ABSTRACT

OBJECTIVE: To describe overall survival (OS) and disease-free survival (DFS) in a cohort of tongue cancer patients, together with the corresponding demographic, tumor and surgical characteristics. METHODS: A retrospective study was made of 205 consecutive patients with primary tongue cancer subjected to surgery and adjuvant therapy according to the stage of the disease, in Hospital Clínico Universitario Virgen de la Arrixaca (HUVA) (Murcia, Spain) during the period 2000-2020. Survival was evaluated based on the Kaplan-Meier method, and the existence of significant differences between the different study variables was analyzed using the log-rank test. Cox regression analysis was performed for the identification of risk factors. RESULTS: In relation to overall survival, 72.6% of the patients survived for a mean time of 14.43 years [standard error (SE) = 0.74; 95% CI: 12.98-15.87], with a cumulative survival rate of 49.8 ± 3%. Survival was reduced by the presence of tumor adjacent to resection margins [hazard ratio (HR) 2.20; 95% CI 1.09-4.43] (p = 0.028) and infiltrated resection margins (HR 3.86, 95% CI 1.56-9.57) (p = 0.004). Lymphadenectomy in turn increased survival (HR 0.15; 95% CI 0.06-0.42) (p < 0.001). In relation to disease-free survival, 55.3% of the patients suffered no relapse over a mean period of 9.91 years (SE = 0.66; 95% CI: 8.61-11.2), with a cumulative survival rate of 26.6% ± 8.4%. CONCLUSIONS: In tongue cancer patients, overall and specific survival were reduced in the presence of infiltrated resection margins. Lymphadenectomy in turn improved survival compared with patients in which this procedure was not carried out.

ABSTRACT

Bowen Family Systems Theory's central construct, differentiation of self (DoS), is one of the most recognized constructs to systemic researchers and psychology professionals. The present study reviewed the available evidence on DoS from the inception of the construct until July 31, 2020. A scoping review was conducted and a total of 295 primary studies were eligible for inclusion. Literature was categorized according to the hypotheses postulated by Bowen regarding: the associations between DoS and psychological and health outcomes and how DoS stability and similarity in partner levels operate within family systems. Descriptive data regarding studies on the measurement of DoS and in the area of psychotherapy were also compiled. Results revealed that there is ample support for DoS as a predictor of psychological health and marital quality, and that there are positive associations between DoS and better physical health and intergenerational relationships. Results of this review show that there continue to be underexplored hypotheses and a paucity of longitudinal or causal research designs to test the stability of DoS, the intergenerational transmission of DoS, and the efficacy of clinical interventions in modifying DoS to promote optimal well-being. Recommendations and next steps for researchers and practitioners are outlined.

ABSTRACT

Mobile technology has been used to appraise allergic rhinitis control, but more data are needed. To better assess the importance of mobile technologies in rhinitis control, the ARIA (Allergic Rhinitis and its Impact on Asthma) score ranging from 0 to 4 of the Allergy Diary was compared with EQ-5D (EuroQuol) and WPAI-AS (Work Productivity and Activity Impairment in allergy) in 1288 users in 18 countries. This study showed that quality-of-life data (EQ-5D visual analogue scale and WPA-IS Question 9) are similar in users without rhinitis and in those with mild rhinitis (scores 0-2). Users with a score of 3 or 4 had a significant impairment in quality-of-life questionnaires.

ABSTRACT

The endoneurial and/or perineurial origin of the outer core; i.e. the concentric and continuous lamellae located outside the complex formed by the axon and the Schwann-related cells, in human Pacinian corpuscles is still debated. Here we used immunohistochemistry coupled with a battery of antibodies to investigate the expression of perineurial (Glucose transporter 1 and epithelial membrane antigen) or endoneurial (CD34 antigen) markers in human digital Pacinian corpuscles. CD34 immunoreactivity was restricted to one layer immediately outside the inner core, whereas the proper outer core displayed antigens typical of the perineurial cells. These results demonstrate an intermediate endoneurial layer that divides the Pacinian corpuscles into two distinct compartments: the avascular inner neural compartment (formed by the axon and the Schwann-related cells that form the inner core), and the outer non-neural compartment (formed by the outer core). The functional relevance of these findings, if any, remains to be clarified.

Desde el descubrimiento de los rayos X por Roetgen en 1895, su utilización para el diagnóstico y, sobre todo, para el tratamiento de diferentes patologías ha ido en aumento. A pesar de que en los últimos años la tasa de efectos adversos ha disminuido, no debemos olvidar que pueden presentarse en los tejidos circundantes al área tratada. En este artículo hemos querido realizar una revisión de los efectos oculares más frecuentes y presentar el caso de una paciente con alteraciones corneales por la radiación (AU)


Since X-ray discoverment made by Roetgen in 1895, indications for the diagnosis and especially for the treatment of multiple pathologies have increased enormously. Despite during the last years the rate of side effects has decreased, we should not forget they may appear at the tissues around the treated area. In this article we have tried to make a review of ocular side effects, and to present the case of a patient with corneal affectation because of radiation (AU)

ABSTRACT

INTRODUCTION: Paragangliomas are usually benign tumors arising from chromaffin cells located outside the adrenal gland. Prostatic paraganglioma is an unusual entity in adult patients, with only 10 cases reported in the medical literature. CASE REPORT: A 34-year-old male with a history of chronic prostatitis consulted for perineal pain. On digital rectal examination the prostate was enlarged and firm, without nodules. The PSA level was 0.8 ng/mL and the catecholamines in the urine were elevated. On ultrasound a retrovesical 9 cm mass of undetermined origin measuring was present. A PET-CT scan showed a pelvic lesion measuring 9 cm with moderate increase in glucidic metabolism localized in the area of the prostate. A biopsy of the prostate revealed a neuroendocrine tumor, possibly a prostatic paraganglioma. A body scintigraphy with MIBG I-123 ruled out the presence of metastases or multifocal tumor. A radical prostatectomy with excision of the pelvic mass was performed under adrenergic blockade. One year after surgery the patient is asymptomatic and disease free. DISCUSSION/CONCLUSIONS: Prostatic paraganglioma is a rare, usually benign tumor, which should be considered in the differential diagnosis of prostate tumors in young males. Its diagnosis is based on the determination of catecholamine in blood and 24-hour urine and in imaging studies principally scintigraphy with MIBG I-123. Diagnostic confirmation is by histopathological study. The treatment consists of radical resection under adrenergic blockade and volume expansion. Given the limited number of cases reported, it is difficult to establish prognostic factors. Malignancy is defined by clinical criteria, and requires life long follow-up.

INTRODUCTION: Pediculosis capitis was very frequently reported in Cuba since the end of the 90's, particularly in some groups of school children and their relatives. The latter are involved in a chain of transmission of these insects and may play an important role at present as parasite reservoirs. OBJECTIVE: to report on the efficacy of the treatment of one female patient suffering from Pediculus capitis by using 5 % Indigofera suffruticosa Mill (añil cimarrón) tincture. METHODS: a case of persistent infestation with pediculosis capitis was described in which a 55 years-old patient was firstly treated with 1% permethrin solution and later with 5 % Indigofera suffruticosa Mill tincture. RESULTS: the microscopic identification of adult parasites and pre-adult stages of the parasite confirmed the presence of pediculus capitis. The hair treatment with 1 % permethrin was not effective after two applications. As an alternative, 5 % Indigofera suffruticosa Mill tincture was used and then the population of adult ectoparasites was reduced and the infestation was eliminated after the second application, with the paramedical staff continuously taking the nits out from the patient's hair. CONCLUSION: the use of this innocuous method may become a therapeutic alternative to treat this illness.

ABSTRACT

An abandoned cinnabar mining area located in the South-West of Spain has been studied with the aim of assessing its mercury pollution level and enhancing the knowledge about the Hg soil/plant relationship. To do so, soils and plants were sampled near an inactive smelter and around two mining sites present in this area. Critical total Hg concentrations were found in the close environs of pollutant sources. These also show high levels of elemental Hg (up to 8 mg kg(-1)), but quite low exchangeable Hg contents (0.008-0.038 mg kg(-1)). Most plant specimens display in their aboveground tissues Hg concentrations comprised in the range 0.1-10 mg kg(-1), with a great proportion (50%) showing critical levels. Greater Hg contents were found in plant specimens growing in soils with higher elemental Hg concentrations. The plant species displaying the greatest Hg levels are either perennial species of small-medium size and/or showing medium-highly corrugated leaves, or annual plants of small size. Marrubium vulgare L., Bromus madritensis L. and Trifolium angustifolium L. are the plant species with the highest Hg contents (37.6, 12.7 and 9.0 mg kg(-1), respectively). Leaf specific surface seems an important feature in the atmospheric Hg uptake by plants.

ABSTRACT

Introducción: El linfangioma es una malformación del sistemalinfático. El abordaje clásico ha sido la cirugía. El OK-432 (Picibanil) tieneacción esclerosante y se está utilizando cómo primer escalón terapéutico.El objetivo es aportar un nuevo caso de linfangioma tratado con OK-432y hacer una revisión de la literatura.Material y método: Aportamos un varón de 16 años con un linfangiomacervical macroquístico de 10 x 6 cm tratado con una dosis de OK-432.Resultados: A las 16 semanas del tratamiento, el tamaño del linfangiomaera de 6 x 2 cm, siendo clínicamente inapreciable.Discusión: El tratamiento con OK-432 tiene una alta tasa de curación,con una baja tasa de recidiva y una fibrosis circunscrita a la lesión. Enrelación con la cirugía, se evitan cicatrices y posibles lesiones de estructurasvitales(AU)


Introduction: Lymphangioma is a malformation of thelymphatic system. The classic approach is surgery. OK-432 (Picibanil)has sclerosing action and is being used as the first therapeutic step.The objective was to report a new case of lymphangioma treatedwith OK-432 and to review the literature.Material and method: We report the case of a 16-year-old manwith a 10x6-cm macrocystic cervical lymphangioma treated witha dose of OK-432.Results: At 16 weeks of treatment, the size of the lymphangiomawas 6x2 cm and it was clinically unappreciable.Discussion: OK-432 treatment has a high cure rate, low recurrencerate, and fibrosis circumscribed to the lesion. Compared to surgery,scars and possible harm to vital structures are avoided(AU)

ABSTRACT

Peripartum cardiomyopathy (PPCM) is a condition that affects women during the reproductive years in the late pregnancy period and/or early postpartum period. Although it is associated with several risk factors and various hypotheses exist of its etiology the cause of this disorder is still unknown. Standard therapy for PPCM is the same as for heart failure. Studies examining new therapeutic approaches are adding to the armamentarium available to physicians treating patients with PPCM. Despite all the current knowledge the mortality rates associated with PPCM remain relatively high. This article is a review of the current knowledge of etiology, diagnosis, treatment and prognosis of PPCM and attempts to present areas of need of further research.

ABSTRACT

La gestación heterotópica es aquella en la que coexisten una gestación intra y extrauterina. Su incidencia ha aumentado en la última década debido al incremento de la enfermedad tubárica y pélvica y al incremento del número de parejas sometidas a técnicas de reproducción asistida (TRA). El caso clínico pretende establecer la dificultad en el diagnóstico temprano de este tipo de gestaciones, tanto por la clínica inicial de un síndrome de hiperestimulación ovárica, como por la manifestación tardía (semana 16 de gestación) de una gestación heterotópica. En este caso se establece la duda de si la gestación ectópica asociada deriva de la transferencia embrionaria o de una gestación espontánea posterior. (AU)
