ABSTRACT

Presentamos el caso clínico de una paciente de 58 años de edad que llegó al Servicio de Urgencias por cuadro de pérdida de conciencia acompañado de cortejo vegetativo (sudoración fría, mareos, bradicardia, hipotensión arterial, sensación de inestabilidad a la marcha sin giro de objetos y temblor). Ante la sospecha de un síncope vasovagal se inician las medidas generales y las pruebas diagnósticas encaminadas a descartar la patología potencialmente grave que requiera una atención urgente. Dado que la paciente presentó en las siguientes dos horas de la llegada a Urgencias tres episodios similares donde, aparte de la pérdida de conciencia, se objetivó bradicardia, hipotensión e hipoglucemias severas, a pesar de haber instaurado tratamiento para dicha sintomatología, se dirigen las pruebas diagnósticas y la orientación etiológica a una causa endocrino-metabólica, resultando compatible con un cuadro de silla turca vacía que respondió a corticoterapia


We present the clinical case of a 58 year old female patient who came to the Emergency Service due to a picture of loss of consciousness accompanied by vegetative symptoms (cold sweating, dizziness, bradycardia, low blood pressure, sensation of instability when walking with spinning of objects, tremor). Due to the suspicion of a vasovagal syncope, general measures and diagnostic tests aimed at ruling out potentially serious disease requiring urgent care were initiated. Since the patient had 3 similar episodes in the 2 hours following arrival to the Emergency Service in which, besides loss of consciousness, bradycardia, hypotension and severe hypoglycemias were observed in spite of having begun treatment for these symptoms, the diagnostic tests and etiological orientation were directed towards an endocrine-metabolic cause. This was consistent with an empty sella turcica picture that responded to corticosteriod treatment
