotactic breast biopsy (dvab) allowed to reduce the number of unnecessary surgeries and plan with better results biopsies and radiosurgical surgeries. Objectives The aim was to analyze the underestimation rate of dvab and correlate the mammographic image with histopathology. Materials and method 247 patients with subclinical breast lesions diagnosed by mammography and subjected to a dvab (suros) during the period May 2013 - December 2015 were evaluated. The age of each patient, bi-rads classification, radiological morphology of lesions, dvab histopathologic reports and subsequent biopsies and/or radiosurgical surgeries (bp/cxrdq) were registered. 237 biopsies were analyzed. The average age was 52 years. Of 237 Bp realized, we obtained 168 benign lesions (lb), 25 high risk histologically lesions (hrhl) and 44 malignant lesions (lm). The dvab sensitivity and specificity was 97.7% and 100%, respectively. The rate of underestimation of the method was 4% for hrhl and 26% for dcis. Microcalcifications were the predominant radiological lesions. Conclusions The dvab has proved to be an excellent method of diagnosis, with multiple advantages over the BpRDQ, with a good correlation histopathological and very low rate of underestimation in hrhl.

ABSTRACT

El tumor mixto cutáneo es una neoplasia se aparición inusual al que se ha denominado también siringoma condroide. Se reporta un caso de Tumor mixto benigno apocrino en una paciente de 63 años. Paciente mujer de 63 años, con lesión tumoral de un año de evolución en zona frontal, asintomática. La ecografía de partes blandas fue compatible con lesión quística y el estudio histológico mostró la presencia de una neoplasia mixta benigna epitelial estromal, correspondiente a un tumor mixto benigno apocrino. Los tumores mixtos benignos apocrinos son neoplasias poco frecuentes, localizadas en las glándulas sudoríparas. Se caracterizan por presentarse como un nódulo subcutáneo o intradérmico, solitario, que puede alcanzar hasta 3 cm. de diámetro. Son asintomáticos y de crecimiento lento. Su principal ubicación corresponde a la cabeza y cuello y predominan en pacientes de sexo masculino, de edad media. El diagnóstico de certeza es histopatológico, encontrándose a la microscopía un patrón epitelial compuesto por estructuras tubulares, ductales, túbulo-alveolares o áreas sólidas con diferenciación pilo-sebácea focal, en un estroma que puede ser mixoide o condroide Se presenta este caso por ser un tumor de baja frecuencia, cuyo diagnóstico es esencialmente histopatológico y cuya terapia resolutiva es la extirpación quirúrgica completa.


Apocrine mixed tumor of the skin: a diagnosis challenge to consider. Cutaneous mixed tumor is an uncommon, benign adnexal neoplasm arising from apocrine or eccrine glands of the dermis. Apocrine mixed tumors often exhibit decapitation secretion, a feature of apocrine epithelium, but they may exhibit a wide range of metaplastic changes and differentiation in the epithelial, the myoepithelial, and the stromal components. We present two clinical cases of Apocrine mixed tumour of the skin in two different patients who have seen in our Department.

ABSTRACT

Growth failure is almost inextricably linked with chronic kidney disease (CKD) and end-stage renal disease (ESRD). Growth failure in CKD has been associated with both increased morbidity and mortality. Growth failure in the setting of kidney disease is multifactorial and is related to poor nutritional status as well as comorbidities, such as anemia, bone and mineral disorders, and alterations in hormonal responses, as well as to aspects of treatment such as steroid exposure. This review covers updated management of growth failure in these children including adequate nutrition, treatment of metabolic alterations, and early administration of recombinant human growth hormone (GH).

BACKGROUND: The interests that motivate medical students to study this career are diverse and they may change during the seven years of study. In Chile, 22 universities offer medicine and the number of graduated students has increased by more than 50% over the last 10 years. AIM: To determine the motivational profile of medical students at admission, and at the end of their career. SUBJECTS AND METHODS: A voluntary anonymous survey was applied to 275 first and 140 seventh year medical students from one traditional public and two private schools. RESULTS: The main reason for applying to medical school was social interest (68.7%), followed by interest in science and academia. Thirty six percent of students from seventh year would not study medicine again. In the seventh year, the interest in medical care persists in 88% of students, followed by academic interests in 64%. Only 24% had research interests. Fifty nine and 57% of students projected their medical work in private and public hospital settings, respectively. Only 11% projected themselves as doing research. Sixty nine percent of students would like to receive more information about post graduate education. CONCLUSIONS: There is a low interest in research and a high percentage of seventh year students that would not apply to medicine again. Medical schools should perform a systematic analysis of students' interests to improve faulty areas.

ABSTRACT

Se evaluó la calidad bacteriológica del agua de 8 clínicas de una institución de salud pública (ISSSTE) de 2 zonas de la ciudad de México, determinándose coliformes fecales y coliformes totales, por medio de la técnica de filtro de membrana, realizándose en 2 diferentes periodos y 3 puntos de muestreo (jeringa triple, llena vasos y suministro) en 9 unidades dentales. De las 8 clínicas analizadas 7 presentaron contaminación bacteriana. Perteneciendo 4 a la zona norte y 3 a la zona sur. El problema de la contaminación en la zona norte se debe principalmente a la mala calidad del agua de suministro la cual en esta zona provenía en su mayoría de las botellas del sistema abastecedor de agua. La otra causa de contaminación se debió a la deficiente o nula desinfección de la jeringa triple. En la zona sur 3 de las 4 clínicas en general presentaron baja contaminación bacteriana solo en el primer periodo, lo cual se puede atribuir a la mejor calidad del agua proveniente de los suministros. Se concluye que los factores determinantes de la presencia de contaminación bacteriana en el agua utilizada en las unidades dentales de ambas zonas fueron: la calidad del agua de suministro y la inadecuada desinfección de la jeringa triple. Mantener una buena calidad de agua en las unidades dentales es muy importante, pues el líquido entra directamente en contacto con la mucosa de cavidad oral, estructuras dentarias, sangre y saliva del paciente, evitando procesos infecciosos que podrían poner en riesgo su salud integral


It was assessed the bacteriological quality of water in 8 clinics of a public health institution (ISSSTE) in 2 areas of Mexico City, determined fecal coliform and total coliform, by means of the technique of membrane filter. There were made 2 different periods and 3 sampling points (syringe triple, flood glasses and supply) in 9 dental units. Of the 8 clinics analyzed 7 presented bacterial pollution. 4 Belonged to the north and 3 to the south zone. The problem of pollution in the north zone is mainly due to the poor water quality of supply which in this area came mostly from the bottles of the flush system. The other source of pollution was due to the poor or non-existent disinfection of the triple syringe. In the south zone in general 3 of 4 clinics presented low bacterial pollution only in the first period, which can be attributed to the better quality of water from the supplies. It concludes that the determining factors in the presence of bacterial pollution in water used in dental units of both areas were: the quality of water supply and inadequate disinfection of the triple syringe. Maintain a good water quality in dental units is very important, because liquid makes direct contact with the mucous membrane of oral cavity, dental structures, blood and saliva of the patient, avoiding infectious processes that could put in risk their integral health

ABSTRACT

Se determina la incidencia del Streptococcus beta hemolítico del grupo A y otros estreptoccos beta hemolíticos en la orofaringe de 231 escolares sanos con edades comprendidas entre 7 y 12 años que acuden a la institución " Fundación del Niño " del estado Zulia, durante el período Octubre 1991 - Julio 1992. La muestra de exudado faringeo es obtenida y colocada en medio de transporte de Cary y Blair, y luego es inoculada en placas de agar sangre de carnero con Kanamicina (atmósfera de anaerobiosis) y en agar sangre de carnero (5-10 por ciento CO2) ambas inacubadas a 35-37 oC, durante 18-24 h. A las colonias compatibles con estreptoccos beta hemolíticos se les realiza un frotis coloreado con la técnica de Gram, si la morfología es compatible con la familia Streptococcaceae se procede a la identificación presuntiva siguiendo la metodología descrita por Facklan y colaboradores. La identificación definitiva se determina por pruebas serológicas empleando la técnica de Slide Strepto Kit. De las 231 muestra estudiadas 76 (32.90 por ciento) resultan positivas, correspondiendo para el grupo B23 (30.20 por ciento), no agrupables 17 (22.37 por ciento), grupo G 15 (19.74 por ciento), grupo A 13 (17.10 por ciento) y grupo C8 (10.53 por ciento). Por otra parte se encuentra que los grupos A, C y no agrupables serológicamente muestran el mayor porcentaje de aislamientos entre las edades de 9 y 10 años, el grupo G se aisla mayormente de escolares entre 7-8 años y el grupo B entre 7-10 años. En relación al sexo no se detecta una marcada variabilidad en los resultados de este estudio

Se realiza un estudio en 75 pacientes que solicitaron interrupción tardía de la gestación (segundo trimestre) durante el año 1986 en el Hospital General Docente "Enrique Cabrera", a las cuales se les llenó una encuesta previamente confeccionada. Se analizan una serie de factores que inciden en la interrupción del embarazo tales como: edad de la paciente, paridad, nivel educacional, ocupación, situación matrimonial, procedencia, uso de anticonceptivos, condiciones de la vivienda, renta per cápita, etcétera. Se concluye que los problemas socioeconómicos y la defiente educación sexual constituyen los factores primordiales que motivan una interrupción tardía de la gestación
