ABSTRACT

RESUMEN El trasplante se constituye en una alternativa de vida para pacientes con falla terminal de un órgano. Este tipo de tratamiento viene en aumento y para el caso de la falla cardiaca refractaria mejoran no sólo la expectativa de vida, sino la calidad de vida. Sin embargo cerca del 20% de las complicaciones observadas en estos pacientes, constituyen a complicaciones que afectan al sistema nervioso central, algunas relacionadas con el tratamiento inmunosupresor, como es el caso de las neuroinfecciones y las neoplasias. Se presenta y analiza el caso de una paciente quien posterior a un trasplante cardiaco, presenta una neoplasia del sistema nervioso central, en quién por patología de hace un diagnóstico post-morten de Linfoma de células B.


SUMMARY Transplantation is a life alternative for patients with organ terminal failure. This type of treatment is on the rise and in the case of refractory heart failure improves not only the life expectancy, too the quality of life. However, about 20% of the complications observed in these patients constitute complications that affect the central nervous system, some related to immunosuppressive treatment, such as infections and tumors. We present and analyze the case of a patient who, after a heart transplant, has central nervous system tumors, in whom by post-mortem diagnosis of B-cell lymphoma.

ABSTRACT

Los hemangiomas capilares son lesiones tumorales benignas comunes en diferentes localizaciones. Sin embargo la afección del sistema central es extremadamente inusual. En la literatura tan solo han sido reportados 7 casos de hemangiomas capilares de localización exclusiva intramedular y poco más de 20 casos con compromiso extramedular adicional. Reportamos un paciente adolescente alteración motora y sensitiva progresivo del miembro superior izquierdo asociado a cervicalgia y espasmos musculares cervicales. La resonancia magnética reveló la presencia de una lesión medular cervical sugestiva de neoplasia con un importante componente vascular. Durante la resección no se observó compromiso extra-medular. Los exámenes histopatológico e inmunohistoquímico confirmaron el diagnostico de hemangioma capilar. Si bien, se han publicado muy pocos casos de hemangiomas capilares intramedulares, se cree que puede existir un importante subregistro. En general constituyen una patología benigna de buen pronóstico y que no requiere de manejos adyuvantes como quimio o radioterapia, pero si de un seguimiento neurológico.

ABSTRACT

OBJECT: So-called direct carotid-cavernous fistulas (CCFs) are commonly treated by detachable balloons or coils to occlude the shunt while sparing the carotid artery. Liquid embolic agents have been rarely used, and in particular, to the authors' knowledge, the use of Onyx as the sole agent has never been reported in an indexed publication. METHODS: The authors describe a case series of 5 patients with posttraumatic CCF in whom embolization with Onyx was prospectively used as the sole strategy of management. RESULTS: Complete occlusion was obtained at the end of the procedure in 4 cases, and the lesion in the remaining patient subsequently occluded at the 6-month follow-up evaluation. CONCLUSIONS: As endovascular techniques for treatment of direct CCFs continue to evolve, this novel approach with Onyx as the sole embolic material seems promising in treating these lesions.
