ABSTRACT

Selective recognition and sensing of catecholamine-based neurotransmitters by fluorescent synthetic receptors capable of operating in pure water is a central topic of modern supramolecular chemistry that impacts biological and analytical chemistry. Despite advances achieved in the recognition of some neurotransmitters such as dopamine, little effort has been invested in the optical recognition of other neurotransmitters of paramount importance in biochemistry and medicinal chemistry such as the drug L-dihydroxy-phenylalanine (levodopa). Herein, a cationic Cu(II)-terpyridine complex bearing an intramolecular fluorescent quinolinium ring covalently linked to phenylboronic acid (CuL1) was synthesized, structurally described by single-crystal X-ray diffraction and studied in-depth as a fluorescent receptor for neurotransmitters in water. The complex CuL1 was designed to act as a receptor for levodopa through two Lewis acids of different natures (Cu(II) and B atoms) as cooperative binding points. The receptor CuL1 was found to have a strongly acidified -B(OH)2 group (pKa = 6.2) and exceptionally high affinity for levodopa (K = 4.8 × 106 M-1) with selectivity over other related neurotransmitters such as dopamine, epinephrine, norepinephrine and nucleosides in the micromolar concentration range at physiological pH. Such levodopa affinity/selectivity for a boronic acid-based receptor in water is still rare. On the basis of spectroscopic tools (11B NMR, UV-vis, EPR, and fluorescence), high-resolution ESI-MS, crystal structure, and DFT calculations, the interaction mode of CuL1 with levodopa is proposed in a 1 : 1 model using two-point recognition involving a boronate-catechol esterification and a coordination bond Cu(II)-carboxylate. Furthermore, a visual sensing ensemble was constructed using CuL1 and the commercial fluorescent dye eosin Y. Levodopa is efficiently detected by the displacement of the eosin Y bound to the Cu(II)-receptor, monitoring its green emission. The use of Cu(II)-boronate complexes for fast and selective neurotransmitter sensing was unexplored until now.

ABSTRACT

ZnO nanoparticles (NPs) were prepared and characterized by different analytical methods and then they were used to decorate with N, N´-bis(salicylidene)ethylenediamine (salen) in order to perform as receptor for the metal ions in an aqueous medium. The results show that ZnO-salen selectively detects Al3+ ions in aqueous medium since the intensity of fluorescence has been enhanced significantly. However, the presence of K+ in the medium further intensified the fluorescence emission for the [ZnO-salen-Al3+] system. The above system has been applied to recognize Al3+ and K+ in cells by developing the cell images, for which, the fluorescence image is brightened if a human glioblastoma U251 cell contains [ZnO-salen-Al3+] + K+ ions, consisting of the fluorescence titration. The binding global constant for Al3+ and the subsequent recognition of K+ by ZnO-salen resulted in ß2(Al3+) = 6.61 × 103 and ß2(K+) = 3.71 × 103 with a detection limit of 36.51 µM for Al3+ and 17.39 µM for K+. In the cell toxicity analysis, the cell viability was over 85% for the ZnO-salen even in the concentration as high as 100 mM.

ABSTRACT

Selective recognition of fructosyl amino acids in water by arylboronic acid-based receptors is a central field of modern supramolecular chemistry that impacts biological and medicinal chemistry. Fructosyl valine (FV) and fructosyl glycyl histidine (FGH) occur as N-terminal moieties of human glycated hemoglobin; therefore, the molecular design of biomimetic receptors is an attractive, but very challenging goal. Herein, we report three novel cationic Zn-terpyridine complexes bearing a fluorescent N-quinolinium nucleus covalently linked to three different isomers of strongly acidified phenylboronic acids (ortho-, 2Zn; meta-, 3Zn and para-, 4Zn) for the optical recognition of FV, FGH and comparative analytes (D-fructose, Gly, Val and His) in pure water at physiological pH. The complexes were designed to act as fluorescent receptors using a cooperative action of boric acid and a metal chelate. Complex 3Zn was found to display the most acidic -B(OH)2 group (pKa = 6.98) and exceptionally tight affinity for FV (K = 1.43 × 105 M-1) with a strong quenching analytical response in the micromolar concentration range. The addition of fructose and the other amino acids only induced moderate optical changes. On the basis of several spectroscopic tools (1H, 11B NMR, UV-Vis, and fluorescence titrations), ESI mass spectrometry, X-ray crystal structure, and DFT calculations, the interaction mode between 3Zn and FV is proposed in a 1 : 1 model through a cooperative two-point recognition involving a sp3 boronate-diol esterification with simultaneous coordination bonding of the carboxylate group of Val to the Zn atom. Fluorescence quenching is attributed to a static complexation photoinduced electron transfer mechanism as evidenced by lifetime experiments. The addition of FGH to 3Zn notably enhanced its emission intensity with micromolar affinity, but with a lower apparent binding constant than that observed for FV. FGH interacts with 3Zn through boronate-diol complexation and coordination of the imidazole ring of His. DFT-optimized structures of complexes 3Zn-FV and 3Zn-FGH show a picture of binding which shows that the Zn-complex has a suitable (B⋯Zn) distance to the two-point recognition with these analytes. Molecular recognition of fructosyl amino acids by transition-metal-based receptors has not been explored until now.

Introducción: El análisis de la situación de salud es una investigación primordial que se realiza en el sistema de salud cubano, en el que se estudia la salud, los elementos que la determinan y las formas de mejorarla. Objetivo: Evaluar la propuesta de diseño de un esquema que contribuya a la mejora del aprendizaje del análisis de la situación de salud de la comunidad para los residentes de la especialidad Medicina General Integral, pertenecientes a la Universidad de Ciencias Médicas de Guantánamo. Método: Se realizó una investigación pedagógica de tipo descriptiva en la antes mencionada institución durante el período marzo-julio de 2022. De un universo de 67 profesores que aceptaron participar, se seleccionaron 26 por muestreo intencional. Para obtener los datos primarios fueron aplicados tres cuestionarios tipo Likert para evaluar el esquema, en consideración con la estructura, la funcionalidad y el aporte en el aprendizaje. Dichos cuestionarios incluyeron las respuestas: Totalmente en desacuerdo, En desacuerdo, Ni de acuerdo ni en desacuerdo, De acuerdo y Totalmente de acuerdo. Los datos se resumieron en números absolutos y porcentajes. Se presentaron en tablas de distribución de frecuencias. Resultados: Los subtotales de respuestas Totalmente de acuerdo y De acuerdo sobre la evaluación estructural y funcional, así como la influencia que se espera de este para mejorar el aprendizaje del análisis de situación de salud, se consideraron adecuados al superar el estándar. Conclusiones: El esquema propuesto es evaluado como adecuado en estructura y funcionalidad. Se considera positiva la influencia que se espera de este para mejorar el aprendizaje del análisis de situación de salud de la comunidad.(AU)


Introduction: The analysis of the health situation is a fundamental investigation carried out in the Cuban health system, in which health is studied, the elements that determine it and the ways to improve it. Objective: To evaluate the design proposal of a scheme that contributes to the improvement of learning of the analysis of the health situation of the community for the residents of the Comprehensive General Medicine specialty, belonging to the Universidad de Ciencias Médicas de Guantánamo. Method: A descriptive pedagogical research was carried out in the aforementioned institution during the period March-July 2022. From a universe of 67 teachers who agreed to participate, 26 were selected by intentional sampling. To obtain primary data, three Likert-type questionnaires were applied to evaluate the scheme, taking into consideration the structure, functionality and contribution to learning. These questionnaires included the responses: Totally disagree, Disagree, Neither agree nor disagree, Agree and Totally agree. Data were summarized in absolute numbers and percentages. They were presented in frequency distribution tables. Results: The subtotals of Completely Agree and Agree responses on the structural and functional evaluation, as well as the expected influence of this to improve the learning of health situation analysis, were considered adequate when exceeding the standard. Conclusions: The proposed scheme is evaluated as adequate in structure and functionality. The influence expected from this to improve learning about the analysis of the community's health situation is considered positive.(AU)


Introdução: A análise da situação sanitária é uma investigação fundamental realizada no sistema de saúde cubano, no qual se estuda a saúde, os elementos que a determinam e as formas de melhorá-la. Objetivo: Avaliar a proposta de desenho de um esquema que contribua para a melhoria da aprendizagem da análise da situação de saúde da comunidade para os residentes da especialidade Medicina Geral Integral, pertencente à Universidad de Ciencias Médicas de Guantánamo. Método: Foi realizada uma pesquisa pedagógica descritiva na referida instituição durante o período de março a julho de 2022. De um universo de 67 professores que aceitaram participar, 26 foram selecionados por amostragem intencional. Para obtenção dos dados primários, foram aplicados três questionários do tipo Likert para avaliação do esquema, levando em consideração a estrutura, funcionalidade e contribuição para a aprendizagem. Esses questionários incluíram as respostas: Discordo totalmente, Discordo, Nem concordo nem discordo, Concordo e Concordo totalmente. Os dados foram resumidos em números absolutos e porcentagens. Eles foram apresentados em tabelas de distribuição de frequência. Resultados: Os subtotais das respostas Concordo Totalmente e Concordo na avaliação estrutural e funcional, bem como a influência esperada desta para melhorar a aprendizagem da análise da situação de saúde, foram considerados adequados quando excedem o padrão. Conclusões: O esquema proposto é avaliado como adequado em estrutura e funcionalidade.A influência que se espera disto para melhorar a aprendizagem sobre a análise da situação de saúde da comunidade é considerada positiva.(AU)

ABSTRACT

Background: Reactive oxygen species (ROS) regulate glucose metabolism (GM) in skeletal muscle by improving the translocation of GLUT4. Antioxidant supplementation could block this physiological effect, altering glucose signaling during exercise. However, there is limited evidence in humans on whether antioxidant intake affects GM. Therefore, we aimed to determine the effect of an antioxidant cocktail (AOC) on GM at rest and during metabolic challenges. Methods: Ten healthy male subjects received AOC supplementation (1000 mg of Vitamin C, 600 IU of Vitamin E, and 600 mg of α-lipoic acid) or placebo (2.000 mg of talc) before two trials conducted 7 days apart. Trial 1: AOC 120 and 90 minutes before an endurance exercise (EEX) bout at 60 % of maximal oxygen uptake (VO2max); Trial 2: AOC 120 and 90 minutes before an oral glucose tolerance test (OGTT; 75 g glucose). Measurements of gas exchange and capillary blood samples were collected every 15 minutes during both trials. Results: AOC supplementation increased resting glucose levels (p<0.05). During Trial 1 (EEX), the AOC increased carbohydrate oxidation (CHOox) (p= 0.03), without effect in glucose blood levels. During Trial 2 (OGTT), the AOC supplementation had no significant effect on GM parameters. Conclusion: Acute supplementation with AOC increased resting glucose levels and CHOox during EEX in healthy subjects, with no effect on GM during the OGTT.


Antecedentes: Las especies reactivas de oxígeno (ROS) regulan el metabolismo de la glucosa (GM) en el músculo esquelético al mejorar la translocación de GLUT4. La suplementación con antioxidantes podría bloquear este efecto fisiológico, alterando la señalización de la glucosa durante el ejercicio. Sin embargo, existe evidencia limitada en humanos sobre si la ingesta de antioxidantes afecta el GM. Por lo tanto, nuestro objetivo fue determinar el efecto de un cóctel de antioxidantes (AOC) en el GM en reposo y durante desafíos metabólicos. Métodos: Sujetos sanos (sexo masculino; n= 10) recibieron suplementos de AOC (1.000 mg de vitamina C, 600 UI de vitamina E y 600 mg de ácido α-lipoico) o placebo (2.000 mg de talco) previo a dos pruebas realizadas con 7 días de diferencia. Prueba 1: AOC 120 y 90 minutos antes de una serie de ejercicio de resistencia (EEX) al 60% del consumo máximo de oxígeno (VO2max); prueba 2: AOC 120 y 90 minutos antes de una prueba de tolerancia oral a la glucosa (OGTT; 75 g de glucosa). Se obtuvieron datos de intercambio de gaseoso y muestras de sangre capilar cada 15 minutos durante ambas pruebas. Resultados: la suplementación con AOC aumentó los niveles de glucosa en reposo (p<0,05). Durante la prueba 1 (EEX), el AOC aumentó la oxidación de carbohidratos (CHOox) (p= 0,03), sin efecto en los niveles de glucosa en sangre. Durante la prueba 2 (OGTT), la suplementación con AOC no tuvo un efecto significativo en los parámetros de GM. Conclusión: Una suplementación aguda con AOC aumentó los niveles de glucosa en reposo y la CHOox durante EEX en sujetos sanos, sin efecto sobre el GM durante la OGTT.

Daily, people are exposed to chemicals and environmental compounds such as bisphenols (BPs). These substances are present in more than 80% of human fluids. Human exposure to BPs is associated with male reproductive health disorders. Some of the main targets of BPs are intercellular junction proteins of the blood-testis barrier (BTB) in Sertoli cells because BPs alter the expression or induce aberrant localization of these proteins. In this systematic review, we explore the effects of BP exposure on the expression of BTB junction proteins and the characteristics of in vivo studies to identify potential gaps and priorities for future research. To this end, we conducted a systematic review of articles. Thirteen studies met our inclusion criteria. In most studies, animals treated with bisphenol-A (BPA) showed decreased occludin expression at all tested doses. However, bisphenol-AF treatment did not alter occludin expression. Cx43, ZO-1, ß-catenin, nectin-3, cortactin, paladin, and claudin-11 expression also decreased in some tested doses of BP, while N-cadherin and FAK expression increased. BP treatment did not alter the expression of α and γ catenin, E-cadherin, JAM-A, and Arp 3. However, the expression of all these proteins was altered when BPA was administered to neonatal rodents in microgram doses. The results show significant heterogeneity between studies. Thus, it is necessary to perform more research to characterize the changes in BTB protein expression induced by BPs in animals to highlight future research directions that can inform the evaluation of risk of toxicity in humans.

Introduction: During the covid-19 pandemic, mental health services were unable to cope with the high demand from the population, so many people chose to search the Internet for information that could help them cope with the psychological process they were experiencing at the time. The aim of this study was to characterize the global search trend for the term «psychiatry¼ in the context of covid-19 using Google Trends and Our World in Data. Methods: Descriptive-cross-sectional study on global search trends for Psychiatry in the context of covid-19 under the terms «psychiatry¼, «depression¼, «anxiety¼, «stress¼, «insomnia¼ and «suicide¼ in the category of health, this was conducted over the period 2020-2021 and time graphs were generated. Results: The term «psychiatry¼ remained at a consistently high relative search volume (between 60 and 90), with a significant and gradual search in the month of April. The relative search volume for «depression¼, «anxiety¼ and «stress¼ remained constant with some non-significant fluctuations over the period 2020-2021. The term «insomnia¼ was predominant between January and June 2020, gradually declining in April and remaining constant until October 2021. Finally, the term «suicide¼ had a fluctuating RBV between 60 and 100 during this period. Conclusions: During the study period, the topics related to mental health and the speciality of psychiatry remained constant, with some fluctuating, but not outstanding variations.

Introducción: Las fístulas enterocutáneas representan una enfermedad grave que deben afrontar un gran número de cirujanos durante el ejercicio de la profesión. El tratamiento suele ser extremadamente complejo y siempre requiere de una intervención multidisciplinaria adaptada para cada caso en particular. Objetivo: Evaluar las características clínico-epidemiológicas y terapéuticas de las fístulas enterocutáneas posoperatorias. Métodos: Se realizó un estudio observacional, descriptivo, con recogida prospectiva de datos en el quinquenio 2013-2017. El universo fue de 28 pacientes diagnosticados con fístula enterocutánea posoperatoria y se usaron las variables: edad, sexo, carácter de la intervención, diagnóstico operatorio, tipo de fístula, tratamiento definitivo, complicaciones y pilares de tratamiento. Resultados: La edad media fue de 49 años. Las fístulas fueron más frecuentes en el sexo femenino (53,57 por ciento). El 78,57 por ciento de los pacientes fueron intervenidos con carácter de urgencia, y el 25 por ciento tuvieron un diagnóstico operatorio de oclusión intestinal mecánica por bridas seguida de la oclusión intestinal por tumor de colon izquierdo (17,86 por ciento). La hemicolectomía izquierda con anastomosis término-terminal (21,43 por ciento) y la resección intestinal con anastomosis término-terminal (17,86 por ciento) fueron los principales procedimientos quirúrgicos realizados. Conclusiones: Las fístulas de tipo II y de bajo gasto prevalecieron en la serie de casos en una media de tiempo que se corresponde con lo reportado en la literatura. La infección del sitio quirúrgico fue la complicación más observada y los pilares del tratamiento fueron cumplidos en la mayoría de los pacientes prevaleciendo el cierre espontáneo como tratamiento definitivo(AU)


Introduction: Enterocutaneous fistulas are a serious disease that a large number of surgeons must face during the practice of their profession. Their treatment is usually extremely complex and always requires a multidisciplinary intervention adapted to each particular case. Objective: To evaluate the clinical-epidemiological and therapeutic characteristics of postoperative enterocutaneous fistulas. Methods: An observational and descriptive study was performed, with prospective data collection, in the five-year period 2013-2017. The study universe was 28 patients diagnosed with postoperative enterocutaneous fistula. The following variables were used: age, sex, nature of the intervention, operative diagnosis, type of fistula, definitive treatment, complications and treatment cornerstones. Results: The mean age was 49 years. Fistulas were more frequent in the female sex (53.57 percent). 78.57 percent of the patients underwent emergency surgery, while 25 percent had an operative diagnosis of mechanical intestinal occlusion due to adherences, followed by intestinal occlusion due to left colon tumor (17.86 percent). Left hemicolectomy with end-to-end anastomosis (21.43 percent) and intestinal resection with end-to-end anastomosis (17.86 percent) were the main surgical procedures. Conclusions: Type II and low-output fistulas prevailed in the case series at a mean time that corresponds to that reported in the literature. Surgical site infection was the most frequently observed complication, while the treatment cornerstones were fulfilled in most patients, with spontaneous closure prevailing as a definitive treatment(AU)

ABSTRACT

Three new diboronic acid-substituted bisquinolinium salts were synthesized, structurally described by single-crystal X-ray diffraction, and studied in-depth as fluorescent receptors for six monosaccharides and two open-chain polyols in water at physiological pH. The dicationic pyridine-2,6-dicarboxamide-based receptors contain two N-quinolinium rings as the fluorescent units covalently linked to three different isomers of phenylboronic acid (ortho, 2; meta, 3; and para, 4) as chelating binding sites for polyols. Additions of glucose/fructose in the micromolar concentration range to receptors 2 and 3 induce significant fluorescence changes, but in the presence of arabinose, galactose, mannose, and xylose, only modest optical changes are observed. This optical change is attributed to a static photoinduced electron transfer mechanism. The meta-diboronic receptor 3 exhibited a high affinity/selectivity toward glucose (K = 3800 M-1) over other monosaccharides including common interfering species such as fructose and mannitol. Based on multiple spectroscopic tools, electrospray ionization high-resolution mass spectrometry, crystal structures, and density functional theory calculations, the binding mode between 3 and glucose is proposed as a 1:1 complex with the glucofuranose form involving a cooperative chelating diboronate binding. These results demonstrate the usefulness of a new set of cationic fluorescent diboronic acid receptors with a strong ability for optical recognition of glucose in the sub-millimolar concentration range.

Plant responses to phosphate starvation (-Pi) are very well characterized at the biochemical and molecular levels. The expression of thousands of genes is modified under this stress condition, depending on the action of Phosphate starvation response 1 (PHR1). Existing data indicate that neither the PHR1 transcript nor the quantity or localization of its protein increase during nutrient stress, raising the question of how its activity is regulated. Here, we present data showing that SnRK1 kinase is able to phosphorylate some phosphate starvation response proteins (PSRs), including PHR1. Based on a model of the three-dimensional structure of the catalytic subunit SnRK1α1, docking simulations predicted the binding modes of peptides from PHT1;8, PHO1 and PHR1 with SnRK1. PHR1 recombinant protein interacted in vitro with the catalytic subunits SnRK1α1 and SnRK1α2. A BiFC assay corroborated the in vivo interaction between PHR1 and SnRK1α1 in the cytoplasm and nucleus. Analysis of phosphorylated residues suggested the presence of one phosphorylated site containing the SnRK1 motif at S11, and mutation in this residue disrupted the incorporation of 32 P, suggesting that it is a major phosphorylation site. Electrophoretic mobility shift assay results indicated that the binding of PHR1 to P1BS motifs was not influenced by phosphorylation. Importantly, transient expression assays in Arabidopsis protoplasts showed a decrease in PHR1 activity in contrast with the S11A mutant, suggesting a role for Ser11 as a negative regulatory phosphorylation site. Taken together, these findings suggest that phosphorylation of PHR1 at Ser11 is a mechanism to control the PHR1-mediated adaptive response to -Pi.

Resumen La neumonía sigue siendo una de las principales causas de consulta y de hospitalización a la que, además de su un alto impacto en términos de morbilidad y mortalidad, se suma la actual problemática de resistencia a los antimicrobianos, por lo que establecer directrices que permitan su adecuado diagnóstico y tratamiento es de gran importancia para obtener mejores desenlaces clínicos y promover un uso racional de antibióticos en estos pacientes. La presente guía de práctica clínica (GPC) contiene recomendaciones basadas en la evidencia para el diagnóstico y tratamiento de la neumonía adquirida en la comunidad en adultos, las cuales fueron realizadas mediante el proceso de adaptación de GPC basadas en la evidencia para el contexto colombiano.


Abstract Pneumonia continues to be one of the main causes of consultation and hospitalization to which, besides its high impact on morbidity and mortality, the current problem of antimicrobial resistance is added; thus, establishing guidelines that allow its adequate diagnosis and treatment is of great importance to obtain better clinical outcomes and promote a rational use of antibiotics in these patients. This clinical practice guideline (CPG) contains evidence-based recommendations for the diagnosis and treatment of community-acquired pneumonia in adult population; these recommendations were made by means of the process of adaptation of evidence-based CPGs for the Colombian context.

ABSTRACT

The presence of cardio-metabolic and respiratory comorbidities, immunosuppression, and chronic kidney disease have been associated with an increase in mortality from COVID-19. The objective of this study is to establish the risk factors associated with 30-day mortality in a cohort of hospitalized patients with COVID-19. This paper conducts a retrospective and analytical study of patients hospitalized for COVID-19 in a tertiary care center. A Cox proportional hazard analysis was performed to estimate the association of comorbidities with 30-day mortality. A total of 1215 patients with a median age of 59 years were included. In the adjusted Cox proportional hazards regression model, hypothyroidism, D-dimer ≥ 0.8 µg/mL, LHD ≥ 430 IU/L, CRP ≥ 4.83 ng/mL, and triglycerides ≥ 214 mg/dL were associated with an increased risk of death. The presence of a history of hypothyroidism and biomarkers (D-dimer, lactic dehydrogenase, CRP, and triglycerides) were associated with an increase in mortality in the studied cohort.

INTRODUCCIÓN: El profesional de enfermería colabora en la prestación de cuidado de la salud de personas que presentan invalidez congénita y adquirida. Las consecuencias asociadas a lesiones medulares no se derivan de la lesión en sí, sino de la falta de efectividad de los servicios de atención médica. OBJETIVO: Identificar intervenciones de enfermería en la práctica social para la salud de la persona posoperada por lesión medular. MÉTODO: Estudio cualitativo, abordaje descriptivo-exploratorio. Paciente posoperado por lesión medular, (Durango, México). Se utilizó la entrevista semiestructurada en el año 2020, a través del análisis de discurso oral. RESULTADOS: Se evidenció, la presencia de barreras sociales, que coartan el pleno desarrollo y participación de la persona con discapacidad por lesión medular en su diario vivir con la familia, comunidad y sociedad. También la autonomía de la persona no sólo se coarta al hecho de realizar tareas o labores o participar con otras personas. Incluso engloba las capacidades de pensar, de decidir por ellos, decidiendo por sí solo y que estas decisiones sean respetadas por los demás. CONCLUSIONES: Se hace necesaria la contribución del profesional de enfermería en las Políticas Públicas en el ámbito local y del país donde se propongan y establezcan, estrategias acordes a la prestación de servicios: atención a la enfermedad, promoción, prevención y rehabilitación incluyéndolo en las actividades y servicios sociales; por otra parte las necesidades de las personas con discapacidades deben considerarse también en las políticas, programas y proyectos desarrollados a nivel local y nacional. Estas personas deben tener acceso a los servicios acostumbrados, sostenidos con servicios especializados para mejorar de esta manera la calidad de vida de la persona con discapacidad.


INTRODUCTION: The nursing professional collaborates in the provision of health care for people with congenital and acquired disabilities. The consequences associated with spinal cord injuries do not derive from the injury itself, but from the lack of effectiveness of medical care services. OBJECTIVE: To identify nursing interventions in social practice for the health of the post-operated person for spinal cord injury. Method: Qualitative study, descriptive-exploratory approach. Postoperative patient for spinal cord injury, (Durango, Mexico). The semi-structured interview was used in the year 2020, using oral discourse analysis. RESULTS: The presence of social barriers was evidenced, which restrict the full development and participation of the person with disabilities due to spinal cord injury in her daily life with the family, community and society. Also the autonomy of the person is not only limited to the fact of carrying out tasks or tasks or participating with other people. It even encompasses the ability to think, to decide for them, deciding for themselves and for these decisions to be respected by others. CONCLUSIONS: The contribution of the nursing professional is necessary in Public Policies at the local level and in the country where strategies according to the provision of services are proposed and established: care for illness, promotion, prevention and rehabilitation, including it in the activities and social services; on the other hand, the needs of people with disabilities must also be considered in the policies, programs and projects developed at the local and national levels. These people must have access to the usual services, supported by specialized services to improve the quality of life of the person with disabilities.

Los datos de acceso público en plataformas de redes sociales se pueden utilizar para identificar percepciones, actitudes y sentimientos en relación con la COVID-19; ello puede ser útil para formular políticas de estado y enfrentar adecuadamente la emergencia. El presente trabajo tuvo por objetivo analizar las publicaciones y percepciones de los usuarios a través de sus comentarios en Facebook sobre el fármaco ivermectina durante la pandemia de COVID-19 en Perú. Las publicaciones y sus comentarios fueron seleccionados mediante búsqueda manual, utilizando los términos: ivermectina e #ivermectina, en julio de 2020. Su interpretación se efectuó por análisis de contenido cualitativo convencional, con proceso inductivo y apoyado por literatura científica. Se encontraron 17 publicaciones realizadas entre el 18 de mayo y 7 de julio del 2020, clasificadas en 5 temáticas: rumores (58,8 por ciento), basadas en evidencia científica (11,8 por ciento), precaución (11,8 por ciento), producción del fármaco (11,8 por ciento) y confrontación (5,9 por ciento). El 70,0 por ciento de los rumores surgieron de páginas de medios de comunicación. Los comentarios se clasificaron, según percepciones de los usuarios hacia la ivermectina, en cuatro temas: ivermectina como cura efectiva (64,8 por ciento), solicitud de información (25,3 por ciento), en contra del consumo de ivermectina (6,3 por ciento) y otros tratamientos (3,6 por ciento). Más de la mitad de la información sobre ivermectina en Facebook aparece en rumores, en su mayoría originados por páginas de medios de comunicación. Entre los comentarios se destaca la percepción de los usuarios que la consideran cura efectiva contra la COVID-19(AU)


The purpose of the study was to analyze Facebook publications and perceptions of users, expressed in their comments, about the drug ivermectin during the COVID-19 pandemic in Peru. The publications and their comments were selected by manual search using the terms ivermectin and #ivermectin in July 2020. Interpretation of publications and their comments was performed by conventional qualitative content analysis with inductive processing and supported by scientific literature. A total 17 publications were retrieved from the period extending from 18 May to 7 July 2020. These were classified into five topics: rumors (58.8percent), based on scientific evidence (11.8percent), caution (11.8percent), production of the drug (11.8percent) and confrontation (5.9percent). Paradoxically, 70.0percent of the rumors were spread by mass media pages. Comments, on the other hand, were classified into four topics based on user perceptions: ivermectin as effective cure (64.8percent), request for information about ivermectin (25.3percent), against the use of ivermectin (6.3percent) and other treatments (3.6percent). As a conclusion, it was found that more than half of the information about ivermectin circulating in Facebook publications is classified as rumors, and most is spread by mass media pages. Analysis of the comments found that user perception considers ivermectin to be the effective cure against COVID-19(AU)

ABSTRACT

In January 2021, the Chilean city of Concepción experienced a second wave of coronavirus 2019 (COVID-19) while in early April 2021, the entire country faced the same situation. This outbreak generated the need to modify and validate a method for detecting severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) in saliva, thereby expanding the capacity and versatility of testing for COVID-19. This study was conducted in February 2021 in the Chilean city of Concepción during which time, the town was under total quarantine. The study participants were mostly symptomatic (87.4%), not hospitalized, and attended care centers because of their health status rather than being asked by the researchers. People coming to the health center in Concepción to be tested for COVID-19 (via reverse transcriptase polymerase chain reaction [RT-PCR]) from a specimen of nasopharyngeal swab (NPS) were then invited to participate in this study. A total of 131 participants agreed to sign an informed consent and to provide saliva and NPS specimens to validate a method in terms of sensitivity, specificity, and statistical analysis of the cycle threshold (Ct) values from the RT-PCR. Calculations pertaining to the 127 participants who were ultimately included in the analysis showed sensitivity and specificity at 94.34% (95% CI: 84.34-98.82%) and 98.65% (95% CI: 92.70-99.97%), respectively. The saliva specimen showed a performance comparable to NPS as demonstrated by the diagnostic parameters. This RT-PCR method from the saliva specimen is a highly sensitive and specific alternative compared to the reference methodology, which uses the NPS specimen. This modified and validated method is intended for use in the in vitro diagnosis of SARS-CoV-2, which provides health authorities in Chile and local laboratories with a real testing alternative to RT-PCR from NPS.

ABSTRACT

This work details a methodology of design and test of a new prototype emergency mechanical ventilator called Fenix for the COVID-19 crisis in Peru. This equipment was manufactured with industrial equipment for the embedded and pneumatic systems, such as a Programmable Logic Controller (PLC), proportional flow valves, sensors, uninterruptible power supply (UPS), industrial panel HMI 15" and other electrical and pneumatic parts from Festo and Schneider Electric. This selection was in accordance with safety requirements based on ISO 80601-2-12: 2020-02. This study included two ventilatory modes, pressure- controlled in continuous mandatory ventilation (PC-CMV) and volume-controlled in continuous mandatory ventilation (VC-CMV), these control algorithms were evaluated analytically and experimentally in a FLUKE VT-650 Gas Flow Analyzer and an Acculung Fluke connected with a computer for comparing 9 ventilatory parameters in 4 different states as µ, simulation of the variation of the pressure control in a patient, and Ï´, simulation of alveolar recruitment in an intensive care patient, both states to PC-CMV, and also ß, simulation of the variation of the flow control in a patient, and α, simulation of alveolar recruitment in an intensive care patient, both last states to VC-CMV. Additionally, we study the pressure, volume, and flow graphs in the Fenix user interface for comparison with data recovered from Fluke Medical VT650 Gas Flow Analyzer. The results demonstrate an error in the flow measurement for the 4 states due to the peaks that are not detected by the low-pass filter of the sensor, however, a similar trend is seen in the control ventilatory graphs of the calibrator. Finally, the ventilator prototype provides ventilatory support, with a maximum tidal volume error of 12.93 % and inspiratory pressure of -20.15 % with respect to the set value; and it allows to monitor the main ventilation parameters with a calculation error between -6 to 25 %.Clinical Relevance - Established the design of emergency mechanical ventilator using PLC and industrial components.

ABSTRACT

Gestational diabetes mellitus (GDM) is one of the most frequent pregnancy complications with potential adverse outcomes for mothers and newborns. Its effects on the newborn appear during the neonatal period or early childhood. Therefore, an early diagnosis is crucial to prevent the development of chronic diseases later in adult life. In this study, the urinary metabolome of babies born to GDM mothers was characterized. In total, 144 neonatal and maternal (second and third trimesters of pregnancy) urinary samples were analyzed using targeted metabolomics, combining liquid chromatographic mass spectrometry (LC-MS/MS) and flow injection analysis mass spectrometry (FIA-MS/MS) techniques. We provide here the neonatal urinary concentration values of 101 metabolites for 26 newborns born to GDM mothers and 22 newborns born to healthy mothers. The univariate analysis of these metabolites revealed statistical differences in 11 metabolites. Multivariate analyses revealed a differential metabolic profile in newborns of GDM mothers characterized by dysregulation of acylcarnitines, amino acids, and polyamine metabolism. Levels of hexadecenoylcarnitine (C16:1) and spermine were also higher in newborns of GDM mothers. The maternal urinary metabolome revealed significant differences in butyric, isobutyric, and uric acid in the second and third trimesters of pregnancy. These metabolic alterations point to the impact of GDM in the neonatal period.

Introducción: La pandemia de la COVID-19 ha generado un incremento de la información sobre esta enfermedad, por lo que es fundamental garantizar la credibilidad y confiabilidad de las páginas web que brindan esta información. Objetivo: Evaluar la confiabilidad de la información sanitaria en español sobre la COVID-19 en el motor de búsqueda Google considerando los criterios de la herramienta HONcode. Material y métodos: Estudio observacional de corte transversal. Las páginas web de Google se analizaron en diciembre del 2020 desde Lima-Perú, utilizando 4 términos de búsqueda. Se evaluó la confiabilidad de la información sanitaria de las páginas web mediante la herramienta HONcode (versión 3.1.3). Se clasificaron según la fuente de información y su procedencia. El análisis estadístico se realizó para un nivel de significancia de p<0,05. Resultados: Se evaluaron 200 páginas web en español, el 16,5% poseían certificado HONcode, la mayoría fue de la OMS (33,3%), la principal fuente de información fue "académica-profesional" (30,0%). Además, el 33,0% de las páginas web eran peruanas, siendo mayormente de tipo gubernamental (42,4%), pero ninguna tenía certificado HONcode. Conclusiones: Solo una de cada seis páginas web proporcionaba información sanitaria confiable sobre la COVID-19. Además, se distingue la presencia de las páginas web de la OMS en proveer información sanitaria sobre la COVID-19 en Google. Si bien este estudio destaca las páginas web de organismos internacionales, se requiere fortalecer la comunicación desde las páginas web gubernamentales peruanas.


Introduction: The COVID-19 pandemic has generated an increase in information about this disease, so it is essential to ensure the credibility and reliability of the web pages that provide this information. Objective: To evaluate the reliability of health information in Spanish on COVID-19 in the Google search engine considering the criteria of the HONcode tool. Material and methods: Observational cross-sectional study. Google web pages were analyzed in December 2020 from Lima-Peru, using 4 search terms. The reliability of the health information on the web pages was assessed using the HONcode tool (version 3.1.3). They were classified according to the source of information and its provenance. Statistical analysis was performed for a significance level of p<0.05. Results: 200 web pages in Spanish were evaluated, 16.5% were HONcode certified, most of them were from the WHO (33.3%), the main source of information was "academic-professional" (30.0%). In addition, 33.0% of the web pages were Peruvian, being mostly governmental (42.4%), but none had HONcode certificate. Conclusions: Only one out of six web pages provided reliable health information on COVID-19. In addition, the presence of WHO web pages in providing health information about COVID-19 on Google stands out. Although this study highlights the web pages of international organizations, there is a need to strengthen communication from Peruvian governmental web pages.

ABSTRACT

Fructooligosaccharides (FOSs)-fructose-based oligosaccharides-are typical prebiotics with health-promoting effects in humans and animals. The trisaccharide 1-kestotriose is the most attractive inulin-type FOS. We previously reported a recombinant sucrose:sucrose 1-fructosyltransferase (1-SST, EC 2.4.1.99) from Schedonorus arundinaceus (Sa) that efficiently converts sucrose into 1-kestotriose. In this study, Pichia pastoris PGFT6x-308 constitutively expressing nine copies of the Sa1-SST gene displayed fructosyltransferase activity in undisrupted biomass (49.8 U/ml) and culture supernatant (120.7 U/ml) in fed-batch fermentation (72 hr) with sugarcane molasses. Toluene permeabilization increased 2.3-fold the Sa1-SSTrec activity of whole cells entrapped in calcium-alginate beads. The reaction with refined or raw sugar (600 g/l) yielded 1-kestotriose and 1,1-kestotetraose in a ratio of 8:2 with their sum representing above 55% (wt/wt) of total carbohydrates. The FOSs yield decreased to 45% (wt/wt) when sugarcane syrup and molasses were used as cheaper sucrose sources. The beads retained 80% residual Sa1-SSTrec activity after a 30-day batchwise operation with refined cane sugar at 30°C and pH 5.5. The immobilized biocatalyst is attractive for the continuous production of short-chain FOSs, most particularly 1-kestotriose.

ABSTRACT

INTRODUCTION: Parathyroidectomy is the curative treatment option in primary hyperparathyroidism (PHPT). The decrease of parathormone (PTH) by 50% or more from levels prior to surgery after excision predicts successful parathyroidectomy. Serum calcium is expected to return to normal within 24-72 hours after the surgery; however, nearly 10% have transient, persistent postoperative hypercalcemia. We present a case report of delayed calcium normalization after successful parathyroidectomy in a 38-year-old patient with PHPT. METHODS: Parathyroidectomy was performed, with evidence of a decrease in PTH levels of more than 50% in the first 24 hours postoperatively compared to presurgical PTH; however, despite curative parathyroidectomy, a delayed calcium normalization was evidenced, with hypercalcemia persistence up to 120 hours postoperatively. RESULTS: After the first month postoperatively, serum calcium remained normal. In conclusion, approximately 10% of patients with curative parathyroidectomy have transient, persistent postoperative hypercalcemia, which is more likely to occur in patients with higher preoperative serum calcium and PTH levels. CONCLUSION: Persistent hypercalcemia after the first month postoperatively is related with persistent PHPT, highlighting the importance of calcium monitoring after parathyroidectomy to predict short-term, medium-term, and long-term outcomes and prognosis.
