ABSTRACT

INTRODUCTION: Patients with high-risk prostate cancer (HRPCa) are prone to have worse pathological features, resulting in early biochemical recurrence after radical prostatectomy (RP). There is an urgent need to develop novel treatment strategies for this group of patients to optimize their outcomes. The purpose of this study is to perform a systematic review of the role of neoadjuvant hormonal therapy (NHT) followed by RP in HRPCa patients. EVIDENCE ACQUISITION: We performed a systematic review of the following databases, MEDLINE (PubMed), EMBASE, Cochrane Library, and clinical Trial.gov; between January 2007 and August 2023, following the PRISMA guidelines. EVIDENCE SYNTHESIS: After screening and deduplication, we included ten studies from an initial pool of 1275. The risk of bias was low in observational studies but ranged from moderate to low in controlled trials. Five studies utilized traditional androgen deprivation treatments (ADT), revealing favorable pathological outcomes but inconsistency in evaluating oncological results. Additionally, four studies focused on RP combined with androgen receptor pathway inhibitors (ARPIs) in the NHT setting, all showing primarily positive pathological outcome, with no clear evidence of an oncological benefit. Limited long-term follow-up data and a shortage of randomized controlled trials were evident among all the studies included in this review, regardless of the type of hormonal treatment used. CONCLUSIONS: Different hormonal treatments, including traditional ADT and ARPIs, yield positive pathology outcomes. Oncological evidence remains limited, echoing older findings predating ARPIs. Definitive conclusions require longer follow-ups and precise patient selection. Currently, insufficient evidence support ARPIs' superiority over conventional therapy before RP.

ABSTRACT

El envejecimiento global es uno de los grandes desafíos sanitarios del siglo XXI, el 2030 una de cada seis personas tendrá ≥ 60 años. En Chile se proyecta un 32,1% para el 2050. Esta realidad demográfica desafía a la enfermería en el abordaje multidimensional del envejecimiento. La espiritualidad como atributo de la autotrascendencia es un factor condicionante de acciones promotoras de salud en las personas mayores. Desde la perspectiva de la Dra. Reed, se presentan los resultados de validación de su Escala de Perspectiva Espiritual. La población fue de 202 personas mayores en promedio de 71 años (D.E.= 8,2338). El método utilizado fue diseño psicométrico y corte transversal. La escala general presentó; α de Cronbach=0,92; dimensiones prácticas espirituales α de Cronbach= 0,822; dimensión creencias espirituales α de Cronbach=0,914. El análisis de modelo de un factor (CFI=0,956, RMSEA=0,167 IC95% [0,147 - 0,188] y un c2=232,762; p=0,000), como de dos factores coherentes con la estructura teórica propuesta por Reed (CFI=0,984; RMSEA=0,102 IC95% [0,081 ­ 0,125] y c2=105,890; p=0,000), presentaron cargas factoriales altas y relativamente homogéneas. La escala general y las subescalas de creencias y prácticas espirituales mostraron adecuada confiabilidad. El análisis de modelos anidados mostró un mejor ajuste del modelo de dos factores. Los resultados suman para afirmar que la Escala de Perspectiva Espiritual, contribuye a la precisión empírica, potencial heurístico y verificabilidad de la Teoría de Transcendencia de Reed. Se vislumbra que la espiritualidad es un factor mediador en la relación entre la vulnerabilidad, autotrascendencia y bienestar de la persona mayor.


Global aging is one of the great health challenges of the 21st century. By 2030, one in six people will be ≥60 years of age, with 32.1% projected for Chile by 2050. This demographic reality challenges nursing in the multidimensional approach to aging. The dimension of spirituality as an attribute of self-transcendence is a conditioning factor of health promoting actions in the elderly. The validation results of Dr. Reed's Spiritual Perspective Scale are presented. The population included 202 people older than 71 years on average (SD= 8.2338). The method used was a psychometric and cross-sectional design. The general scale presented Cronbach's α=0.92; spiritual practical dimension, Cronbach's α=0.822; spiritual beliefs dimension, Cronbach's α=0.914. The analysis of a one-factor model (CFI=.956, RMSEA=0.167 IC95% [0.147-0.188] and c2=232.762; p=0.000), as two factors consistent with the theoretical structure proposed by Reed (CFI=0.984; RMSEA=0.102 IC95% [0.081-0.125] and c2=105.890; p=0.000), presented high and relatively homogeneous factorial loads. The general scale and the subscales of spiritual beliefs and practices showed adequate reliability. The analysis of nested models showed a better adjustment of the two-factor model. The results collectively affirm that the Spiritual Perspective Scale contributes to the empirical precision, heuristic potential, and verifiability of Reed's Theory of Transcendence. It is observed that spirituality is a mediating factor in the relationship between vulnerability, self-transcendence, and well-being of humans.

ABSTRACT

Background: Congenital heart disease (CHD) entails structural defects in the morphogenesis of the heart or its main vessels. Analyzing exercise capacity of children and adolescents with CHD is important to improve their functional condition and quality of life, since it can allow timely intervention on poor prognostic factors associated with higher risk of morbidity and mortality. Objective: To describe exercise capacity in children and adolescents with CHD compared with healthy controls. Methods: A systematic review was carried out. Randomized clinical trials and observational studies were included assessing exercise capacity through direct and indirect methods in children and adolescents between 5 and 17 years-old. A sensitive analysis was performed including studies with CHD repaired participants. Additionally, it was sub-analyzed by age range (< and ≥ 12 years old). Two independent reviewers analyzed the studies, extracted the data, and assessed the quality of the evidence. Results: 5619 articles were found and 21 were considered for the review. Eighteen articles used the direct exercise capacity measurement method by cardiopulmonary exercise test (CPET). The CHD group showed significant differences in peak oxygen consumption (VO2peak) with a value of -7.9 ml/Kg/min (95% CI: -9.9, -5.9, p = 0.00001), maximum workload (Wmax) -41.5 (95% CI: -57.9, -25.1 watts, p = 0.00001), ventilatory equivalent (VE/VCO2 ) slope 2.6 (95% CI: 0.3, 4.8), oxygen pulse (O2 pulse)-2.4 ml/beat (95% CI: -3.7, -1.1, p = 0.0003), and maximum heart rate (HRmax) -15 bpm (95% CI: -18, -12 bpm, p = 0.00001), compared with healthy controls. Adolescents (≥ 12 yrs) with CHD had a greater reduction in VO2peak (-10.0 ml/Kg/min (95% CI: -12.0, -5.3), p < 0.00001), Wmax (-45.5 watts (95% CI: -54.4, -36.7), p < 0.00001) and HRmax (-21 bpm (95% CI: -28, -14), p<0.00001). Conclusion: Suffering CHD in childhood and adolescence is associated with lower exercise capacity as shown by worse VO2peak, Wmax, VE/VCO2 slope, O2 pulse, and HRmax compared with matched healthy controls. ABSTRACT

The aim of this study is the identification of metabolomic biomarkers of sepsis and sepsis-induced acute kidney injury (AKI) in an experimental model. Pigs were anesthetized and monitored to measure mean arterial pressure (MAP), systemic blood flow (QT), mean pulmonary arterial pressure, renal artery blood flow (QRA), renal cortical blood flow (QRC), and urine output (UO). Sepsis was induced at t = 0 min by the administration of live Escherichia coli ( n = 6) or saline ( n = 8). At t = 300 min, animals were killed. Renal tissue, urine, and serum samples were analyzed by nuclear magnetic resonance (NMR) spectroscopy. Principal component analyses were performed on the processed NMR spectra to highlight kidney injury biomarkers. Sepsis was associated with decreased QT and MAP and decreased QRA, QRC, and UO. Creatinine serum concentration and neutrophil gelatinase-associated lipocalin (NGAL) serum and urine concentrations increased. NMR-based metabolomics analysis found metabolic differences between control and septic animals: 1) in kidney tissue, increased lactate and nicotinuric acid and decreased valine, aspartate, glucose, and threonine; 2) in urine, increased isovaleroglycine, aminoadipic acid, N-acetylglutamine, N-acetylaspartate, and ascorbic acid and decreased myoinositol and phenylacetylglycine; and 3) in serum, increased lactate, alanine, pyruvate, and glutamine and decreased valine, glucose, and betaine concentrations. The concentration of several metabolites altered in renal tissue and urine samples from septic animals showed a significant correlation with markers of AKI (i.e., creatinine and NGAL serum concentrations). NMR-based metabolomics is a potentially useful tool for biomarker identification of sepsis-induced AKI.

ABSTRACT

Chest physiotherapy is currently an outstanding therapeutic tool for the management of pediatric respiratory patients. Many and varied techniques, manual and assisted, have been developed, some requiring patient collaboration. The development of knowledge and the pathophysiological understanding of respiratory diseases in recent years has generated the need to agree on the terminology, the fundamentals and the application of each of the kinesthetic techniques of respiratory treatment in the pediatric field. The objective of this document was to compile the available literature related to the physiological mechanisms, indications, procedural aspects and most frequently used techniques in our country, with the purpose of being a guideline for the clinical practice of professionals and students. The following techniques were included: prolonged slow expiration, slow expiration with glottis opened, autogenic drainage, assisted autogenic drainage, active cycle, compression/decompression, airway clearance, forced expiration techniques, cough and respiratory exercises.


La kinesiología respiratoria es actualmente una herramienta terapéutica destacada para el manejo de los pacientes respiratorios pediátricos. Se han desarrollado muchas y variadas técnicas, manuales y asistidas, algunas requiriendo colaboración del paciente. El desarrollo del conocimiento y la comprensión fisiopatológica de las enfermedades respiratorias en los últimos años ha generado la necesidad de consensuar la terminología, los fundamentos y la aplicación de cada una de las técnicas kinésicas de tratamiento respiratorio en el ámbito pediátrico. El objetivo de este documento fue recopilar la literatura disponible relacionada con los mecanismos fisiológicos, indicaciones, aspectos procedimentales y técnicas más utilizadas en nuestro país, con el propósito de ser una directriz que oriente la práctica clínica de profesionales y estudiantes en formación. Se incluyeron así las siguientes técnicas: espiración lenta prolongada, espiración lenta con glotis abierta, drenaje autógeno, drenaje autógeno asistido, ciclo activo, presión/descompresión, bloqueos torácicos, técnicas de espiración forzada, tos y ejercicios respiratorios.

BACKGROUND: Frailty is not universal among older people but increases the risk of dependence. AIM: To assess frailty among older people and its relationship with biological, psychological and social factors. MATERIAL AND METHODS: Seven hundred fifty four older people aged 73 ± 6 years (61% females), attending a public primary care were assessed. Frailty was defined according to Fried criteria that considers inexplicable weight loss, tiredness, muscle weakness and lack of physical activity. RESULTS: Absence of frailty, pre-frailty and frailty was found in 26, 69 and 5% of participants, respectively. Significant differences between frailty groups were observed for age, gender, years of studies, minimental and self-efficacy scores. Among participants defined as being in a pre-frail condition, 59% were non-disabled without risk and 41% non-disabled in risk, according to the functional assessment for older people used in Chilean primary care clinics. CONCLUSIONS: Frailty among older people is associated with increasing age, education, cognitive status and self-efficacy.

ABSTRACT

Objetivo: Analizar el Registro para la Localización y la Caracterización de las personas con discapacidad, en relación a los marcos de referencia. Metodología: Investigación de tipo descriptiva transversal, desarrollada entre los años 2009-2010 en la Universidad de Boyacá, que contempla la aplicación del registro en los municipios del departamento y posterior interpretación de la información, con análisis de contenido y estadístico descriptivo; este último, realizado con base en los datos suministrados por el Departamento Nacional de Estadísticas y por el Ministerio de Salud y Protección Social, de los 123 municipios de Boyacá (42.705 registros), denominado “Análisis del Registro para la Localización y la Caracterización de las personas con discapacidad, en relación con los marcos de referencia en el departamento de Boyacá”. Resultados: La distribución de las preguntas del registro, se focalizan en detectar las características correspondientes al marco de protección social básica con el 58,42%, el cual se centra en la subsistencia, protección y afecto; con un 38,20% y en segunda instancia, se ubica el marco de compensación y cohesión social y finalmente, se encuentra el marco de equiparación y desarrollo local con un 3,37%. De igual forma, se observa que el predominio de las personas con discapacidad, es del sexo femenino mayor de 60 años, ubicada en el área rural dispersa, con bajos niveles de escolaridad y escasa participación laboral y social. Conclusiones: Se evidenció que la mayor cantidad de preguntas que contiene el registro, corresponden al marco denominado de protección social básica, que busca conocer las características sociodemográficas, funcionales y de salud de la población...

ABSTRACT

Objetivo El propósito del presente artículo es presentar la magnitud de la discapacidad en el Departamento de Boyacá a través del establecimiento de sus distribuciones y relaciones de dependencia. Materiales y Métodos Estudio de corte transversal, donde el instrumento empleado fue el propuesto por el DANE para el registro, localización y caracterización de la discapacidad. Se realiza un análisis univariado y bivariado para las 89 preguntas del instrumento. Resultados Se encontró una mayor proporción de discapacidad en mujeres, con ubicación geográfica en el área rural dispersa, con bajos niveles educativos y de participación. Predomino la deficiencia del movimiento de cuerpo, manos, brazos y piernas, así como la dificultad para caminar, correr y saltar. Conclusión Se concluye que existe una dependencia estadísticamente significativa calculada a través del Chi cuadrado de Pearson, entre el fenómeno y los factores sociales, culturales y biológicos.

MATERIALS AND METHODS: This was a cross-sectional study which involved using the instrument proposed by DANE for recording, localizing and characterizing disability. The instrument's 89 questions were subjected to univariate and bivariate analysis. RESULTS: A greater prevalence of disability was found amongst females; it was geographically located in scattered rural areas and amongst those having low educational attainment and poor attendance at school. A deficiency in body, hand, arm and leg movements predominated, as did difficulty in walking, running and jumping. CONCLUSIONS: There was a statistically significant dependence (calculated by Pearson's chi-square test) between disability and social, cultural and biological factors.

ABSTRACT

La miel de abeja es un fluido dulce y viscoso producido por las abejas a partir del néctar y de la mielada. Se trata de una medicina antigua utilizada principalmente en el tratamiento de heridas, úlceras y quemaduras. En el presente trabajo se evaluó la actividad antimicrobiana de nueve mieles venezolanas, para lo cual se utilizaron cepas ATCC de Escherichia coli y Staphylococcus aureus. Las mieles analizadas se utilizaron concentradas y diluidas (1:2, 1:4, 1:8). Se observó la inhibición del crecimiento de S. aureus en la miel procedente del estado Trujillo sin diluir y en su dilución 1:2, mientras que no se observó inhibición del crecimiento de las cepas de E. coli, por lo que deberían probarse otras bacterias para futuras aplicaciones.


Honey is a sweet and viscous fluid produced by bees from nectar and honeydew. It is an ancient natural medicine mainly used to treat wounds, ulcers and burns. In this study we have assessed the antibacterial activity against E. coli and S. aureus of nine Venezuelan honeys participating in the national honey contest year 2005, compared with a control artificial honey. The honeys were tested at three dilutions (1:2, 1:4, 1:8) and undiluted. Only S. aureus was inhibited by one undiluted honey sample and at the 1:2 dilution. The honeys from the contest, were of low therapeutical value against E. coli and S. aureus, but other bacterias should be tested for further applications.
